Bu çalışmanın amacı, sıçanlarda tek akciğer ventilasyonu sonrasında
oluşan akciğer hasarı üzerine melatoninin koruyucu rolünü araştırmaktır.
Çalışma için 20 adet Sprague Dawley cinsi sıçan randomize olarak, eşit
sayıda (n=10) iki gruba ayrıldı. Kontrol grubuna 60 dakika süreyle tek
akciğer ventilasyonu (TAV) devamında 30 dakika süreyle çift akciğer
ventilasyonu (ÇAV) uygulandı. Melatonin grubundaki sıçanlara deneye
başlamadan 10 dakika önce 10 mg/kg dozunda intraperitoneal melatonin
verildi ve aynı ventilasyon protokolü uygulandı. Kontrol ve çalışma
gruplarından TAV ve ÇAV sonunda biyokimyasal analiz ve histopatolojik
inceleme için akciğerden doku örnekleri alındı. Biyokimyasal analizde doku
superoksid dismutaz (SOD), malondialdehit (MDA) ve tümör nekroz faktör
alfa (TNF-α) düzeyleri ölçüldü. Histopatolojik incelemede dokular
hemotoksilen eosin ile boyandı ve akciğerlerde oluşan hasar alveolar
konjesyon, intraalveoler kanama, lökosit ve lenfosit infiltrasyonu varlığı ve
miktarına göre skorlandı. TAV ve ÇAV sonunda çalışma grubu MDA ve TNF-
α düzeylerinde kontrol grubuna göre istatistiksel olarak anlamlı düşüş olduğu
görüldü. Yine çalışma grubunda, SOD düzeylerinde ÇAV sonunda yükselme
izlendi, bu yükselme özellikle TAV sonunda istatistiksel olarak anlamlı
bulundu. Histopatolojik incelemede melatonin verilen grupta oluşan doku
hasarının daha az olduğu gözlendi. Sonuç olarak mevcut bu bulgular
ışığında melatoninin TAV sonrasında oluşan akciğer hasarına karşı dokuyu
koruyucu rolü vardır.
The aim of this study was to invastigate the protective effect of melatonin on
lung injury after one lung ventilation in rats. 20 Sprague Dawley rats were
randomly divided into two groups in equal numbers, for the study (n=10). In
control group we performed one lung ventilation for 60 minutes, following 30
minutes double lung ventilation. In melatonin group same ventilation
procedure was performed, additionally 10 mg/kg intraperitoneal melatonin
was administered 10 minutes before the start of the experiment. Tissue
samples of the lung from control and study group were taken for biochemical
analyse and histopathological evaluation at the end of one and double lung
ventilation. Superoxide dismutase (SOD), malondialdehyde (MDA) and
Tumor Necrosis Factor Alpha (TNF-α) levels were determined biochemically.
Tissue samples were stained with hematoxylin-eosin for histopathological
evaluation and were scored acording to the alveolar congestion,
polymorphonuclear leukocytes infiltration, lymphocyte infiltration and intra
alveolar hemorrhage amount. At the end of one and double lung ventilation,
statistically significant decrease in MDA and TNF-α levels were seen in the
study group. Also SOD values were increased in the study group at the end
of double lung ventilation and the increase in SOD levels were statistically
significant at the end of one lung ventilation. In histopathological evaluation,
tissue injury in melatonin group was observed lesser than control group.
With all these findings melatonin has a protective role on lung injury after
one lung ventilation.
