Çalışmamızda hipertansif ebeveynlerin
normotansif çocuklarında (HENÇ) ileride gelişebilecek hipertansiyonun öngörücüsü
sayılabilecek, sol ventrikül yapı ve fonksiyonel değişikliklerinin belirlemesi
amaçlanmıştır. Bu amaçla sol ventrikül yapı ve fonksiyonları M-mode, pulse wave
Doppler (PWD) gibi konvansiyonel ekokardiyografi yöntemleri ile beraber doku
Doppler görüntüleme (DDG) ile değerlendirildi. Septal ve lateral mitral anulus doku
doppler miyokard performans indeksi (MPI) verilerinin etkinliği ve birbirine
üstünlüğü olup olmadığı araştırıldı. Çalışmaya ebeveynlerinin en az birinde
hipertansiyon öyküsü olan 92 çocuk ile, ebeveynlerinde hipertansiyon öyküsü
bulunmayan (NENÇ) 90 çocuk dahil edildi. Cinsiyet, yaş, kilo, boy ve vücut kitle
indeksi (VKİ) açısından gruplar arasında fark yoktu (p > 0.05). HENÇ grubunda
ortalama sistolik ve diyastolik kan basıncı değerleri NENÇ grubuna göre daha
yüksek tespit edildi (sırasıyla p = 0.003, p = 0.001, p < 0.001). HENÇ grubunda
interventriküler septum ve sol ventrikül arka duvar kalınlığı ile rölatif duvar kalınlığı
(RDK) NENÇ grubuna göre daha fazla saptandı. (sırasıyla p = 0.029, p = 0.016, p =
0.041). Doku Doppler görüntüleme ile septal ve lateral mitral anulus izovolümetrik
gevşeme zamanı (IVGZ) ve myokard performas indeksi (MPI) daha yüksek,
(sırasıyla p < 0.001, p < 0.001,p = 0.001, p = 0.001,) ejeksiyon zamanı daha kısa (p =
0.015, p < 0.02), septal izovolümetrik kasılma zamanı (IVKZ) daha uzun tespit
edildi (p < 0.002). Hem lateral hem de septal MPI ile sistolik kan basıncı arasında
pozitif, ejeksiyon fraksiyonu arasında negatif korelasyon saptandı (sırasıyla p =
0.042, p = 0.025, p = 0.032, p = 0.044). Septal ve lateral MPI Alıcı İşlem
Karakteristikleri (ROC) eğrisi analizleri arasında fark tespit edilmedi ( p > 0.05).
Sonuç olarak, hipertansif ebeveynlerin normotansif çocuklarında klinik hipertansiyon
bulguları oluşmadan önce sol ventrikülde morfolojik değişikliklere eşlik eden sistolik
ve diyastolik fonksiyon bozuklukları olduğu doku Doppler görüntüleme ile
gösterilmiştir.
In our study, left ventricular structural and
functional changes are intended to identify that can be considered as predictors of
hypertension in normotensive children of hypertensive parents (NCHP) may develop
hypertension in the future. For this purpose, the left ventricular structure and function
were evaluated by conventional echocardiography methods such as M-mode, pulsed
wave Doppler (PWD) along with tissue Doppler imaging (TDI). These were
investigated the effectiveness of septal and lateral mitral annular tissue Doppler
myocardial performance index (MPI) data and whether there is one another
superiority. Ninety-two children with history of hypertension at least one of their
parents and 90 children without a history of parental hypertension (NCNP) were in
included study. Gender, age, weight, height and body mass index showed no
statistical difference between the groups (p>0.05). The mean systolic and diastolic
blood pressure values were higher in the NCHP group (respectively p = 0.003, p =
0.001, p < 0.001). Interventricular septum and left ventricular posterior wall
thickness and relative wall thickness (RWT) were higher in the NCHP group
(respectively p = 0.029 and p = 0.016, p = 0.041). Septal and lateral mitral annular
isovolumetric relaxation time (IRT) and myocardial performances index (MPI) with
TDI were higher (p <0.001, p <0.001, p = 0.001, p = 0.001), ejection time was
shorter (p = 0.015, P <0.02), septal isovolumetric contraction time (IVCT)
determined longer (p <0.002). There was positive correlation between both the lateral
and septal MPI with systolic blood pressure and a negative correlation with ejection
fraction (p = 0.042, p = 0.025, p = 0.032, p = 0.044). There was no differences
between Receiver Operator Characteristic (ROC) curve analysis of septal and lateral
MPI (p> 0.05). As a result, systolic and diastolic dysfunction that accompanied by
morphological changes in left ventricul were shown by TDI in normotensive
children of hypertensive parents, even if there are not signs of clinical hypertension.
