Bu deneysel çalışmamızda ki amacımız rat alt ekstremite iskemi reperfüzyon sonrası gastrokinemius kasında oluşan iskemi-reperfüzyon hasarına iloprostun koruyucu etkisini araştırmaktır.
Çalışmamızda 24 adet Wistar Albino cinsi rat kullanıldı. Ratlar randomize olarak, eşit sayıda (n=8) üç gruba ayrıldı. Grup 1‟ de (sham) 6 saat süresince sadece anestezi uygulandı. Grup 2‟ de (kontrol) ve grup 3‟ de (İloprost tedavi grubu) anestezi verildikten sonra, turnike yöntemiyle ratların sağ arka ekstremitelerine, kalça eklemi hizasından turnike uygulanılarak 3 saat iskemi ve turnike açılarak 3 saat reperfüzyon uygulandı. Grup 3‟ te reperfüzyondan 30 dakika önce sağ internal juguler venden ıv 20 μg/kg dozda iloprost infüzyonu başlanıldı. İnfüzyon reperfüzyon süresince devam etti.
Deney sonunda tüm gruplardan biyokimyasal analizler için kan örneği ve immünohistokimyasal değerlendirme için gastrokinemius kas doku örnekleri alındı. Alınan kan örneklerinde TNF- α, IL-1, IL-6 plazma düzeyleri ölçüldü. Gastrokinemius kas doku örnekleri formaldehit ile fikse edildi. Alınan kas dokudan biyokimyasal olarak MDA ve katalaz düzeyleri, histolojik olarak ıĢık mikroskobu altında hemotoksilen eosin ile genel doku değerlendirilmesi ve immünohistokimyasal olarak kaspaz tayini yapıldı.
Biyokimyasal inceleme sonuçlarına göre ĠR grubunda (grup 2) kas dokusunda MDA, serumda TNF-α ve IL-1 düzeyleri tedavi grubuna (grup 3) göre anlamlı derecede düşük bulunurken; kas dokusunda katalaz,serumda IL-6 düzeylerinde anlamlı fark bulunamamıştır. Kaspaz -3 immunohistokimyasal sonuçlarımıza göre ise İloprost iskelet kasında İR hasarının meydana getirdiği apopitozise karşı korumaktadır.
Totally, 24 Wistar Albino-type rat were used in the present study. Rats were randomly divided into three groups (ne=8 in each group). Group 1 (sham) received only anaesthesia for 6 hours. In Group 2 (control) and Group 3 (Iloprost treatment group), ischemia was generated by tourniquet method applying the tourniquet onto right back extremities at the level of hip joint for 3 hours after anaesthesia, then reperfusion was completed releasing the tourniquet for 3 hours. Intravenous 20 μg per kilogram infusion was started into right internal jugular vein 30 min before reperfusion in Group 3. The infusion continued during reperfusion.
At the end of the experiment, blood sample for biochemical analysis and gastrocnemius muscle tissue sample for immunohistochemical assessment were obtained in all groups. Tumour necrosis factor-α, IL-1 and IL-6 plasma levels were measured. Gastrocnemius muscle tissue samples were fixed into formaldehyde. Biochemical MDA and catalase levels were measured, histopathologic tissue assessment was performed with hematoxylin eosin under light microscope and immunohistochemical caspase was determined.
According to the biochemical assessments of ischemia-reperfusion groups, while serum TNF-α and IL-1 levels and MDA levels into muscle tissue were significantly lower in Group 2 compared to Group 3, there was no significant difference in serum IL-6 and catalase into muscle tissue between the groups. Based on our immunohistochemical caspase-3 results, iloprost protects the skeletal muscle against the apoptosis result from IR.
