Çalışmamızda abdominal histerektomi
uygulanan hastalarda deksketoprofen ve ketamin infüzyonunun preemptif analjezi
üzerine etkisini araştırmayı amaçladık. Bu çalışma, Kasım 2011 – Kasım 2012
tarihleri arasında, genel anestezi altında abdominal histerektomi operasyonu geçiren
18 – 60 yaş arasında, ASA I-II, 50 hasta üzerinde gerçekleştirildi. Çalışmaya dahil
edilen hastalar rastgele iki gruba ayrıldı: Grup 1’e operasyondan 30 dakika önce 50
mg deksketoprofen 100 ml serum fizyolojik içerisinde İV infüzyon ile verildi.
Anestezi indüksiyonu sonrası 0,5 mg/kg ketamin bolus yapılıp ardından 0,07
mg/kg/saat olacak şekilde infüzyon başlandı ve cilt kapatılana kadar devam edildi.
Cilt insizyonundan 15 dakika sonra 100 ml serum fizyolojik İV infüzyon ile verildi.
Ekstübasyondan hemen önce 3 mg morfin yapıldı. Grup 2’ye operasyondan 30
dakika önce 100 ml serum fizyolojik İV infüzyon ile verildi. Anestezi indüksiyonunu
takiben cilt insizyonundan 15 dakika sonra 50 mg deksketoprofen 100 ml serum
fizyolojik içerisinde İV infüzyon ile verildi. 0,5 mg/kg ketamin bolus yapılıp
ardından 0,07 mg/kg/saat olacak şekilde infüzyon başlandı ve cilt kapatıldıktan sonra
sonlandırıldı. Ekstübasyondan hemen önce 3 mg morfin yapıldı. Ameliyattan sonra
her iki gruba da hasta kontrollü analjezi (HKA) yöntemi ile morfin başlandı. Aldrete
derlenme skorunun 9 olduğu dakika 0. dakika kabul edilip 1, 4, 8, 12 ve 24. saatlerde
sistolik kan basıncı (SKB), diyastolik kan basıncı (DKB), ortalama kan basıncı
(OKB), kalp atım hızı (KAH) ve periferik oksijen satürasyonları (SpO2), RSS, VAS,
VRS, morfin tüketimi ve yan etkiler kaydedildi. Çalışmamızın sonucunda; 1 ve 4.
saatlerdeki VAS değerleri, 4 ve 8. saatlerdeki VRS değerleri, 4, 8, 12 ve 24
saatlerdeki morfin tüketim miktarları grup 1’de anlamlı olarak düşük saptandı. Sonuç
olarak; preoperatif dönemde başlanan deksketoprofen ve ketamin infüzyonunun
preemptif analjezi üzerine etkisi olduğu kanaatine varıldı.
In our study, we
aim to investigate the effect of dexketoprofen and ketamin infusion on preemptive
analgesia in patients undergoing abdominal hysterectomy under general anesthesia.
This study was conducted between November 2011 – November 2012, on 50 patients
of ASA I-II aged 18-60 years who were scheduled for elective hysterectomy
operations. Included patients were randomized into two groups: First group received
50 mg dexketoprofen in 100 ml physiologic saline solution via IV infusion 30
minutes before operation. After anesthetic induction 0,50 mg/kg ketamin bolus was
given and then infusion was started as 0,07 mg/kg/hour until skin closure. 15 minutes
after skin incision, 100 ml physiologic saline solution was administered. 3 mg
morphin IV was given just before extubation. Second group received 100 ml
physiologic saline solution via IV infusion 30 minutes before operation. After
anesthetic induction, 15 minutes after skin incision, 50 mg dexketoprofen in 100 ml
physiologic saline solution via IV infusion was administered. 0,50 mg/kg ketamin
bolus was given and then infusion was started as 0,07 mg/kg/hour. 3 mg morphin IV
was given just before extubation. After operation both groups received PCA
(patient controlled analgesia) with morphin. When Aldrete Discharge Score
become 9, the minute was recorded as 0th minute and at the 1st, 4th, 8th, 12th and
24th hours systolic blood pressure (SBP), diastolic blood pressure (DBP), mean
blood pressure (MBP), heart rate (HR) and peripheral oxygene saturations (SpO2),
RSS, VAS, VRS, morphin consumption and side effects were recorded. After
evaluations, 1st and 4th hour VAS values, 4th and 8th hour VRS values and 4th, 8th,
12th, 24th hour morphin consumption was found to be significantly lower in group I;
whereas there were no differences between other parameters. In conclusion,
preoperative dexketoprofen and ketamin infusion had effects on preemptive
analgesia.
