KOAH atağı hastanın yaşam kalitesini bozan, ek tedavi
gerektiren solunumsal yakınmalarda bozulmadır. Hastaların tedaviye uyumsuzluğu
atak nedenlerinden biridir. Hastaların tedaviye uyumu ile KOAH atak arasındaki
ilişkiyi araştırmayı amaçladık. Prospektif olarak 04.10.2017-04.10.2018 tarihleri
arasında başvuran 357 hasta çalışmaya alındı. Öğrenim düzeyi ilk öğretim ve altı
olanlar, tedaviye ulaşmakta zorluk çekenler (p=0.047), evde oksijen tedavisi alanlar
(p=0.011), acil serviste oksijen tedavi alan hastalar (p=0.029), cerrahi tedavi alanlar
(p=0.029) ve KOAH nedeniyle yaşam kalitesi her zaman-sıklıkla etkilenenler
(p=0.040) atak nedeniyle acil servise daha sık başvurdu. Hastaların kışın atak
nedeniyle başvuru sayısı diğer mevsimlere göre daha az idi (p= 0.008). Hastaların acil
serviste antibiyotik alanları (p=0.021), günlük düzenli fiziksel aktivitelerini her
zaman-sıklıkla yapanları (p=0.024) ve yakınlarından her zaman-sıklıkla destek alanları
(p=0.005) atak nedeniyle acil servise daha az sayıda başvurdu. Düzenli tedavi alan ve
almayan hastalarla (p=0.886) hekim tavsiyesine uyan ve uymayan hastaların (p=0.623)
acil servise atak nedeniyle başvuru sıklığında anlamlı fark saptanmadı. Sonuç olarak;
öğrenim düzeyi düşük olan, tedaviye ulaşmakta zorlanan, evde ve acil serviste oksijen
ihtiyacı olan ve cerrahi tedavi alan KOAH hastalarında atak geçirme sıklığı daha
fazladır. Acil serviste antibiyotik alan, günlük fiziksel aktivitesini iyi yapan ve
yakınlarından iyi destek alan KOAH hastalarında atak geçirme sıklığı daha azdır.
Düzenli tedavi alan, hekim tavsiyesine uyan tedaviye uyumlu KOAH hastalarında atak
sıklığı etkilenmemektedir
COPD exacerbations are
disturbances in respiratory symptoms, which require additional treatment and impair the
patient's quality of life. Patients' noncompliance with treatment is one of the causes of
exacerbation. We aimed to investigate the correlation between patients' compliance to
treatment and COPD exacerbation. Prospectively, 357 patients who applied to the
emergency department between October 04, 2017 and October 04, 2018 were included in
the study. Those with primary education and lower education level (p = 0.048), those who
have difficulty in accessing treatment (p = 0.047), those who receive oxygen therapy at
home (p = 0.011), the patients who receive oxygen therapy in the ED (p = 0.029), those
who receive surgical treatment (p = 0.029) and those whose quality of life has always been
frequently affected by COPD (p = 0.040) applied to the ED more frequently due to the
exacerbation. The number of admissions was lower in winter than in other seasons (p =
0.008). Patients on antibiotic (p = 0.021) in the ED, those who always do their daily
physical activities regularly (p = 0.024), and those who receive support from their relatives
(p = 0.005) applied to the emergency room less frequently due to the exacerbation. There
was no significant difference in the frequency of admittance to the emergency department
between patients who did and did not receive regular treatment (p = 0.886), and between
those who did and did not comply with physician's advice (p = 0.623). As a result, the
frequency of exacerbations is higher in COPD patients who have low education levels,
have difficulty in accessing treatment, need oxygen at home and in the ED, and receive
surgical treatment. The frequency of exacerbations is lower in COPD patients who take
antibiotics in the ED, do their daily physical activity and receive good support from their
relatives. The frequency of exacerbations is not affected in COPD patients who receive
regular treatment and comply with the advice of a physician
