Wingless (Wnt)/β-katenin sinyal iletimi ve OPG/RANKL sistemi kemik
metabolizmasını etkileyen en önemli hücresel yolaklardır. Çalışmamızın amacı,
postmenopozal osteoporozlu (PMO) hastalarda serum OPG, sRANKL, sklerostin,
DKK-1 düzeylerini ölçmek, biyolojik bir belirteç olarak kullanılıp
kullanılamayacağını belirlemekti. Çalışmaya 61 PMO tanılı hasta ve 23 PMO
olmayan kadın dahil edildi. PMO ’lu hastalar da kullandıkları osteoporoz ilaçlarına
göre G1 (ibandronik asit), G2 (teriparatid) G3 (yeni tanı konmuş hastalar) olarak
gruplandırıldı. Serum sklerostin (p<0.001), DKK-1 (p=0.002) ve sRANKL (p=0.013)
düzeyleri PMO ’lu grupta anlamlı yüksek bulundu. G1 ve G2 gruplarıyla G3 arasında
ise DKK-1 ve sklerostin seviyelerinde anlamlı bir farklılık bulunamadı (p>0.05).
sRANKL düzeyleri G3 grubunda kontrollere göre yüksek (p<0.05), OPG düzeyleri
ise düşük (p<0.05) bulundu. Ayrıca serum OPG düzeyleri G1 ve G2 grubunda G3
grubuna göre anlamlı yüksek bulundu (p<0.05). G3 grubunda hem sklerostin (r=-
0,584 p=0.002) hem de DKK-1 (r=-0.408, p=0.043) düzeyleriyle serum östrodiol
düzeyleri arasında; serum sklerostin düzeyleriyle de (r=-0.678, p<0.001) serum
parathormon (PTH) düzeyleri arasında ise negatif yönlü bir ilişki bulundu. Kemik
dansitesi ile sadece OPG düzeyleri arasında anlamlı pozitif (r=0.457, p=0.022), kırık
riski ile sadece sklerostin düzeyleri arasında pozitif bir ilişki saptandı (r=0,305,
p=0,018). Bu sonuçlar Wnt/β-katenin ve OPG/RANKL yolağının OP patogenezinde
rol oynayabileceğini, özellikle bisfosfonat tedavisinin OPG/RANKL yolağı üzerinde
etkili olabileceğini; östrojen ve PTH’nın kemik kütlesi üzerindeki etkilerinde ve kırık
oluşum riskinde sklerostinin ilave bir serum belirteci olabileceğini düşündürmüştür.
The wingless (Wnt)/β-catenin signaling and
OPG/RANKL systems are the most important cellular pathways affecting bone
metabolism. The purpose of the present study is to measure the levels of serum OPG,
sRANKL, sclerostin, and DKK-1 in postmenopausal patients with osteoporosis
(PMO) and to assess whether these values can be used as biological markers. Sixtyone
patients diagnosed with PMO and 23 women without PMO were included in the
study. Patients with PMO were divided into three groups based on the medications
they use for osteoporosis: G1 (ibandronic acid), G2 (teriparatide) and G3 (drugnaïve).
Serum sclerostin (p<0.001), DKK-1 (p=0.002) and sRANKL (p=0.013) levels
were significantly higher in the group with OP. In the G3 group, sRANKL levels
were higher (p<0.05) and OPG levels were lower (p<0.05) compared to the control
group. In addition, serum OPG levels in the G1 and G2 groups were significantly
higher than the levels measured in the G3 group (p<0.05). Negative correlations were
observed between serum estradiol levels and both sclerostin (r=-0.584 p=0.002) and
DKK-1 (r=-0.408, p=0.043) levels, as well as between serum sclerostin (r=-0.678,
p<0.001) levels and serum parathormone (PTH) levels in the G3 group. Bone density
had a significant positive correlation only with OPG levels (r=0.457, p=0.022), and
fracture risk had a positive correlation only with sclerostin levels (r=0.305, p=0.018).
These findings suggest that Wnt/β-catenin and OPG/RANKL pathways may play
roles in OP pathogenesis and bisphosphonate treatment particularly may be effective
on OPG/RANKL pathway; sclerostin levels can represent an additional serum
marker to predict the effects of estrogen and PTH on bone mass, as well as to predict
the risk of fracture.
