Bu çalışmada inferior konka hipertrofisinde kemik konkanın rolünü araştırmayı ve
kemik konkaya cerrahi müdahale sonucunda burun tıkanıklığını objektif ve subjektif
testler ile değerlendirmeyi amaçladık. Burun tıkanıklığı şikayeti olan ve bilgisayarlı
paranazal tomografi ile kemik konka hipertrofisi saptanan 30 hasta çalışma grubunu
oluşturdu. Burun tıkanıklığı olmayan ve başka nedenlerle temporal kemik
bilgisayarlı tomografisi çekilen 30 hasta kontrol grubunu oluşturdu. Çalışma
grubundaki hastalara burun tıkanıklığını düzeltmek amacıyla submukozal rezeksiyon
tekniği uygulandı. Bu hastalara operasyon öncesi ve sonrası VAS ve akustik
rinometri testleri uygulandı. Kontrol grubundaki hastalara da VAS ve akustik
rinometri testleri uygulandı. Bütün hastaların bilgisayarlı tomografilerinde inferior
konkanın medial mukoza, lateral mukoza ve kemik konka kalınlıklarının horizontal
ölçümleri yapıldı. Subjektif değerlendirmeler sonucunda, çalışma grubundaki
hastaların burun tıkanıklığı şikayetinde operasyon öncesine göre azalma sağlandı
(p<0.001). Akustik rinometri ile elde edilen objektif verilere göre de hastaların burun
tıkanıklığı şikayetinde belirgin düzelme olduğu izlendi (p<0.001). Çalışma
grubundaki hastaların medial mukoza, lateral mukoza ve kemik konka
kalınlıklarının, kontrol grubunda ki hastaların ölçümlerine göre yüksek olduğu
görüldü (p<0.001). Burun tıkanıklığının en sık nedenlerinden birisi olan konka
hipertrofilerinde doğru tedavi seçimi için ayrıntılı muayene gereklidir. Hem tanı hem
tedavi seçiminde bilgisayarlı tomografi gibi görüntüleme yöntemleri ve hastanın
burun tıkanıklığını objektif değerlendiren akustik rinometri gibi testler oldukça
yararlıdır. Kemik konka hipertrofisi saptanan hastalarda submukozal rezeksiyon
tekniği güvenle kullanılabilen bir yöntemdir.

study, we aimed to research the role of the bony hypertrophy in turbinate
hypertrophy and to evaluate nasal obstruction after the surgery of bone component of
inferior turbinate with objective and subjective tests. 30 patients, who have
complaints of nasal obstruction and who detected bony hypertrophy with paranasal
computed tomography, were included in the study group. 30 patients, who don’t have
complaints of nasal obstruction and have temporal bone computed tomography with
other reasons, were included in control group. Submucosal resection technique was
performed to patients in study group. Preoperative and postoperative VAS and
acoustic rhinometry tests were administered to these patients VAS and acoustic
rhinometry tests were also administered to patients in control group. All patients’
horizontal measurements of medial, lateral mucosa and bone component thickness of
the inferior turbinate at computed tomography was made. As a result of subjective
evaluations, the patients in the study group having nasal obstruction were provided
by the reduction of preoperative symptoms (p <0.001). According to objective data
obtained by acoustic rhinometry revealed significant improvement in symptoms of
nasal obstruction (p <0.001). Measurements of medial, lateral mucosa and bone
thickness of the inferior turbinate of study group was found higher than the
measurement of the patients in the control group (p <0.001). At turbinate
hypertrophy, one of the most common causes of nasal obstruction, it is required
detailed examination for the selection of the correct treatment. Imaging techniques
such as computed tomography and acoustic rhinometry tests evaluating objectively
the patient's nasal obstruction, is very useful for to choice the true treatment and
diagnostic tests. Submucosal resection technique is a method that can be used safely
at patients diagnosed bony hypertrophy.
