Akut iskemik inmede, semptom başlangıcı sonrası ilk 3-4,5
saat içinde uygulanan İV rt-PA tedavisinin etkinliği gösterilmiş olup, semptomatik
intraserebral kanamalar (SİSK), bu tedavinin en korkulan komplikasyonudur. Bu
çalışmadaki amacımız; akut orta serebral arter iskemik inmesi ile başvuran, semptom
sonrası ilk 3-4,5 saat içerisinde İV rt-PA tedavisi verilen hastalarda, intraserebral
kanama için risk oluşturabilecek olan faktörleri ve bu kanamaların klinik üzerine
etkilerini değerlendirmek, bunun için alınabilecek önlemleri belirlemektir. Çalışmaya
02.11.2008-29.07.2013 tarihleri arasında Eskişehir Osmangazi Üniversitesi Tıp
Fakültesi Acil Servisi’ne semptom sonrası ilk 4,5 saat içerisine başvuran, orta
serebral arter iskemik inmesi tanısı konulan, rt-PA tedavisi için kontrendikasyonu
olmayıp, tedavi uygulanan 18 yaş üzeri 100’ü kadın (% 45,2), 121’i erkek (% 54,8)
olan, toplam 221 hasta dahil edildi. SİSK için SITS MOST’un tanımı kullanıldı. 221
hastanın 50’sinde (%22,6) asemptomatik intraserebral kanama (AİSK) gözlenirken,
9’unda (%4,1) SİSK gözlendi. SİSK’sı olan hastalarda, olmayanlara göre, başvuru
anındaki ortalama ASPECT skoru, 5. gün ölçülen glukoz seviyeleri, 24 saat sonraki
NIHSS puanı, 3. Ay mRS ortalaması ve mortalite oranları istatiksel olarak anlamlı
yüksekti. Önceki çalışmalara göre beklenen sınırlar arasında saptanan SİSK’nın, kötü
prognostik özellikte olduğu görüldü. Tedavi öncesi serebral BT’deki erken iskemik
değişikikler ve ölçülen kan glukoz yüksekliği; SİSK için risk faktörü olarak
saptanmış olup, bunlara yönelik iyi değerlendirme ve müdahalelerin kanama oranını
azaltabileceği sonucuna varılmıştır.
The efficacy of İV rt-PA
administered between 3 and 4.5 hours after the onset of symptoms in acute ischemic
stroke has been demonstrated but symptomatic intracerebral hemorrhage (SIH) is the
most feared complication of this treatment. We aimed to identify the risk factors for
intracerebral hemorrhages in acute middle cerebral artery stroke patients that treated
with İV rt-PA between 3 and 4.5 hours after the onset of symptoms and the effects of
these hemorrhages on clinical state, also to determine the countermeasures for them.
221 patients with acute middle cerebral artery stroke who admitted to Eskişehir
Osmangazi University Department of Emergency Medicine within 4,5 hours of
symptom onset between 2 November 2008 and 29 July 2013 enrolled in the study.
They were over 18 years old and had no contraindication for rt-PA medication. 100
of 221 patients were female (% 45,2) and 121 of them were male (% 54,8).
Defination of SITS MOST for SIH was used. 50 of 221 patients had asymptomatic
intracerebral hemorrhage (%22,6) and 9 of 221 had SIH (%4,1). The average
ASPECT score on admission, the NIHSS score of patients after 24 hours from
baseline assessment, the mean mRS score at 3 months, mortality and the glucose
levels measured after 5 day from the treatment were significantly higher in patients
who showed SIH than in patients without SIH. According to previous studies, the
rate of SIH was within the expected range. It was seen that SIH was a poor
prognostic factor. In our study, early ischemic changes on serebral CT and high
glucose levels were independent risk factors for SIH. A good assessment of these
factors and interventions for them can reduce the SIH rate.
