Çalışmada medial malleol kırıklarında
kapalı redüksiyon perkütan tespit yönteminin ileriye dönük olarak klinik ve
radyolojik sonuçlarının değerlendirilmesi amaçlandı. Çalışma Mart 2011 ile Temmuz
2013 tarihleri arasında ayak bileği travması nedeniyle hastanemize başvurup medial
malleol kırığı tanısı alan 21’i erkek (%61.8) ve 13’ü kadın ( %38.2) toplam 34
hastayı kapsamaktadır. Hastaların ilk başvuruları sırasında yaş, cinsiyet, travma
nedeni, eşlik eden diğer travma ve hastalık bilgileri kaydedildi. Ayak bileklerine ön
arka, yan ve mortis grafileri çekilerek kırık tipleri belirlendi. Operasyon sırasında alt
ekstremiteye turnike uygulanmadı. Ortalama ameliyat süresi 39.2 (14-78) dakika
olarak saptandı. Ameliyat sonrasında hastalara elastik ayak bileği bandajı dışında
herhangi bir atel vb harici rijit tespit kullanılmadı. Ameliyat sonrası erken dönemde,
özellikle ayak bileği fleksiyon ve eksktansiyon hareketlerine hemen başlandı.
Ortalama takip süresi 17.9 (3-31) ay olarak saptandı. Ameliyattan sonra 1 ay, 3 ay ve
yıllık periyodik poliklinik kontrolleri sırasında hastalar klinik ve radyolojik açıdan
incelendi. Veriler Weber, AOFAS ve Frieburg protokollerine göre değerlendirildi.
Weber protokolüne göre yapılan değerlendirmede 21(%65.6) hasta mükemmel, 6
(%18.7) hasta iyi, 5(%15.7) hasta kötü olarak belirlendi. Olgular AOFAS
protokolüne göre değerlendirildiğinde 30(%93.8) hasta iyi, 2(%6.2) hasta kötü olarak
tespit edildi. Frieburg protokolüne göre değerlendirildiğinde ise 30(%93.8) hastada
iyi, 1(%3.1) hastada orta, 1(%3.1) hastada kötü sonuç elde edildi. Sonuç olarak
medial malleol kırığının cerrahi tedavisinde kapalı redüksiyon ve perkütan tespit
yöntemi hem minimal invaziv bir cerrahi teknik, hem de klinik ve radyolojik olarak
oldukça iyi sonuçları ile etkili ve ekonomik bir tedavi yöntemidir.

prospectively evaluation of the clinical and radiological results of closed reduction
and percutaneus fixation method in medial malleolus fractures was intended. The
study was completed with 21 male (61,8%) and 13 female (38,2%) 34 patients totally
who admitted to our hospital between March 2011 and Jully 2013 because of ankle
injury. Informations about age, sex, cause of trauma, comorbid trauma and other
diseases of patients were recorded during the initial application. Types of fractures
were determined by taking antero-posterior, lateral and mortise radiographs of ankle.
The average time of operations was reported as 39(14-78) minutes. During the
operation no tourniquet was applied to lower exremities and any splints were not
used except the ankle bandages after the operations. The average time of follow-up
was determined as 17,9 months (3-31 months). During the 1th month, 3th month and
annual periodic checks after the operation, the patients were examined clinically and
radyologically, the data were evaluated according to the Weber, AOFAS and
Freiburg protocols. In the assessment according to the Weber protocol 21(65,6%)
patients were recorded as excellent, 6(18,7%) patients were recorded as good,
5(15,7%) patients were reported as poor. In the assessment according to the AOFAS
protocol 30(93,8%) patients were reported as good, 2(6,2%) patients were reported
as poor. When the patients were evaluated according to the Freiburg protocol, the
results were recorded as good in 30(93,8%) patient, moderate in 1(3,1%) patient,
poor in 1(3,1%) patient. Consequently in the surgical treatment of medial malleolus
fractures, the closed reduction and percutaneus fixation method is both a minimal
invasive surgical technique as well as effective and economical treatment method
clinically and radiologically with quite good results
