IVF sikluslarında elde edilecek oosit sayısı, tedavinin başarısı için en önemli klinik ölçülerden biridir. Over rezerv testleri, ovaryen folikül havuzunun büyüklüğünün ve kalitesinin biyokimyasal ve ultrasonografik ölçümlerini içermektedir. Biyokimyasal testler folikül stimülan hormon (FSH), östradiol (E2), antimülleryan hormon (AMH) gibi ölçümleri içermektedir. Over rezervinin ultrasonografik ölçümleri, antral folikül sayısı ve over hacmini içermektedir. Bu testlerden yola çıkılarak Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı Üreme Sağlığı Merkezinde IVF tedavisi görmüş 793 infertil çift, OPU işlemi ile elde edilen oosit sayısı (over yanıtı) açısından incelendi. Analizler sonucu over yanıtına etki eden ve ayrımsama gücü en yüksek olan kombinasyon lojistik regresyon analizi kullanılarak formülize dahil edildi. Günlük pratikte kolayca uygulanabilecek bu formül OYS (Over yanıt skoru) olarak isimlendirildi. Bu formüle göre veriler girildikten sonra elde edilen skor, IVF tedavisi sonucu grafikte karşılık gelen normal over yanıtı için olasılık tahmin değerini (OPU ile dört ve üzerinde oosit elde etme olasılığı) vermektedir. Bu formül ile elde edilen normal over yanıtını elde etme olasılığı sayesinde, infertil çiftlere tedavi öncesi yol gösterebilecek somut veriler ortaya kondu. Bu sayede çiftlerin tedavi maliyetlerini ve tedavinin başarılı olma şansını tekrar gözden geçirebilmesi, hekim ile hasta ilişkisinin sağlam temeller üzerine kurulması amaçlandı. Çalışmada kullanılan veriler çoğunlukla belirli demografik özellikteki popülasyondan elde edildi. Irk, etnisite, yaşanılan coğrafik bölge, genetik polimorfizm gibi özelliklerin IVF yanıtında değişiklikler oluşturduğu düşünülmektedir. Ayrıca laboratuvar parametrelerinin ölçüm yöntemi, saklama koşulları, ölçüm cihaz ve kitlerindeki değişiklikler dolayısıyla çalışmalar arasında farklar görülebilir. Bu nedenle her infertilite kliniğinin prediksiyon açısından kendi verilerini kullanmasının en doğru yaklaşım olduğuna inanmaktayız
One of the most important clinical predictors of success is “retrieved oocyte number” in IVF cycles. Ovarian reserve tests include biochemical and ultrasonographic measurements and quality of the ovarian follicle. Biochemical tests include measurements such as follicle stimulating hormone (FSH), estradiol (E2), antimullerian hormone (AMH). Ultrasonographic measurements of ovarian reserve include the number of antral follicles and ovarian volume. Based on these tests, 793 infertile couples who underwent IVF treatment in Eskisehir Osmangazi University Faculty of Medicine, Department of Obstetrics and Gynecology, Reproductive Health Center were examined in terms of ovarian response. The combination affecting the ovarian response and having the highest discrimination power was formulated using logistic regression analysis. This formula, which can be easily applied in daily practice, was named as ORS (Ovarian response score). This score obtained after entering the data according to this formula gives the probability of predicted value (probability of retrieving four or more oocytes with oocyte pick-up) for the corresponding normal ovarian response in the graph as a result of IVF treatment. Through the probability of assessing a normal ovarian response obtained with this formula, concrete data have been presented that can guide infertile couples before treatment. In this way, it was aimed to enable couples to review the treatment cost and the chances of IVF success, and to establish relationship between physician and patient on a solid basis. The data used in this study were mostly obtained from the population with specific characteristics. Traits such as race, ethnicity, geographic region and genetic polymorphism are thought to cause changes in IVF response. In addition, differences may occur between studies due to the measurement method of laboratory parameters, storage conditions, changes in measurement devices and kits.
