Bu çalışmanın amacı, kliğimizde asetabulum kırığı tanısıyla cerrahi olarak tedavi edilen hastaların klinik ve radyolojik sonuçlarının değerlendirilmesidir. Çalışmaya en az 2 yıl izlemi olan 115 hasta alındı. Kırıkların tümü elde edilen direk grafi ve bilgisayarlı tomografi bulguları ile Judet-Letournel sınıflamasına göre sınıflandırıldı. Radyolojik değerlendirme Matta’nın Radyolojik Kriterlerine göre yapıldı. Klinik sonuçlar Modifiye Merle D’Aubigne Değerlendirme ölçeği’ne göre değerlendirildi. Ortalama yaş 39.3 (14-80) ve ortalama takip süresi
6.8 yıl (2-18) idi. Hastaların 95’i erkek (%82,6), 20’si kadındı (%17,4). Hastaların 67’sinde sağ (%58,3), 48’inde sol (%41.7) asetabulum kırığı mevcuttu. Kırık oluşma nedeni 60 hastada AİTK (% 66,2), 41 hastada düşme ve yüksekten düşme (%35,7), 14 hastada ADTK (%12,2) idi.11 hastada preop peroneal araz mevcuttu (%9.6). hastada postop peroneal araz (%5,2) mevcut olup 5 i takiplerde tamamen düzeldi. 13 hastada posterior dislokasyon (%11,3) 6 hastada santral dislokasyon (%5,2) 22
hastada posttravmatik artroz (%19,1) 4 hastada klinik bulgu veren DVT (%3,5) 6
debridman gerektiren CAE (%5,2) 3 klinik bulgu veren PTE (%2,6) saptandı. 8 hastada klinik başarısızlık nedenli revizyon cerrahisi gerekli oldu.(%6,9) (6 TKP, 2 Artrodez) 8 hastada Brooker Evre III-IV heterotropik ossifikasyon bulgusu mevcuttu (%7,0). Matta’nın Radyolojik Kriterleri’ne göre redüksiyon, 64 hastada anatomik (%55.7), 38 hastada orta (%33), 13 hastada kötü (%11,3) olarak değerlemdirildi. Modifiye Merle D’Aubigne Değerlendirme Ölçeği’ne göre ; 53 hastada mükemmel (%46,1), 37 hastada iyi (%32,2), 14 hastada orta (%12,2) 11 hastada kötü (%9,6) klinik sonuç elde edildi.Çalışmamızda ; redüksiyon kalitesi, yaş (> 45 yaş) ve kırık tipi (T tip) ile klinik sonuçları kuvvetli ilişki olduğu görüldü.İyi ve mükemmel sonuç elde edebilmenin, optimal şartlarda uygulanan bir cerrahi, stabil bir internal tespit ve tam anatomik redüksiyon ile mümkün olduğu görüldü.
The objective of this study was evaluation of the clinical and radiological outcomes of the patients who had surgically treated acetabuar fractures in our clinic. 115 patients with at least 2 years follow-up were included in this study. Mean age of the patients was 39.3 years (14-80) and mean follow-up time was 6.8 years (2-18). 95 of the patients were male (%82.6) and 20 were female (%17.4). Fractures was detected in right acetabulum in 67 patients (%58.3), left acetabulum in 48 patients (%41.7). All petients classified with Letournel Judeth classification. Radiological evaluation was made based on Matta’s Radiological Criteria. Clinical outcomes were evaluated according to Modified Merle D’Aubigne Scale . All preoperative and postoperative complications was recorded. Mean age was 39,3(14-80) and the mean follow up time was 6.8 years (2-18). 11 patients had preoperative peroneal nerve symptoms (9.6%) . 13 patients had posterior dislocation (11.3%) , 6 patients had
central dislocation (5.2%) and 22 patients had posttraumatic arthrosis (19.1%). 4 patients had deep vein thrombosis (3.5%) 6 patients had surgical site infections (5.2%).8 patients had signs of stage III-IV heterotropik ossification (7.0%). The quality of the reduction, according to Matta's Radiological Criteria was recorded anatomical in 64 patients (55.7%), imperfect in 38 patients (33%) and poor in 13 patients (11.3%). According to Modified Merle D’Aubigné scale; 53 patients had excellent (46.1%), 37 patients had good (32.2%), 14 patients had moderate (% 12.2) 11 patients worse (9.6%) clinical results. At the final follow-up, radiologically 33 hips (55%) were graded as excellent, 11 (18.3%) as good, one as fair (1.7%) and 15 (25%)as poor.İn our study, detected strong relationship between the quality of the reduction, age (<45), fracture type ( T type) and the clinical result.Good and excellent result is possible with, optimal surgery, stable internaly fixation and anatomical reduction.
