Çocuğun yoğun bakıma yatması, hem çocuk hem de ailede
travma etkisi oluşturabilir. Yaşanan travmaya bağlı çocuk ve ailelerinde
akut stres bozukluğu (ASB) ve travma sonrası stres bozukluğu (TSSB)
görülebilir.
Amaç: Çalışma pediatri yoğun bakım ünitesi (PYBÜ)’nden
taburcu olan ve/veya servise devredilen 8-12 yaş arası çocuklar ve
ebeveynlerinde ASB ve TSSB riskini ve etkileyen faktörleri belirlemek
amacıyla yapıldı.
Gereç-Yöntem: Kesitsel ve ilişki arayıcı tipte bir çalışmadır.
Araştırma, Hacettepe Üniversitesi Tıp Fakültesi İhsan Doğramacı Çocuk
Hastanesi PYBÜ’de yatan çocuk ve ebeveynleri ile yapıldı. Çalışmaya;
ulaşılabilen, araştırmayı kabul eden ve örneklem seçim kriterlerine uyan
110 çocuk, 98 anne ve 80 baba dahil edildi. Veriler 1 Ocak 2019 ile 31
Mart 2019 tarihleri arasında toplandı. Çalışma yapılmadan önce etik
kurul izni ve gerekli kurum izinleri alındı. Çalışmada veri toplamak
amacıyla “Kişisel Veri Toplama Formu”, ASB ve TSSB’yi belirlemek için
“Travma Sonrası Stres Tepki Ölçeği” ve “TSSB Soru Listesi - Sivil
Versiyonu” kullanıldı. Çalışmada kullanılan travma sonrası stres tepki
ölçeğinde; 12-24 puan arası “Hafif Düzeyde TSSB”, 25-39 puan arası
“Orta Düzeyde TSSB”, 40-59 puan arası “Ağır Düzeyde TSSB”, 60 ve
üstü puan ise “Çok Ağır Düzeyde TSSB” şeklinde yorumlanmaktadır.
Çalışmada anne ve babalar için kullanılan “travma sonrası stres tepki
ölçeği”nin kesme puanı ise 22-24 arasındadır. Bu puanların üstü yüksek
risk kabul edilmektedir. İstatistiksel analizler SPSS paket programı ile
yapıldı ve p<0,05 önemli kabul edildi. Çalışma sonunda Gpower 3.0.10

programı kullanılarak (alpha=0,05) yapılan güç analizinde araştırmanın
gücü; 110 çocuk için %85, 98 anne için %93,3 ve 80 baba için %81,5
olarak bulundu.
Bulgular: Çalışmada çocuklarda ASB puanı ortalaması
35,74±12,43 (min. 12, maks. 74), TSSB puanı ortalaması ise 34,46±15,89
(min. 8, maks. 74) olarak bulunmuştur. Buna göre yaptığımız çalışmada
çocuklarda hafif düzeyden çok ağır düzeye kadar ASB ve TSSB riski
bulunmaktadır. Çalışmada anneler için ASB puanı ortalaması
47,79±16,32 (min. 19, max. 85) ve TSSB puanı ortalaması 53,66±16,93
(min. 17, max. 85) olarak bulunmuştur. Babalar için ASB puanı
ortalaması 37,23±12,74 (min. 17, max, 74) ve TSSB puanı ortalaması
33,24±14,61 (min. 17, max. 71) olarak bulunmuştur. Buna göre
yaptığımız çalışmada çocuğu yoğun bakımda yatan anne ve babalar da
TSSB yüksek risk olarak saptamıştır. Çalışmada çocuk, anne ve babanın
ASB puanları arasında pozitif yönde istatistiksel olarak önemli ilişki
saptandı (p<0,05). Aynı ilişki TSSB puanları arasında da bulundu
(p<0,05). Çocuğun, anne ve babanın ASB ve TSSB puanları arasında da
pozitif yönde ilişki tespit edildi (p<0,05). Ebeveyninin kendisine destek
olduğunu belirten çocukların ASB puanları, ebeveyn desteğinin
olmadığını belirten çocuklardan daha yüksek bulundu (p<0,05). Aile
gelir durumu ile annenin TSSB puanı arasında, çocuğun yoğun bakımda
ölüme tanık olma durumu ile çocuğun ASB puanı arasında istatistiksel
olarak anlamlılık saptandı (p<0,05).
Sonuç ve Öneriler: Çocuğun ASB puanı arttıkça, annenin ve
babanın ASB puanının arttığı, aynı şekilde çocuğun ASB puanı
azaldıkça, annenin ve babanın ASB puanının azaldığı belirlendi. Aynı
ilişki TSSB puanları arasında da bulundu. Çocuğun, annenin ve babanın
ASB puanı ile TSSB puanı arasında da pozitif yönde ilişki olduğu
saptandı. Önerimiz PYBÜ’den taburcu olan ve/veya servise devredilen
tüm çocuk ve ebeveynlerinde TSSB taramasının yapılmasıdır. Çocuk ve
ebeveynlerinde TSSB taraması yapılması, durumun erken dönemde

