Kist hidatik ön tanılı hasta serumlarında elisa ve western blottiing yöntemleriyle antikor varlığının araştırılması

Abstract

Echinococcus granulosus’un etken olduğu kist hidatik (KH) hastalığı, gelişmekte olan ülkelerde ve ülkemizde yaygın bir zoonotik enfeksiyon olup, önemli bir halk sağlığı problemidir. KH hastalığı tanısı klinik, radyolojik ve serolojik bulgular birlikte değerlendirilerek konulmaktadır. Çalışmamızda KH ön tanısı ile girişimsel radyoloji bölümünden perkütan aspirasyonla alınan kist sıvıları laboratuvarımızda direkt mikroskobik inceleme ile değerlendirilmiştir. Değerlendirme sonrası, KH hastalığı ile uyumlu yapıların görüldüğü 16 hastadan serum örnekleri istenerek, ELISA ve Western Blotting testleriyle E. granulosus’ a karşı antikor varlığı araştırılmıştır. Bu çalışmada, KH hastalığının laboratuvar tanısında ELISA ve Wetern Blotting serolojik tanı yöntemlerinin birbirleriyle ve direkt mikroskobik incelemeyle karşılaştırılması ve laboratuvarımızda rutinde kullanılabilecek güvenilir ve hızlı bir tanı modeli oluşturulması amaçlanmıştır. 16 serum örneğinin 8’inde ELISA IgG pozitif, 3’ünde şüpheli pozitif, 5’inde negatif bulunmuştur. 16 serumun 14’ünde Western Blotting testi ile pozitif, 2 tanesi negatif sonuçlanmıştır. Direkt mikroskobik incelemenin altın standart olarak değerlendirildiği çalışmamızda sonuçlar istatistiksel olarak değerlendirilmiş ve ELISA testinin direkt mikroskobik inceleme ile uyumu %53.3, Western Blotting testinin uyumu %93.3 oranında bulunmuştur. Sonuç olarak; KH hastalığının serolojik tanısında Western Blotting yönteminin daha güvenilir olduğu ve mümkünse birden fazla serolojik yöntemin kombine edilmesinin tanıya katkı sağlayacağı düşünülmüştür

Cyst hydatid (CH) disease caused by Echinococcus granulosus is a common zoonotic infection in developing countries and our country and is an important public healthy problem. The diagnosis is made by evaluating clinical, radiological and serological findings. In our study, cyst fluids taken from the interventional radiology department with a initial diagnosis of CH were evaluated by direct microscobic examination in our laboratory. Serum samples were requested from 16 patients with structures compatible with CH disease, and the presence of antibodies againts E. granulosus was investigated by ELISA and Western Blotting tests. It was aimed to compare ELISA and Western Blotting serological diagnosis methods in the diagnosis of CH disease with each other and with direct microscobic examination and to create a reliable diagnostic model that can be used in our laboratory. Of the 16 serum samples, 8 were found to be ELISA IgG positive, 3 were suspected positive, 5 were negative. Of the 16 sera, specific antibody was detected by Western Blotting test and 2 were negative. In our study, in which direct microscobic examination was evaluated, as the gold standard, as the result were evaluated statistically and the compliance of the ELISA test with direct microscobic examination was found to be 53.3%, and the complience of Western Blotting test with 93.3%. As a result, it is thought that Western Blotting method is more reliable in the serological diagnosis of CH disease and combining more than one serological method will contribute to the diagnosis
