Bu çalışmada, fiziksel, sosyal ve psikolojik yönleriyle önemli
bir halk sağlığı sorunu olan obezitenin özerklik, özerkliğe saygı ilkesi
(ÖSİ) ve yaşam kalitesi (YK) üzerindeki etkisi, başka bir ifade ile obez
hastalar ile obez olmayan bireylerin özerklik, ÖSİ ve YK bakımından
farklılaşıp farklılaşmadığının araştırılması amaçlanmıştır.
Kapsam: Çalışmanın evrenini Eskişehir Yunusemre Devlet Hastanesi,
Eskişehir Devlet Hastanesi, Eskişehir Osmangazi Üniversitesi Sağlık
Uygulama ve Araştırma Hastanesi Beslenme ve Diyet poliklinikleri ile
Eskişehir Büyükşehir Belediyesi Kadın Danışma ve Dayanışma
Merkezi Beslenme ve Diyet Birimi’ne başvuran ve araştırmaya
katılmayı kabul eden hastalar ve danışanlar oluşturmuştur.
Araştırmanın vaka grubunu beden kütle indeksi (BKİ)≥30,00 kg/m²
olan 354 kişi; kontrol grubu ise, BKİ değeri 18,50-24,99 kg/m² arasında
olan 354 kişi oluşturmuştur.
Yöntem: Vaka ve kontrol gruplarından oluşan kesitsel, çözümleyici ve
tanımlayıcı özellikte planlanan çalışma, ilgili kurumlardan yazılı izin
alındıktan sonra, Ağustos-Ekim 2018 tarihleri arasında yapılmıştır.
Katılımcılara, çalışmanın amaçları göz önünde bulundurularak
oluşturulan sosyodemografik bilgiler formu, özerklik ve ÖSİ’ne ilişkin
soruların yer aldığı anket formu ile Türkçe geçerlilik ve güvenilirlik
çalışması yapılmış Obezlere Özgü Yaşam Kalitesi Ölçeği (OÖYKÖ)
uygulanmıştır. Verilerin değerlendirilmesinde tanımlayıcı
istatistikler, Kolmogorov-Smirnov Testi, Mann Whitney U Testi,
Kruskal Wallis H Testi, Ki-kare Bağımsızlık Analizi, Dunn’s Çoklu
Analiz Testi ve Spearman Korelasyon Analizi kullanılmıştır.
Bulgular: Görüş ve düşüncelerini rahatlıkla açıklayabildiğini ifade
eden obez hastalar ile görüş ve düşüncelerini rahatlıkla
açıklayamadığını ifade eden obez hastalar arasında OÖYKÖ skoru

açısından görüş ve düşüncelerini rahatlıkla açıklayabildiğini ifade
eden obez hastalar lehine istatistiksel olarak anlamlı farklılık
bulunmuştur (p<0,001). Kendi bedeni ve sağlığıyla ilgili konularda
kendisine de seçim hakkı verilmesini istediğini belirten obez hastalar
ile kendi bedeni ve sağlığıyla ilgili konularda kendisine de seçim
hakkı verilmesini istemediğini belirten obez hastalar arasında
OÖYKÖ skoru açısından istatistiksel olarak anlamlı farklılık
bulunmamıştır (p=0,961>0,05). Karar ve eylemlerinde başkalarının
düşüncelerinden etkilendiğini bildiren obez hastalar ile karar ve
eylemlerinde başkalarının düşüncelerinden etkilenmediğini bildiren
obez hastalar arasında OÖYKÖ skoru açısından karar ve eylemlerinde
başkalarının düşüncelerinden etkilenmediğini bildiren katılımcılar
lehine istatistiksel olarak anlamlı farklılık bulunmuştur (p<0,001).
Çalışmamızda obezite ile OÖYKÖ skoru arasında istatistiksel olarak
kontrol grubu lehine anlamlı farklılık bulunmuştur (p<0,001).
