Bu çalışmanın amacı konik ışınlı bilgisayarlı tomografi (KIBT) ile mandibular lingulanın, mandibula iskeletsel paternleri arasındaki anatomik ilişkisini ve ramus kalınlığını değerlendirerek, Sagittal Split Ramus Osteotomisi (SSRO) sırasında oluşabilecek komplikasyonları minimalize etmek, böylelikle operasyonların daha hızlı, güvenli ve kontrollü yapılmasını sağlamaktır.
Materyal ve Metot: Bu çalışmaya 66’sı sınıf I, 98’i sınıf II, 86’sı sınıf III maloklüzyona sahip 250 hasta dahil edildi. Mandibular lingula ile internal oblik ridge (L-İOR), anterior ramus (L-AR), posterior ramus (L-PR), oklüzal plan (L-OP), sigmoid çentik (L-N) arasındaki mesafe ve ramusun B1/3, B2/3, B3/3, BL1/3, BL2/3, BL3/3, L1/3, L2/3 ve L3/3 bölgesindeki kalınlığı ölçüldü. Yapılan ölçümler cinsiyetler, maloklüzyonlar ve lingula tipleri arasında karşılaştırıldı.
Bulgular: Cinsiyetler arasında L-N, L-P, L-M2, L-OP, B3/3, BL2/3, BL3/3 değerlerinde istatistiksel olarak fark (p<0.05) bulundu. L-N, L-A, L-P, L-M2, L-OP değerleri erkeklerde kadınlardan daha yüksek bulundu. L-İOR, L-M2, L-OP, B1/3, B2/3, B3/3, BL1/3, BL2/3, BL3/3, L1/3, L2/3 değerleri maloklüzyonlar arasında karşılaştırıldığında istatistiksel olarak fark (p<0.05) görüldü. Lingula tipleri arasında karşılaştırmada gruplar arasında istatistiksel olarak anlamlı bir fark (p>0.05) görülmedi.
Sonuç: SSRO’da medial osteotomi hattı erkek hastalarda ve sınıf III maloklüzyona sahip hastalarda daha yukarıdan olmalı, ayrıca bu hastalarda ramusun mediolateral kalınlığı daha az olduğundan komplikasyonları önlemek için daha dikkatli davranılmalıdır
The aim of this study is to minimize the complications that may occur during Sagittal Split Ramus Osteotomy (SSRO) by evaluating the anatomical relationship between cone beam computed tomography (CBCT) and the skeletal patterns of the mandible and the lingula, and the thickness of the ramus with CBCT, therefore to ensure faster, safer and controlled operations.
Material and method: 250 patients with 66 class I, 98 class II, and 86 class III malocclusions were included in this study. The distance between lingula and internal oblique ridge (L-IOR), anterior ramus (L-AR), posterior ramus (L-PR), occlusal plane (L-OP), the sigmoid notch (LN), and the ramus in the B1/3, B2/3, B3/3, BL1/3, BL2/3, BL3/3, L1/3, L2/3, and L3/3 regions thickness was measured. Measurements were compared between genders, malocclusion, and lingula types.
Results: LN, LP, L-M2, L-OP, B3/3, BL2/3, BL3/3 values were statistically significant differences between genders (p<0.05). LN, LA, LP, L-M2, L-OP values were found higher in males than females. L-IOR, L-M2, L-OP, B1/3, B2/3, B3/3, BL1/3, BL2/3, BL3/3, L1/3, L2/3 values were statistically significant differences found between malocclusions (p<0.05). No statistically significant difference was found between lingula types groups.
Conclusion: In SSRO, the medial osteotomy line should be higher in male patients and in patients with class III malocclusion. Since the mediolateral thickness of the ramus is less in male patients and patients with class III malocclusion, more care should be taken to prevent complications
