Bu çalışmanın amacı biyopsi sonucu rektum adenokarsinomu
tanısı alan hastalarda prospektif olarak 3 Tesla manyetik rezonans görüntüleme
(MRG) ile T ve N evrelemesi yapmak, neoadjuvan kemoradyoterapi (KRT) alan
hastalarda neoadjuvan KRT’nin etkinliğini ve tümör regresyonunu tahmin
edebilmektir. Bu çalışmaya 28 hasta (hastaların 14’ü operasyondan önce KRT aldı)
dahil edildi. 3Tesla MRG ile T ve N evrelemesi yapıldı. Neoadjuvan KRT alan
hastalarda KRT sonrası, operasyon öncesi tekrar MRG tetkiki yapıldı. Tüm hastalara
difüzyon ağırlıklı görüntüleme çalışıldı ve apparent diffusion coefficient (ADC)
haritası çıkarıldı. T evrelemesinde ekstramural invazyonu değerlendirmede duyarlılık
%95, özgüllük %67, doğruluk oranı %86 olarak bulundu. MRG ekstramural
invazyonu saptamada neoadjuvan KRT almayan hastalarda, neoadjuvan KRT alan
hastalara göre daha başarılıydı (doğruluk oranı sırasıyla %93, %79). N evrelemesi
DAG, T2AG ile ve iki yöntem beraber olarak değerlendirildi. Lenf nodu metastazı
varlığını saptamada doğruluk oranları DAG ve DAG+T2AG’nin, T2AG’den daha
yüksek bulundu (sırasıyla %75,%75,%71). Neoadjuvan KRT’nin etkinliğini
değerlendirmek amacıyla tümörün ADC değerleri pre-KRT ve post-KRT MRG’de
hesaplandı. Tümör regresyon grade analizine göre iyi yanıt gösteren hastalarda
ortalama post-KRT ADC ve iki MRG’de ölçülen ADC’lerin ortalama farkı (ΔADC)
kısmi yanıt gösteren hastalardan anlamlı olarak daha yüksekti (sırasıyla p=0,019,
p=0,02). ΔADC için cut-off değeri 0,26x10-3 mm2/sn alındığında duyarlılık %90,
özgüllük %100, doğruluk oranı %93 ve p değeri <0,001 bulundu. Sonuç olarak,
3Tesla MRG rektum kanserinin T evrelemesinde ekstramural invazyonu belirlemede
yüksek doğruluk oranına sahiptir. N evrelemesinde DAG ve T2AG’nin birlikte
kullanılması doğruluk oranını artırmaktadır. DAG ve ADC neoadjuvan KRT’ye
yanıtı değerlendirmede yüksek prediktif değere sahiptir.
The purpose of this study was to prospectively assess
the T and N staging in rectal adenocancer with 3 Tesla magnetic resonance imaging
(MRI), and to evaluate the tumor response to neoadjuvant chemoradiotherapy (CRT).
Twenty eight patients (14 patients underwent preoperative neoadjuvant CRT while
14 patients did not) included in the study. T and N staging were assessed in all
patients. The patients who underwent neoadjuvant CRT were reevualuated
preoperatively after completion of CRT. Diffusion weighted imaging (DWI) was
performed and apparent diffusion coefficient (ADC) map was analyzed in all
patients. In the prediction of extramural invasion with 3 Tesla MRI, the sensitivity,
specificity and diagnostic accuracy were 95%, 67% and 86% respectively. The
diagnostic accuracy of MRI for extramural invasion in the patients who did not have
neoadjuvant CRT (93%) than who had neoadjuvant CRT (79%). N staging was
assessed with DWI, T2WI and DWI+T2WI. The diagnostic accuracy in predicting a
nodal metastasis for DWI, T2WI and DWI+T2WI were 75%, 75%, %71,
respectively. Apparent diffusion coefficients (ADCs) in the pre-CRT MRI and post-
CRT MRI were calculated and compared between the good response group and the
partial response group. The mean post-KRT ADC and the mean absolute differences
ADC (ΔADC) were significantly higher in the good response group (p=0,019,
p=0,02, respectively). An ΔADC cut-off value of 0,26x10-3 mm2/sn had 90%
sensitivity, 100% specificity, 93% diagnostic accuruacy (p <0,001). In conclusion,
3Tesla MRI had high accuracy for the prediction of T staging and extramural
invasion. Adding DWI to convantional MRI yields better diagnostic accuracy than
use of conventional MRI alone in the evaluation of N staging. DWI and ADC have
high predictive value in the evaluation of tumor response to neoadjuvant
chemoradiation.
