Amaç
Bu araştırmada amacımız deneysel olarak indüklenen periodontitis varlığında
sistemik olarak uygulanan Eritropoetin (EPO)'nin periodontal dokulardaki inflamasyon
ve kemik kaybı üzerindeki etkilerini gözlemlemek ve terapötik etkisini araştırmaktır.
Gereç ve Yöntem
Çalışmada 250-300 gram ağırlığında 50 adet 12-14 haftalık erkek Sprague Dawley
rat kullanılmıştır. Ratlar beş eşit gruba ayrıldı: Kontrol grubu (K, n=10), Periodontitis
grubu (P, n=10), Kontrol+Darbepoetin alfa 0.25 µg/kg/gün grubu (K+EPO 0.25, n=10),
Periodontitis+Darbepoetin alfa 0.1 µg/kg/gün grubu (P+EPO 0.1, n=10) ve
Periodontitis+Darbepoetin alfa 0.25 µg/kg/gün grubu (P+EPO 0.1, n=10). Deneysel
periodontitis oluşturmak için ratların sağ mandibular 1.moları dişlerine 4/0 ipek ligatürler
submarjinal olarak yerleştirildi. 11 günlük EPO uygulamasından sonra ratlar sakrifiye
edildi. Ratlardan dişeti örnekleri ve mandibula total olarak alınmıştır. Mikro-bilgisayarlı
tomografi (Mikro-BT) analizi yaparak alveoler kemik kaybı miktarı mandibular 1.molar
ve 2.molar arasında interproksimal bölgede mine-sement sınırından alveoler kret arası
mesafeler ölçülerek hesaplanmıştır. Gruplardan alınan diğer kemik örneklerinde
histolojik yöntemlerle osteoklast hücre sayımı yapılmıştır ve immünhistokimyasal
yöntemlerle apoptoz oranı hesaplanmıştır. qRT-PCR yöntemi ile EPO, EPO-R, TNF-α,
IL-1β ve IL-6 mRNA düzeyleri ölçülmüştür.
Bulgular
Mikro-BT analizi sonuçlarına göre, P+EPO 0.1 (0,7980±0,05978) ve P+EPO 0.25
(0,7720±0,03967) gruplarında kemik kaybı miktarı periodontitis (1,2720±0,07729)
grubuna göre istatistiksel olarak anlamlı düzeyde azalmıştır (p<0.05). Yapılan histolojik
analizlerin sonucuna göre, P+EPO 0.1 ve P+EPO 0.25 gruplarında osteoklastik aktivite
periodontitis grubuna göre istatistiksel olarak anlamlı düzeyde düşük bulunmuştur
(p<0.05). İmmünhistokimyasal analizin sonuçlarına göre TUNEL boyaması
incelendiğinde, P+EPO 0.1 (7,060±4,076) ve P+EPO 0.25 (10,19±9,746) gruplarında
apoptotik hücre indeksi periodontitis (44,93±16,86) grubuna göre istatistiksel olarak
anlamlı düzeyde düşük bulunmuştur (p<0.05). qRt-PCR yöntemi analizi sonucunda,
P+EPO 0.1 ve P+EPO 0.25 gruplarında TNF-α, IL-1β ve IL-6 gen ekspresyon düzeyleri
kontrol grubuna yakın bulunmuştur (p>0.05). EPO-R gen ekspresyon düzeyleri ise
P+EPO 0.1 ve P+EPO 0.25 gruplarında periodontitis grubuna göre istatistiksel olarak
anlamlı olmasa da yüksek çıkmıştır (p>0.05).
Sonuç
Bu çalışmanın sonucunda EPO’nin anti-inflamatuar, antiapoptotik ve sitoprotektif
etkileriyle periodontal hastalık esnasında perioodontal dokuları koruyabileceğini
söyleyebiliriz
In this study, our aim is to observe and investigate the therapeutic effect of
Erythropoietin (EPO) systemically applied in the presence of experimentally induced
periodontitis on inflammation and bone loss in periodontal tissues.
Material and Method
In this study, fifty 12-14 week old male Sprague Dawley rats weighing 250-300
grams were used. Rats were divided into five equal groups: Control group (K, n = 10),
Periodontitis group (P, n = 10), Control + Darbepoetin alpha 0.25 µg / kg / day group (K
+ EPO 0.25, n = 10), Periodontitis + Darbepoetin alpha 0.1 µg / kg / day group (P + EPO
0.1, n = 10) and Periodontitis + Darbepoetin alpha 0.25 µg / kg / day group (P + EPO 0.1,
n = 10). To create experimental periodontitis, 4/0 silk ligatures were placed submarginally
in the right mandibular 1st molar teeth of the rats. After 11 days of EPO application, the
rats were sacrificed. Gingival samples and mandible were collected from rats. By micro computed tomography (Micro-CT) analysis, the amount of alveolar bone loss was
calculated by measuring the distances between the mandibular 1st molar and the 2nd
molar between the alveolar crest in the interproximal region. In other bone samples taken
from the groups, osteoclast cell counts were performed by histological methods and the
rate of apoptosis was calculated by immunohistochemical methods. EPO, EPO-R, TNF α, IL-1β and IL-6 mRNA levels were measured by qRT PCR method.
Results
According to micro-computed tomography (Micro-ct) analysis, bone loss amount
in group P+ EPO 0.1 (0.7980 ± 0.05978) and group P+ EPO 0.25 (0.77720 ± 0.03967)
was decreased significantly compared to the periodontitis group (1,2720 ± 0,07729)
(p<0.05). According to the results of histological analysis, the osteoclastic activity is
significantly lower in the P+ EPO 0.1 and P+ EPO 0.25 groups compared to the
periodontitis group (p<0.05). According to the results of the immunohistochemical
analysis, apoptotic cell index was statistically significantly lower in the P+ EPO 0.1
(7,060 ± 4,076) and P+ EPO 0.25 (10,19 ± 9,746) groups compared to the periodontitis
group (44.93 ± 16.86) (p <0.05). According to the results of qRt-PCR method analysis,
TNF-α, IL-1β and IL-6 gene expression levels were found close to the control group in
P+ EPO 0.1 and P+ EPO 0.25 groups (p>0.05). EPO-R gene expression levels were higher
in P+ EPO 0.1 and P+ EPO 0.25 groups, although not statistically significant compared
to the periodontitis group (p>0.05).
Conclusion
As a result of this study, we can say that EPO can protect the perioodontal tissues
during periodontal disease with its anti-inflammatory, antiapoptotic and cytoprotective
effects
