Hipertansiyon, bugün hem
gelişmiş, hem de gelişmekte olan ülkelerde toplumun büyük bir kısmını etkileyen ve
öneminin giderek artmasının yanı sıra hastaların pek çoğunun tedavisinin yetersiz
kaldığı bir hastalıktır. Hipertansiyonun değiştirilebilir ve önlenebilir olması
nedeniyle, gerek halk sağlığı çalışmalarına ışık tutması, gerekse birincil koruyucu
önlemlerin alınabilmesi amacıyla hipertansiyon prevalansının yanı sıra bir yıllık
hipertansiyon insidansının ve risk faktörlerinin belirlenmesi için çalışma yapıldı.
Çalışma, Kasım 2011- Ocak 2013 tarihleri arasında Mahmudiye İlçe Merkezinde, 18
yaş ve üzeri tüm erişkinlerde bir yıl ara ile iki aşamada yapılan prospektif bir
araştırmadır. Birinci aşamada Mahmudiye’de yaşayan erişkinler evlerinde tek tek
ziyaret edilerek, anket uygulaması ve kan basıncı ve antropometrik ölçümleri yapıldı.
Çalışmanın ikinci aşaması, hipertansiyon insidansının saptanması için bir yıl aradan
sonra gerçekleştirildi. Birinci aşama çalışmada kan basıncı normal sınırlar içinde
saptanan ve antihipertansif tedavi almayanlar tekrar hanelerinde tek tek ziyaret
edilerek anket uygulaması ve ölçümlerin tekrarı yapıldı. Mahmudiye’de birinci
aşama çalışmada 1553 kişiye ulaşıldı ve hipertansiyon prevalans hızı %33.6 olarak
bulundu. Çalışmanın ikinci aşamasında ise birinci aşamada kan basıncı normal
sınırlar içinde saptanan ve antihipertansif tedavi almayan 1031 kişinin 564
(%54.7)’üne ulaşıldı. Çalışma grubunda hipertansiyon insidans hızı %8.7, yaşa
standardize hipertansiyon insidans hızı ise %8.8 olarak bulundu. Hipertansiyon
insidans hızı ile ilişkili faktörler olarak yaş, fizik aktivite düzeyi ve abdominal
obezite varlığı bulundu. Hipertansiyondan korunma ve risk faktörlerinin kan basıncı
üzerine etkisini değerlendirmek için zamanında ve doğru yapılan hipertansiyon ve
risk faktörleri sürveyans çalışmalarının gerekli olduğu sonucuna varıldı.
Hypertension, both in developed and developing countries, affects large part of
the community, and today, the importance of the disease is increasing due to the
failure of treatment of most of the patients. Due to the preventable status of
hypertension, the present study was conducted to determine the prevalence as well as
the one-year incidence and risk factors of hypertension in order to take primary
preventive measures and to light the way for future public health studies. The study
is a prospective study conducted on all adults aged 18 years or older living in
Mahmudiye district center, in two stages with an interval of one year between
November 2011 and January 2013. In the first stage, all adults living in Mahmudiye
were visited at their home and a questionnaire form implemented, and blood pressure
and anthropometric measurements were obtained. The second stage of the study was
conducted after one year, in order to evaluate the incidence of hypertension. The
adults having normal blood pressure in the first stage and not using antihypertensive
medication were visited again at their home and the questionnaire form and the
measurements were repeated. A total of 1553 individuals were visited during the first
stage and the prevalence rate of hypertension was found to be 33.6%. During the
second stage, 564 (54.7%) of the 1031 individuals who had normal blood pressure in
the first stage and not using antihypertensive medication were visited at their home.
The incidence rate of hypertension was 8.7% and age-standardized incidence rate of
hypertension was found to be 8.8%. Age, physical activity level and abdominal
obesity were found to be significantly associated with incidence rate of hypertension.
It is concluded that, accurate hypertension and risk factors surveillance studies are
necessary in order to evaluate the effects of risk factors of hypertension and
preventing from hypertension.
