Çalışmamızın amacı; rotator manşon lezyonu saptanan hastalarda proprioseptif
egzersizler ile birlikte uygulanan ayna tedavisinin klasik egzersizlere göre ağrı, kas
gücü, fonksiyonellik ve kinezyofobi üzerindeki etkisini değerlendirmektir.
Randomize, kontrollü, tek kör olarak planlanan bu prospektif çalışmada hastalar iki
gruba ayrıldı. Haftada 3 seans olmak üzere 6 hafta boyunca, birinci gruptaki 16
hastaya klasik egzersiz programı, ikinci gruptaki 16 hastaya ise proprioseptif
egzersizler ile birlikte ayna tedavisi uygulandı. Hastalar tedavi öncesi ve tedavi
sonrası, Vizuel Analog Skala (VAS), eklem hareket açıklığı (EHA) ve manuel kas
gücü ölçümü, Omuz Ağrı ve Özürlülük İndeksi (OAÖİ), Tampa Kinezyofobi Ölçeği
(TKÖ), Constant Omuz Skorlaması (COS) ile değerlendirildi. Tedavi sonunda iki
grupta da VAS (istirahat, hareket, gece), EHA (fleksiyon, abduksiyon, aktif dış
rotasyon), omuz kas gücü ölçümü, OAÖİ (ağrı, özürlülük, toplam), COS (ağrı,
günlük aktivite, aktif EHA, toplam) skorlarında iyileşme saptandı (p<0,05). Sadece
grup 1’de aktif iç rotasyon, pasif dış rotasyon EHA ölçümünde ve TKÖ puanlarında
anlamlı iyileşme gözlenirken, COS kuvvet alt başlığında sadece grup 2’ de anlamlı
iyileşme gözlendi (p<0,05). Gruplar karşılaştırıldığında, VAS gece skorunda ve
OAÖİ’de (ağrı, özürlülük, toplam) proprioseptif egzersiz ve ayna tedavisi alan grupta
daha fazla iyileşme saptandı (p<0,05). Bunlar dışında kalan tüm ölçümlerde gruplar
arasında farklılık yoktu (p>0,05). Sonuçlarımız her iki egzersiz yönteminin de
semptomatik rotator manşon lezyonu tanılı hastalarda etkili olduğunu ve
proprioseptif egzersizler ile birlikte uygulanan ayna tedavisinin gece ağrısında
azalma ve fonksiyonellikte artış yönünden klasik egzersizlere üstün olduğunu
göstermektedir
The aim of this study was to evaluate the effect of
proprioceptive exercises combined with mirror therapy on pain, muscle strength,
functionality and kinesiophobia in patients with rotator cuff lesions. In this
randomised, controlled, and single-blinded prospective study patients were divided
into two groups. For a period of 6 weeks, 3 sessions per week, a classic exercise
program was applied to 16 patients in the first group and a proprioceptive exercise
program combined with mirror therapy was applied to 16 patients in the second
group. Patients were evaluated with Visual Analogue Scale (VAS), joint range of
motion (ROM) and manual muscle strength measurement, Shoulder Pain and
Disability Index (SPADI), Tampa Scale for Kinesiophobia (TSK), and the Constant
Shoulder Score (CS) before and after the treatment. At the end of the treatment; VAS
(rest, movement, night), ROM (flexion, abduction, active external rotation), shoulder
muscle strength measurement, SPADI (pain, disability, total), CS (pain, daily
activity, active ROM, total) scores improved in both groups (p<0,05). Significant
improvement was observed in active internal rotation, passive external rotation ROM
and TSK scores only in group 1, whereas a significant improvement was observed
only in the CS strength subheading in group 2 (p<0,05). As the groups compared
with each other, significant improvements were found in VAS night score and
SPADI (pain, disability, total) in the group receiving proprioceptive exercise and
mirror therapy (p<0,05). There was no difference between the groups with regard to
other measurements (p>0,05). Our results showed that both exercise methods are
effective in the treatment of symptomatic rotator cuff lesion and suggested that
proprioceptive exercises combined with mirror therapy is superior to classical
exercises in terms of decreased night pain and increased functionality
