Şizofreni kişinin ve ailesinin hayatını olumsuz yönde etkileyen ciddi bir toplum
sağlığı sorunudur. Etiyolojisi ve patogenezi tam çözülememiş olmasına rağmen
çevresel genetik faktörlerin şizofreni gelişiminde önemli olduğu belirlenmiştir.
Fibrinolitik sistem fibrin pıhtılarının azaltılması ve uzaklaştırılmasından sorumludur ve
bu olayda plazminojen önemli rol oynar. Plazminojen aktivatör inhibitör 1 (PAI-1)
plazminojen aktivasyonunu inhibe ederek proteolizi ve fibrinolizisi düzenler. PAI-1
seviyesindeki artış fibrinolitik aktivitenin artışına neden olmaktadır. PAI–1 düzeyindeki
artışın bu genin promotor bölgesinde -675’de yer alan 4G/5G polimorfizmine neden
olan 4G aleli ile ilişkilendirilmiştir. Kan koagülasyon faktörleri ve şizofreni arasında
muhtemel bir ilişki olmakla birlikte, akut ve kronik şizofreni hastalarında plazminojen
seviyesinin azaldığı ve bu hastalarda stres bağımlı trombozların PAI-1 tarafından regüle
edildiği gösterilmiştir.
Bu çalışma şizofreni hastalarında plazminojen aktivatör inhibitör tip-1 (PAI-1)
geni 4G/5G polimorfizm sıklığını belirleyerek bunun şizofreni gelişiminde genetik
yatkınlık yönünden ilişkili olup olmadığını araştırmak amacıyla planlandı.
Çalışmada; Şizofreni tanısı konan 100 hasta ve 50 sağlıklı bireyden alınan venöz
kan örneklerinden tuz-ekstraksiyon yöntemi kullanılarak elde edilen genomik DNA
analiz edildi. PCR metodu ile PAI-1 geni 4G/5G baz çiftleri ilgili primerler kullanılarak
çoğaltıldı. PCR ile çoğaltılan örnekler PAI-1 ürünü için %2’lik agaroz jel
elektroforezinde yürütülerek, CCD (Charge Coupled Device) kamera ile değerlendirildi.
4G primeri ile 139 bç de bant verenler 4G homozigot, 5G primeri ile 139 bç de bant
verenler 5G homozigot, her ikisinde de bant verenler 4G/5G heterozigot olarak
belirlendi.
Çalışma sonunda bulgularımız, PAI-1 geni genotipleri ile hastalık arasında
istatistiksel olarak anlamlılık olduğunu göstermekle birlikte, alel frekansı ile hastalık
arasında anlamlı bir ilişki bulunmadığını göstermektedir. Şizofreni oluşumu, çevresel etkenler ve diğer genlerin etkileşimi ile birlikte birçok genetik varyantın katkısının
olabileceği çok etkenli bir durumu açıklar. Bu nedenle, polimorfizm çalışmalarının
geniş popülasyonlarda daha anlamlı sonuçlar verdiği bilgisine dayanarak, hasta
sayısının artırılması ve gen bölgesi ile ilgili diğer polimorfizmlerin de çalışılması,
böylece çeşitli toplumlardan elde edilecek farklı bulguların şizofreni ile PAI-1 geni
4G/5G polimorfizmi etkileşimlerini açıklamak için gerekli olduğunu düşünmekteyiz.
Schizophrenia is a public health problem that affecting negatively both
individual and family. Despite its pathology and etiology is not fully understood,
environmental genetic factors are were determined to be important in the development
of schizophrenia. Fibrinolitic system is responsible for reducing and removing of
coagulum. Plasminogen activator inhibitor 1 (PAI-1) regulates proteolysis and
fibrinolysis by inhibition of plasminogen activation. PAI-1 level increase causes the
increase of fibrinolytic activity. Increase in the level of PAI-1 is associated with 4G
allele that causes 4G/5G polymorphism in the region of -675 promoter site. There is a
possible association between coagulation factors and schizophrenia, it was reported that
plasminogen levels were reduced in acute and chronic schizophrenia patients and stressdependent
thrombosis was regulated by PAI-1 in these patients.
This study was carried out to determine frequency of plasminogen activator
inhibitor type-1 (PAI-1) gene 4G/5G polymorphism and planned to investigate whether
this gene associated with genetic predisposition of development of schizophrenia with
genetic predisposition of schizophrenia or not.
In this study, genomic DNA samples obtained from 100 patients with
schizophrenia and 50 healthy subjects were analyzed. Genomic DNA was prepared
from peripheral blood using the salt-extraction method. The presence of the 4G/5G
polymorphism in the PAI-I gene was determined using the polymerase chain reaction
(PCR) method. PCR products were separated by 2% agarose gel electrophoresis and
visualized by a charge-coupled device (CCD) camera. Samples identified according to
whether they have 139 bp band or not. Samples that produced 139 bp band with 4G
primer were identified as homozygous 4G genotype, samples that produced 139 bp band
with 5G primer were identified as homozygous 5G genotype, samples that produced
139 bp band with both 4G and 5G primer were identified as heterozygous 4G5G
genotype. Our findings show not only there is a statistical significance between PAI-1 gene
genotypes and the disease but also show there is no association between allele frequency and the
disease. Environmental factors and other genes in the formation of schizophrenia with
lots of interaction may be due to the effect of a genetic variant can contribute a lot to
explain. As a result, polymorphism studies based on knowledge of the wide range of
patient populations, increasing the number of more meaningful results and other related
gene polymorphisms on the need for work to be obtained so that the different findings
in the various communities of PAI-1 gene 4G/5G polymorphism with schizophrenia
seem to be necessary to explain the interaction.
