D vitamini immun
regulasyon, solunumsal hastalıklar ve kanser gelişiminde anahtar rol oynamaktadır.
Bu tez çalışmasının amacı alt solunum yolu enfeksiyonu (ASYE) olan hastalarda, D
vitamininin prokalsitonin ve diğer inflamasyon belirteçleri ile ilişkisinin
incelenmesidir. Çalışmaya 90 ASYE tanılı hasta ve 50 sağlıklı gönüllü dahil edildi.
Çalışma grupları; kronik obstruktif akciğer hastalığı akut alevlenmesi (KOAH AA)
(n=30), toplum kökenli pnömoni (TKP) (n=45), KOAH+TKP (n=15) ve kontrol
grubu (n=50) idi. Çalışmamızda katılımcıların D vitamini, prokalsitonin (PCT),
yüksek duyarlılıklı C-reaktif protein (hs-CRP), parathormon, lökosit, nötrofil,
lenfosit sayıları, hemoglobin miktarları, miyeloperoksidaz (MPO) ve kitotriozidaz
aktivitelerini belirledik. D vitamini seviyesi hasta gruplarında kontrol grubundan
düşük iken; MPO, PCT, hs-CRP ve lökosit sayısı ise yüksekti (p=0.000). D vitamini
seviyeleri MPO, PCT, hs-CRP ve lökosit sayısı ile negatif koreleydi (p=0.000).
Ayrıca lökosit ve nötrofil sayıları, D vitamini ile negatif korelasyona sahipken, MPO
ve PCT ile pozitif korelasyona sahipti (p=0.000). D vitamini, MPO, PCT ve bu
testlerin ikili kullanımı pnömoni tanısında yüksek duyarlılık ve özgüllüğe sahipti.
Çizilen Receiver Operating Characteristic (ROC) eğrisinde; D vitamini için eğri
altında kalan alan (EAA) 0.954, sensitivite 81.5, spesifite 98, MPO için EAA 0.975,
sensitivite 100, spesifite 92, PCT için EAA 0.933, sensitivite 85.2, spesifite 98
bulundu. MPO ve D vitamini birlikte değerlendirildiğinde EAA 0.993’e yükselirken,
sensitivite 96.3, spesifite 98 bulundu. MPO ve PCT için ise EAA 0.990’e
yükselirken, sensitivite 96.3, spesifite 94 bulundu. D vitamini eksikliği KOAH ve
pnömoni hastalarında oldukça yaygın olarak görülmektedir. Bu nedenle, alt solunum
yolu enfeksiyonu olan hastalarda D vitamini düzeyi bakılmasını ve gerekli
durumlarda tedaviye eklenmesini önermekteyiz.
Vitamin D may be a key player in
immune regulation, respiratory diseases and in the development of cancer. The aim
of this study was to investigate the role of vitamin D with procalcitonin and other
inflammatory parameters in patients with lower respiratory tract infections (LRTI).
90 patients with LRTI and 50 healthy volunteers were included. The study groups
were; patients with chronic obstructive pulmonary disease acute execarbation (COPD
AE) (n=30), community acquired pneumonia (CAP) (n=45), COPD+CAP (n=15)
and control group (n=50). We evaluated vitamin D, procalcitonin (PCT), high
sensitivity C-reactive protein (hs-CRP), parathormone, leucocyte, neutrophil,
lymphocyte, hemoglobin counts, myeloperoxidase (MPO) ve chitotriosidase
activities. Vitamin D levels were lower and MPO, PCT, hs-CRP and leucocyte count
were higher in patient groups than control (p=0.000). Vitamin D levels had a
negative correlation with MPO, PCT, hs-CRP and leucocyte count (p=0.000).
Leucocyte and neutrophil counts had a negative correlation with vitamin D and a
positive correlation with MPO and PCT (p=0.000). Vitamin D, MPO, PCT had a
considerably sensitivity and specifity for the diagnosis of pneumonia. The Receiver
Operating Characteristic (ROC) analysis was done. The area under the curve (AUC)
for vitamin D was 0.954, sensitivity 81.5, 98. AUC for MPO was 0.975, sensitivity
and specificity were 100, 92. AUC for PCT was 0.933, sensitivity and specificity
were 85.2, 98. AUC for MPO plus vitamin D was 0.993, sensitivity and specificity
were 96.3, 98. AUC for MPO plus PCT was 0.990, sensitivity and specificity were
96.3, 94. According to our results, vitamin D deficiency is frequent in patients with
LRTI. Therefore, we recommend to measure vitamin D levels in these patients and to
add vitamin D supplementation to treatment if necessary.