tespiti, gerekli sağlık kurumlarına yönlendirilmesi ve tedavisi yönünden
önemlidir
A child’s hospitalization in intensive care may
create a trauma both for the child and her/his family. As a result of the
trauma, the child and the family may develop acute stress disorder
(ASD) and posttraumatic stress disorder (PTSD).
Purpose: The study was conducted to determine ASD and PTSD
risk and the affecting factors in children aged 8 to 12 years discharged
from the pediatric intensive care unit (PICU) and/or referred to the
service and their parents.
Materials-Methods: It is a cross-sectional and relationship seeking study. The descriptive study was carried out with children
hospitalized in the PICU of Hacettepe University Medical Faculty Ihsan
Dogramaci Children’s Hospital and their parents. In the study; 110
children, 98 mothers and 80 fathers who were accessed, agreed to
participate and met the sample selection criteria, were included. The
data were collected between January 1

, 2019.
Before the study, an ethics committee approval and necessary institution
permissions were received. In the study, the data were collected using
the “Personal Data Collection Form”. In order to determine ASD and
PTSD, the “Posttraumatic Stress Reaction Scale” and the “PTSD
Question List – Civil Version” were used. In the Posttraumatic Stress
Reaction Scale used in the study; 12-24 points are interpreted as “Mild
PTSD”, 25-39 points “Medium PTSD”, 40-59 points “Severe PTSD” and
60 points and over “Very Severe PTSD”. Cutoff score of the
“Posttraumatic Stress Reaction Scale” used in the study for parents was
between 22-24. Scores above these are accepted as high risk. Statistical
analyses were conducted with the SPSS package program and the

significance was accepted at the level of p<0,05. In the power analysis
which was performed at the end of the study using the Gpower 3.0.10
program (alpha=0,05), power of the study was found to be 85% for 110
children, 93,3% for 98 mothers and 81,5% for 80 fathers.
Findings: In the study, the ASD score average was found to be
35,74±12,43 (min. 12, max. 74) and the PTSD score average 34,46±15,89
(min. 8, max. 74) for children. Accordingly in our study, we determined
ASD and PTSD risk in children, ranging from mild to very severe. In the
study, the ASD score average was found to be 47,79±16,32 (min. 19, max.
85) and the PTSD score average 53,66±16,93 (min. 17, max. 85) for
mothers. Also, the ASD score average was found to be 37,23±12,74 (min.
17, max, 74) and the PTSD score average 33,24±14,61 (min. 17, max. 71)
for fathers. Accordingly in our study, we determined a higher PTSD risk
in parents with children hospitalized in intensive care. It was
determined that there was a positively and statistically significant
correlation between the ASD scores of children, mothers and
fathers (p<0,05). The same correlation was also observed between the
PTSD scores (p<0,05). It was determined that there was a positive
correlation between the ASD and PTSD scores of children, mothers and
fathers as well (p<0,05). The ASD scores of children indicating that their
parents supported them, were found to be higher than children
indicating that their parents did not support them (p<0,05). It was
determined that there was a statistical significance between the family’s
income status and mother’s PTSD score and between the child’s
witnessing a death in intensive care and her/his ASD score (p<0,05).
Conclusion and Recommendation: It was determined that as
the child’s ASD score increased, mother’s and father’s ASD score
increased and as the child’s ASD score decreased, mother’s and father’s
ASD score decreased. The same relationship was found among PTSD
scores. A positive correlation was found between the child’s, mother’s and
father’s ASD and PTSD scores. We suggest that all children discharged
from the PICU and/or referred to the service and their parents are

screened in terms of PTSD. Performing posttraumatic stress disorder
screening in children and parents is important for early diagnosis,
referral to necessary medical institutions and treatment