Sonuç: Obez olmayan bireylerin daha iyi yaşam kalitesine sahip
olduğu bulunmuşken obezitenin, hastaların özerk olmalarını,
kendilerini özgür ve bağımsız hissetmelerini etkilemediği; özerk
seçim yapmalarını, eylem özerkliklerini ve düşünce özerkliklerini
olumsuz etkilediği; birey özerkliklerini, istek özerkliklerini ve ÖSİ’ni
benimsemelerini olumlu etkilediği ifade edilebilir. Obez hastaların
YK’lerinin özerklik ve ÖSİ çerçevesinde daha somut şekilde ele
alınabilmesi için “Obez Hastalarda Algılanan Özerklik Ölçeği”nin
hazırlanarak gelecek çalışmalarda her iki ölçeğin birlikte
kullanılması önerilmektedir
The aim of this study was to investigate the effect of
obesity, an important public health problem in terms of physical,
social and psychological aspects, on autonomy, the principle of
respect for autonomy (PRA) and quality of life (QoL), in other words,
whether obese and non-obese individuals differ in terms of autonomy,
PRA, and QoL.
Scope: The universe of the study consisted of patients and counselees
who were admitted to the Nutrition and Diet outpatient clinics of
Eskisehir Yunusemre State Hospital, Eskisehir State Hospital,
Eskisehir Osmangazi University Health Practice and Research
Hospital and to the Nutrition and Diet Unit of Eskisehir Metropolitan
Municipality Women’s Counseling and Solidarity Center and who
accepted to the participate in the study. The study group consisted of
354 individuals with a body mass index (BMI) of ≥30,00 kg/m² and the
control group consisted of 354 individuals with a BMI between 18.50
and 24.99 kg/m².
Method: This cross-sectional, analytical and descriptive study
consisting of study and control groups was conducted between August
and October 2018 after obtaining written approval from the relevant
institutions. A sociodemographic information form created by
considering the objectives of the study, a questionnaire including
questions about autonomy and PRA, and Obesity and Weight Loss
Quality of Life scale (OWLQOL), the Turkish validity and reliability
study of which was performed, were administered to the participants
were administered. Descriptive statistics, Kolmogorov-Smirnov test,
Mann-Whitney U test, Kruskal-Wallis H Test, Chi-square
Independence Analysis, Dunn’s Test of Multiple Comparisons and
Spearman Correlation Analysis were used to evaluate the data.

Results: There was a statistically significant difference between the
obese patients stating that they could easily explain their opinions
and thoughts and the obese patients stating that they could not easily
explain their opinions and thoughts in terms of OWLQOL score in
favor of the obese patients stating that they could easily explain their
opinions and thoughts (p<0.001). There was no statistically significant
difference between the obese patients stating that they wanted to be
given the right to choose about their own body and health and the
obese patients stating that they did not want to be given the right to
choose about their own body and health in terms of OWLQOL score
(p=0.961>0.05). There was a statistically significant difference between
the obese patients reporting that they were affected by the opinions of
others in their decisions and actions and the obese patients reporting
that they were not affected by the opinions of others in their decisions
and actions in terms of OWLQOL score in favor of the participants
reporting that they were not affected by the opinions of others in
their decisions and actions (p<0.001). In our study, there was a
statistically significant difference between obesity and OWLQOL
score in favor of the control group (p<0.001).
Conclusion: While it was found that non-obese individuals had a
better quality of life, it can be speculated that obesity did not affect
the autonomy of the patients and their feeling of being free and
independent, negatively affected their autonomous choice, autonomy
of action and autonomy of thought, and positively affected their
individual autonomy, autonomy of desire and adoption of PRA. It is
recommended that the “Scale of Perceived Autonomy in Obese
Patients” be prepared and two scales be used together in future
studies in order to be able to address the QoLs of obese patients in a
more concrete manner within the framework of autonomy and PRA
