Hepatit C
virüsü enfeksiyonları (HCV) tüm dünyada yüksek kronikleşme riski, henüz aşılama
ile korunulması mümkün olmaması ve hepatoselüler karsinom gibi ciddi
komplikasyonları nedeniyle önemli bir halk sağlığı sorunudur. Bu araştırmada HCV
kor antijen testinin tanı ve tedavi takibinde HCV RNA ile korelasyonu
değerlendirildi, rutin kullanımda alternatif bir test olma durumu tartışıldı.
Çalışmamızın ilk basamağında Aralık 2016-Aralık 2018 tarihleri arasında Tıbbi
Mikrobiyoloji ABD Moleküler laboratuvarında, HCV ön tanılı hastalardan alınan,
rutin olarak Anti-HCV ve HCV-RNA testi çalışılmış 600 serum örneğinde, HCV kor
antijen testi çalışıldı. İkinci basamakta ise bu zaman dilimi içerisinde
Gastroenteroloji bölümü tarafından tedavisine karar verilen 150 hastanın tedaviye
başlamadan önce, 8. haftada ve tedavi bitiminde alınan serum örneklerinde rutin
olarak çalışılan HCV-RNA testine ilave olarak HCV kor antijen testi çalışılardı.
Tedavi takibi sürecinde iki testin korelasyonu değerlendirildi. Toplam 600 hastanın
49' una test sonuçlarına göre tanı konuldu. Bunlardan 28 hastada, rutin çalışılan HCV
RNA ve Anti HCV'nin yanı sıra HCV kor antijeni pozitif bulundu. HCV kor antijen
testinin duyarlılığı %91.49, özgüllüğü %100, PPD %100, NPD %97.30, doğruluk
oranı ise %87.76 olarak tespit edildi. HCV RNA ve HCV kor antijeni sonuçları
arasında yüksek düzeyde korelasyon saptandı. Çalışmanın ikinci basamağında ise
HCV kor antijen testinin duyarlılığı (%96.52), özgüllüğü (%95.28), PPD (%95.11),
NPD (%95.80) ve doğruluğu (%92.58) hesaplandı. Bu sonuçlar iki test arasında
yüksek oranda korelasyonun olduğunu, tanı ve tedavi takibi sırasında kolay
uygulanabilir ve maliyet etkin bir test olması nedeniyle HCV RNA testine alternatif
bir test olarak kullanılabileceğini göstermektedir
Hepatitis C virus (HCV) infections are an important
public health issue across the world because of the high risk of chronicity potential,
impossibility of protection by vaccination and serious complications such as
hepatocellular carcinoma. In this study, the correlation of HCV core antigen test with
HCV RNA in the diagnosis and treatment follow-up was evaluated and the status of
being an alternative test in routine use was discussed. In the first step of our study,
between December 2016 and December 2018, HCV core antigen test was studied in
600 serum samples taken from patients with HCV prediagnosis routinely tested in
Anti-HCV and HCV-RNA tests in Molecular Laboratory of Medical Microbiology
Department. In the second step, the HCV core antigen test was studied in addition to
the routine HCV-RNA test performed in the serum samples taken before, 8 weeks
and at the end of the treatment of 150 patients who were decided to be treated by the
Gastroenterology Department between the dates indicated. The correlation between
the two tests was evaluated during the treatment follow-up. 49 of 600 patients were
diagnosed according to test results. In 28 patients, HCV core antigen was positive in
addition to HCV RNA and Anti HCV which were routinely studied. The sensitivity
of HCV core antigen test was 91.49%, specificity was 100%, PPD was 100%, NPD
was 97.30%, accuracy was 87.76%. There was a high correlation between HCV
RNA and HCV core antigen results. In the second step of the study, sensitivity
(96.52%), specificity (95.28%), PPD (95.11%), NPD (95.80%) and accuracy
(92.58%) of the HCV core antigen test were calculated. These results show that there
is a high correlation between the two tests and can be used as an alternative test to
HCV RNA test because it is an easily applicable and cost effective test during
diagnosis and treatment follow-up
