Bu çalışmada
böbrek taşlarının in vivo ortamda Renkli Doppler Ultrason ile elde edilen Twinkling
Artefaktı özellikleri ile kimyasal bileşimlerinin tahmini yapılmaya, Twinkling
Artefaktın oluşmasında ve şiddetinde etkili olabilecek faktörler ortaya konmaya
çalışılmıştır. Bu amaçla Ağustos 2011 – Ağustos 2012 tarihleri arasında Eskişehir
Osmangazi Üniversitesi Tıp Fakültesi Üroloji Bölümü‟ne başvuran, yapılan tetkikler
sonucunda 2 cm‟den büyük böbrek taşı saptanan ve daha önce taşlı böbreğe hiçbir
ürolojik tedavi uygulanmayan, perkütan nefrolitotomi ameliyatı için uygun
endikasyonlara sahip 101 hasta çalışmaya dahil edildi. Her bir hastaya Renkli
Doppler Ultrasonografi yapılarak taşlar Twinkling Artefakatları açısından
değerlendirildi. Tüm taşlar Twinkling Artefakatları‟na göre Grade 0,1 ve 2 olmak
üzere 3 gruba ayrıldı. Tüm hastalara perkütan nefrolitotomi operasyonu yapıldı.
Ġşlem sonrası elde edilen tüm taşlar analize gönderildi. Analiz sonuçlarına göre 66
(%65.3) tane kalsiyum oksalat taşı saptandı. Kalsiyum oksalat mono hidrat ise saf
veya mikst halde en çok saptanan (%78,2) kimyasal tür olduğu gözlendi. Taşın
boyutu, kimyasal yapıları, böbrekte hidronefroz varlığı, hastanın vücut kitle indeksi
ve operasyon sonrası taşsızlık durumu ile Twinkling artefakt arasındaki ilişki
istatiksel olarak değerlendirildi. Twinkling artefaktın taşın kimyasal yapısını tahmin
etmeye yardımcı olmadığı, hastanın vücut kitle indeksi ve taşın boyutu arttıkça
Twinkling artefaktın şiddetlendiği saptandı. Bu bulgular sonucunda kilolu hastalar ile
taş yükü fazla olan hastalarda Twinkling artefaktın güvenililirliğinin yeterli
olamayacağı ilk kez gösterilmiş oldu. Yapılacak geniş kapsamlı in vivo çalışmalar
ile Twinkling artefaktın üriner sistem taş hastalığı tanısındaki yerinin daha da
netleşeceğini düşünmekteyiz.
This study
aimed to predict the chemical composition of the kidney stones according to their
twinkling artifact features obtained by in vivo Color Doppler Ultrasonography and to
determine the factors potentially affecting the formation and severity of Twinkling
artifact. For this purpose, a total of 101 patients admitted to the Department of
Urology, Eskisehir Osmangazi University between August 2011 and August 2012 in
whom a kidney stone of 2 mm or over was identified and no previous urological
surgery was performed as well as who had indication for percutaneous
nephrolithotomy were included in the study. All patients were evaluated for
Twinkling artifact by using Color Doppler Ultrasonography. According to the
Twinkling artifact, all kidney stones were divided into 3 groups: grade 0, 1 and 2.
Percutaneous nephrolithotomy was performed in all patients. Immediately after the
procedure, all stones were referred for chemical analysis. According to the results of
chemical analysis, 66 (65.3%) were calcium oxalate stones. The most common pure
or mixed chemical ingredient was calcium oxalate monohydrate (78.2%). The
relationship of Twinkling artifact with the size and chemical composition of the
stone, presence of hydronephrosis, body mass index (BMI) and stone-free rate after
the procedure was statistically evaluated. It was found in this study that twinkling
artifact is not able to help to predict the chemical composition of the stone and that
severity of Twinkling artifact increases with increasing BMI and increasing size of
the stone. In conclusion, the present study is the first showing that Twinkling artifact
may not be reliable in overweight patients and in patients with high burden of stone.
Future comprehensive in vivo studies will clarify the role of twinkling artifact in the
diagnosis of urinary tract stone disease.
