Hematolojik
maligniteli hastalar, gerek hastalık özellikleri gerekse tedavileri ile ilişkili birçok
nedene bağlı olarak enfeksiyonlara yatkınlık oluştururlar. Ampirik antimikrobiyal
tedaviye rağmen, tanımlanamayan ya da etkin tedavi edilemeyen enfeksiyonlar,
nötropenisi olan hastalarda ölümün sık nedenlerindendir. Bu çalışmanın amacı
hematolojik maligniteli febril nötropenik olgularda mortaliteyi etkileyen risk
faktörlerini belirlemektir. Çalışma Kasım 2011-Haziran 2013 tarihleri arasında
Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Hematoloji Kliniğinde takip edilen
18 yaş ve üzeri hematolojik maligniteli febril nötropenik olgular üzerinde yapıldı.
Hastalar febril nötropeniye girdikten sonra taburcu veya exitus oluncaya dek günlük
olarak takip edildi. Hematopoetik kök hücre nakli yapılan hastalar ve solid
malignensili hastalar çalışmaya dahil edilmedi. Her bir hasta için tek bir febril
nötropeni atağı çalışmaya dahil edildi. Çalışmada elde edilen bulgular
değerlendirilirken, istatistiksel analizler için SPSS 15.0 istatistik programı kullanıldı.
Toplam 320 febril nötropenik atak incelendi. İzlenen 320 olgunun 48'inde (% 15)
mortalite gelişti. Hematolojik maligniteli febril nötropenik olgulardaki mortalite ile
ilgili risk faktörlerinin lojistik regresyon analizi sonucunda; çalışmamıza giren elli
beş yaş üstü hematolojik malignitesi olan, febril nötropeni tedavisi izlemi sırasında
antibiyotik modifikasyonu gerektiren olgularda, tedavi başlangıcında hipotansiyon ve
akut böbrek yetmezliği bulguları olan olgularda, ApacheII skoru yedinin altında olan
olgularda, tedavi başlangıcında ve sonunda ölçülen CRP değerleri yüksek saptanan
olgularda ve gaitada VRE kolonizasyonu olan olgularda mortalite riskinin arttığı
saptanmıştır.
The objective of the study
was to identify risk factors of mortality in febrile neutropenic patients with
hematologic malignancies. In this study, febrile neutropenic attacks seen in 320
patients who were hospitalized at Eskişehir Osmangazi Univercity Medicine Faculty
Hematology Clinic between November 2011 and August 2013 were prospectively
evaluated. The patients with hematologic stem cell transplantion were excluded in
the study. Most patients had acute myelogenous leukemia (AML) (186;58.1%),
followed by non-Hodgkin’s lymphoma (NHL) (27;8.4%), myelodysplastic syndrome
(MDS) (20;6.2%), chronic lymphocytic leukemia (CLL) (12;3.7%), multiple
myeloma (MM) (9;2.8%) and chronic myelogenous leukemia (CML) (12;3.7%). At
the end of the study 25/155(%47.8) of female and 23/165(%52.27) male patients
died. Total mortality rate is %15(48/320). Mean age of the dead patients was 57.6 ±
18 years. According to the Multinational Association for Supportive Care in Cancer
(MASCC) criteria, 151 of the patients were in the low-risk group, whereas 121
patients were in the high-risk group. Of the 320 neutropenic attacks, 156 were
clinically and microbiologically defined infection and 164 were fever of unknown
origin. Age, APACHE II score, duration of neutropenic period, first and last count of
CRP values were significantly higher in dead patients than alive ones (p<0,01),
whereas the MASCC score were significantly lower (p<0,001). The mortality risk in
patients with MM was 12.81 (p<0,001); in patients with NHL was 3,26 (p<0,05); and
in patients with AML 2,43 times higher (p<0,05). Clinical and microbiological
approaches should be made carefully in order to detect the diagnosis of infection in
patients with febrile neutropenia. Early initiation of empiric antimicrobial treatment,
reducing the duration of neutropenic period and taking proper isolation preventions
seem to be important in reducing mortality.
