HsTn’e ve risk skorlamalarına rağmen AKS şüphesi olan
hastalarda taburculuk sonrasında MKO gelişebilmektedir. Tezin amacı; acil servise
akut koroner sendrom şüphesi ile başvuran ve hsTn belirteci negatif olup taburcu
edilen hastalarda 1 ay içerisindeki MKO araştırılmasıdır. Retrospektif olarak 2016 ve
2019 yılları arasında başvuran 1919 hasta çalışmaya dahil edildi. Hastaların 56’sında
(%2,9) MKO gelişti. Yaş 10 yıl arttıkça MKO riski yaklaşık 2.72 kat artmış olarak
bulundu (p<0.001). Erkeklerde kadınlara göre MKO gelişme riski 2.07 kat daha fazla
olduğu görüldü (p=0.014). DM (p=0.04) ve dislipidemi (p<0.001) MKO gelişimini
arttıran diğer risk faktörleri idi. 3. Saat troponin değer ortalamaları MKO gelişim
riskini arttırması açısından istatistiksel olarak anlamlı bulundu (p=0.01). Gelişte (0.
Saat) ve 3. Saatte çalışılan hsTn düzeylerinin değer farkının ortalaması MKO
gelişimini arttırmadı (p=0.57). Yatak başı ekokardiyografide miyokardiyal kasılma
kusuru saptanması MKO gelişimini bağımsız olarak arttırdı(p=0.04). Sonuç olarak;
erkek cinsiyet, ileri yaş, diyabetes mellitus, dislipidemi ve ekokardiyografide
miyokardiyal kasılma kusuru saptanması troponin değeri normal olan hastalarda
taburculuk sonrasında MKO gelişimini arttıran risk faktörleridir. Normal referans
aralığındaki delta troponin değişimlerinin MKO gelişimini etkilemediği, normal
referans aralığındaki yüksek troponin değerlerinin MKO gelişim riskini arttırdığı
düşünülmektedir
Despite hsTn and risk classifications, MACE might
develop after discharge of patients with suspected acute coronary syndrome. The aim
of the thesis is; MACE is investigated within 1 month in patients who are admitted to
the emergency department with suspicion of acute coronary syndrome and hsTn
marker is negative and are discharged. Retrospectively, between 2016 and 2019,
1919 patients were included in the study. MACE developed in 56 patients (2.9%). As
the age increased by 10 years, MACE risk was found to be approximately 2.72 times
higher (p <0.001). The risk of developing MACE in men was 2.07-fold higher than
in women (p= 0.014). DM (p = 0.04) and dyslipidemia (p <0.001) were other risk
factors that increased the risk of development of MACE. Mean values of the 3rd hour
troponin were found to be statistically significant in terms of increasing the risk of
developing MACE(p=0.01). The mean of the difference in the value of hsTn studied
at arrival (0 hr) and at 3 hr did not increase the risk of development of MACE (p =
0.57). Detection of myocardial contraction defect on bedside echocardiography
increased the risk of development of MACE independently (p = 0.04). As a result;
male gender, advanced age, diabetes mellitus, dyslipidemia and myocardial
contraction defect on bedside echocardiography are risk factors that increase the risk
of development of MACE in patients with normal troponin valueafter discharge. It is
thought that troponin changes in the normal reference range do not affect the risk of
development of MACE and high troponin values in the normal reference range
increase the risk of developing MACE
