Bu çalışmanın amacı; benign semptomatik tiroid nodülü olan hastalarda
mikrodalga termal ablasyon (MDTA) yönteminin tedavi etkinliğini ve güvenilirliğini
değerlendirmektir. Bu amaçla Mart 2017- Kasım 2019 tarihleri arasında MDTA yöntemi
uygulanan ve 12 aylık takip süreçleri biten, 26 kadın, 12 erkek, toplam 38 hastanın
59 nodülü çalışmaya dahil edildi. Hastaların işlem öncesi ve işlem sonrası kontrol
görüntüleme bulguları, klinik skorlamaları, tiroid hormon ve otoantikor paneli retrospektif
olarak değerlendirildi. Hastaların yaş ortalaması 46.3 ± 13.2 idi. Tüm nodüller
%100 teknik başarı ile tedavi edildi. Klinik başarı oranı % 96.6 idi. 59 nodülün 57 tanesinde
belirgin küçülme saptandı. 2 farklı hastadaki 2 nodülde ise nüks lehine anlamlı
olabilecek revaskülarite ve hacimsel azalmada duraksama izlendi. Nüks kabul
edilen olgularda elastografik ECI değerinde diğer hastaların aksine istatiksel anlamlı
artış izlenmedi. US ile ölçülen ortalama hacimde işlem sonrası 12. ayda işlem öncesine
göre % 76.6 oranında azalma izlenmiş olup istatistiksel olarak anlamlı fark saptandı
(p<0.01). Hastaların hiçbirisinde major komplikasyon izlenmezken, 8 hastada
(%21) minör komplikasyon izlendi. Hastaların tamamında tiroid hormon ve otoantikorları
normal sınırlarda kalmış olup tiroid hormon replasmanı gereken olgu izlenmedi.
Olguların GAS ve KS skorlarında işlem sonrasında istatiksel olarak anlamlı
azalmalar kaydedildi. Sonuç olarak semptomatik benign tiroid nodüllerinin MDTA
yöntemi ile tedavisi etkili ve güvenilir bir yöntemdir. Diğer ablasyon yöntemlerine
göre daha büyük ablasyon zonu sağlaması, daha hızlı yapılabilmesi gibi avantajları
vardır. Ayrıca bu yöntemin cerrahi işleme kıyasla daha az komplikasyon, daha iyi
kozmetik sonuçlar ve işlem sonrası tiroid hormon replasmanı gerektirmemesi gibi
avantajları olup gelecekte benign semptomatik tiroid nodüllerinin tedavisinde MDTA
belki de ilk seçenek olacaktır
The aim of this study is to evaluate the efficacy and reliability of microwave
thermal ablation (MWTA) method in patients with benign symptomatic thyroid
nodules. For this purpose, 59 nodules of 38 patients, 26 females, 12 males, who
underwent the MDTA method between March 2017 and November 2019 and whose
12-month follow-up period was completed, were included in the study. Pre- and
post-procedure control imaging findings, clinical scores, thyroid hormone and autoantibody
panel of the patients were evaluated retrospectively. The mean age of the
patients was 46.3 ± 13.2 years. All nodules were treated with 100% technical success.
The clinical success rate was 96.6%. Significant shrinkage was detected in 57
of 59 nodules. In 2 nodules in 2 different patients, revascularity, which could be significant
in favor of recurrence, and a pause in volumetric reduction were observed.
Unlike the other patients, no statistically significant increase was observed in the
elastographic ECI value in patients with relapse. In the 12th month after the procedure,
the mean volume measured by US decreased by 76.6% compared to the preprocedure,
and a statistically significant difference was found (p <0.01). While none
of the patients had major complications, 8 patients (21%) had minor complications.
Thyroid hormone and autoantibodies remained within normal limits in all patients,
and no patient requiring thyroid hormone replacement was observed. Statistically
significant decreases were recorded in VAS and CS scores after the procedure. In
conclusion, the treatment of symptomatic benign thyroid nodules with MDTA method
is an effective and safe method. It has advantages such as providing a larger ablation
zone and being performed faster than other ablation methods. In addition, this
method has advantages such as fewer complications compared to the surgical procedure,
better cosmetic results and not requiring thyroid hormone replacement after the
procedure, and MWTA may be the first choice in the treatment of benign symptomatic
thyroid nodules in the future
