Alt ekstremite cerrahisi
geçirecek hastalarda spinal anestezide yüksek seviye ve yüksek dozlara ihtiyaç
olmadığı düşüncesiyle, önerilen klasik dozlardan daha düşük bupivakain dozlarının
yeterli olabileceği, doza bağlı komplikasyonların da azalabileceği hipoteziyle;
çalışmamızda 5 mg ve 7.5 mg hiperbarik bupivakain kullanarak tek taraflı spinal
anestezide düşük dozların cerrahi anestezi ve hemodinamiye etkisinin araştırılması
amaçlandı. Çalışma, Eskişehir Osmangazi Üniversitesi Tıp Fakültesi
Ameliyathanesinde ASA I-II grubu 18-60 yaş arası tek taraflı artroskopik diz
cerrahisi geçirecek 40 hasta ile yapıldı. Olgular 20 kişilik 2 gruba ayrılarak, Grup I’e
5 mg hiperbarik bupivakain, Grup II’ye 7.5 mg hiperbarik bupivakain uygulandı.
Uygulama sonrası hastalar lateral dekübit pozisyonda 10 dk bekletildikten sonra
supin pozisyona çevrildi. Hastaların hemodinamik parametreleri başlangıçta ve
spinal anestezi sonrası 1., 5., 10., 15., 20., 25., 30. dakikalarda, ve blok çözülene
kadar 15 dakikada bir kaydedildi. Hastaların duysal blok seviyesi, motor blok
seviyesi, duysal blok iki seviye gerileme zamanı, ayılma biriminde kalış süresi, hasta
memnuniyeti, ilk 24 saatte miksiyon zamanı kaydedildi. Hipotansiyon ve bradikardi
açısından gruplar arasında anlamlı fark bulunamadı. Grup I’de 1 hastada, Grup II’de
2 hastada hipotansiyon gelişti. Grup I’de 3 hastada, Grup II’de ise 5 hastada
bradikardi gelişti. Her iki grupta da ilk 90 dk ek doz ihtiyacı olmadı. Operasyon sonu
motor blok her iki grupta da 20 hastadan 13’ünde 0 olarak bulundu. Sonuç olarak;
gruplar arasında anlamlı fark bulunamamakla birlikte, 5 mg hiperbarik bupivakain
uygulanarak potansiyel risklerin daha az olması ve tüm hastalarda en az 90 dk cerrahi
anestezi sağlaması nedeniyle 90 dk’yı geçmeyen alt ekstremite cerrahisinde tavsiye
edilebilir doz olduğu kararına varıldı.
With the idea that
there is no need for higher doses and high level; the recommended dose of
bupivacaine lower than conventional doses may be adequate, with the hypothesis that
a dose-dependent decrease of complications; we aimed to investigate spinal
anesthesia and hemodynamic effects of lower doses of surgical anesthesia in patients
undergoing lower extremity surgery with spinal anesthesia, using 5 mg and 7.5 mg
hyperbaric bupivacaine. The study was performed with 40 patients, at Eskisehir
Osmangazi University Faculty of Medicine in the operating room, ASA I-II patients
aged 18-60 undergoing unilateral knee arthroscopy. The patients were divided into 2
groups, 20 patients with 5 mg hyperbaric bupivacaine in Group I, 20 patients with
7.5 mg hyperbaric bupivacaine in Group II was. After application patients holded in
the lateral decubitus position 10 minutes and then turned in the supine position.
Hemodynamic parameters at baseline and after spinal anesthesia 1th, 5th, 10th, 15th,
20th, 25th, 30th minutes, and the blocks were recorded every 15 minutes until
dissolved. Patients sensory block level, motor block level, two levels of sensory
block regression time, sobering unit length of stay, patient satisfaction, voiding time
were recorded in the first 24 hours. Hypotension and bradycardia showed no
significant difference between the groups in terms. In one patient in Group I, and 2
patients in Group II developed hypotension. In 3 patients in Group I, and in 5
patienys in Group II developed bradycardia. In both groups, in the first 90 minutes
patients did not require additional doses. Motor block were found 0 at the end of the
operation at 13 patients in both group. As a result; between the two groups there
were no difference significantly, however low dose hyperbaric bupivacaine should be
chosen at lower extremity surgery because 5 mg hyperbaric bupivacaine had less
potential risks and provides at least 90 min of surgical anesthesia in all patients.
