Febril nötropenik lösemi hastalarında enfeksiyon
varlığını ve prognozu belirlemede çeşitli laboratuar parametrelerinin yararlı olduğu
gösterilmişse de maliyeti yüksek bu parametrelerin çoğunun rutin kullanıma girmesi
uygun bulunmamıştır. Endocan anjiogenezis, lenfogenezis ve kanser gelişimde rol
alan anahtar moleküllerden biri olarak tanımlanmıştır. Bu çalışmada, hastanemizde
mayıs 2012 – nisan 2013 tarihleri arasında takip edilen pediatrik febril nötropenik
hastalarımızın atak başlangıcında kantitatif endocan değerleri ölçülerek febril
nötropenik hastalarda enfeksiyon ve mortalite varlığını doğru ve güvenilir bir şekilde
göstermekte endocan’ ın katkısını değerlendirmek amaçlandı. Çalışmamıza 1 ay ile
18 yaş arası ALL ve AML tanılarıyla takip edilen ve febril nötropeni atağı geçiren
toplam 20 hastada 33 febril nötropeni atağı ile kontrol grubu olarak nötropenisi ve
ateşi olmayan 33 ALL ve AML hastaları ile enfeksiyonu olmayan 24 sağlıklı hasta
dahil edildi. Febril nötropeni etyolojisinde atakların 15’inde (%45) bakteriyel
enfeksiyon ve 4’ ünde (%12) invaziv fungal hastalık saptanırken geri kalan 14 ünde
(%43) ise herhangi bir etken gösterilememiş olup idiyopatik febril nötropeni olarak
kabul edildi. Çalışmaya alınan hastalardan febril nötropenik atak gelişen 3 hasta(%9
) eksitus oldu. Serum endocan düzeyleri ferbil nötropenisi olan hasta grubunda 0.094
ile 0.943 ng/ml arasında değişmekte olup, medyan 0.241 ng/ml idi. Febril nötropenisi
olan hasta grubunda medyan serum endocan düzeyi kontrol grubuna göre anlamlı
yüksek saptanırken (p<0.01), febril nötropenisi olmayan lösemi hastaları ile sağlıklı
kontrol grubunda serum endocan düzeyleri arasında fark saptanmadı (p>0.05). Sonuç
olarak; Çalışmamızda serum endocan seviyelerinin febril nötropeninin
tanımlanmasında bir gösterge olarak kullanılabileceği gösterilmiştir. Serum endocan
düzeylerinin seri olarak takip edilmesi, enfeksiyon nedenli veya hastalığa bağlı ateş
tablosunun ayırdedilmesinde yararlı olduğunu göstermektedir.
Although various laboratory parameters have been shown to be useful in determining
the existence of infection and the prognosis in febrile neutropenic patients, none of
these expensive markers are appropriate for routine use. Endocan is a sulfate
proteoglycan which is encoded by a single gene and plays important roles in
inflammation, angiogenesis, and tumor metastasis. The aim of this study was to
determine diagnostic and prognostic value of endocan in febrile neutropenic patients
by taking the Endocan levels at the beginning of the attack from the pediatric febrile
neutropenia patients seen between May 2012 and April 2013 at our hospital. 20
previously diagnosed ALL and AML patients with 33 febrile neutropenia attacks
between the age of 1 month and 18 years are included, as control group 33
previously diagnosed ALL and AML patients without febrile neutropenia and 24
healthy children are included. In 15 (%45) patients bacterial infection, in 4 (%12)
patients invasive fungal infection is determined and in 14 (%43) of the patients, no
infectious agent is shown and are accepted as idiopathic febrile neutropenia. 3 of the
febrile neutropenic patients died during the trial. The serum endocan levels in febrile
neutropenic patients are found between 0.094 and 0.943 ng/ml, with median 0.241
ng/ml. The median serum endocan levels in febrile neutropenic patients is higher
compared to control group(p<0.01). No difference is found between the median
serum endocan levels of the patients without febrile neutropenia and control group
(p>0.05). As result, it is shown that serum endocan levels can be used to determine
febrile neutropenia. It is shown that following the serum endocan levels is useful to
separate the fever caused by infection and the fever related to the illness.
