Bu çalışmanın amacı, implant üstü sabit protezlerde meydana gelen
komplikasyonların demografik ve protez çeşitlerine bağlı değişkenlerle ilişkisini,
nedenlerini ve meydana gelen kemik kayıplarını 2 yıl süreyle retrospektif olarak
incelemektir.
Gereç ve Yöntem: Eskişehir Osmangazi Üniversitesi Diş Hekimliği Fakültesi
Protetik Diş Tedavisi Ana Bilim Dalı’na Ekim 2013 ile Ekim 2018 tarihleri arasında
implant destekli sabit protez ihtiyacı ile başvuran ve implant cerrahisi tamamlanıp, sabit
protezler ile yüklenmesinden itibaren 24 ayı doldurmuş olan hastaların bilgileri arşivden
taranarak, kayıt edilmiş komplikasyon verilerine ve röntgen bilgilerine ulaşıldı. Çalışma,
240 hastaya yerleştirilen 286 Astra (Astra Implant Systems, AstraTech, Mölndal,
Sweden) ve 378 Biohorizons/Birmingham,Alabama, USA) olmak üzere toplam 664
implantla gerçekleşti. Porselen atması, porselen kırılması, vida gevşemesi, vida kırılması
ve desimantasyon gibi mekanik komplikasyonların yaş, cinsiyet, lokalizasyon, implant
markası, protez türü ve implantların tek ya da birbirine bağlı olma durumlarıyla olan
ilişkisinin retrospektif olarak incelenerek, panoramik grafilerden 12. ve 24. ayda kemik
kayıpları değerlendirildi. Elde ettiğimiz veriler değerlendirilirken, istatistiksel analiz için
SPSS for Windows yazılımı (IBM, 2013, sürüm 20.0), %95 güven aralığında p< 0.05
kriteri esas alınarak yapıldı.

Bulgular: 133’ü kadın 107’si erkek olan hastaların yaş aralıkları 18-74 olmakla
birlikte ortalaması 49,8’dir. Yerleştirilen 664 implantın 321’i kadın hastaya, 343’ü erkek
hastaya yerleştirildi. 12. ve 24. ay değerlendirmesinde komplikasyon varlığı ile yaş,
protez türü, implant lokalizasyonu, implant markası ve implant desteği açısından
istatistiksel olarak anlamlı bir fark bulunamazken, 24. ayda erkek hastalarda istatistiksel
olarak anlamlı olarak daha fazla komplikasyon varlığı gözlendi. 12. ve 24. ay
değerlendirmelerinde protez dayanaklarında, protez gövdesine göre yüksek oranda
komplikasyon belirlendi. 12. ayda en yüksek oranda gözlenen komplikasyon çeşitleri
sırasıyla porselen atması, porselen kırılma ve vida gevşemesiyken, 24. ayda en yüksek
orana sahip komplikasyon porselen kırılmasıdır. Kemik kaybı ile cinsiyet, yaş, protez
çeşidi, implant markası, implant lokalizasyonu arasında istatistiksel olarak anlamlı bir
sonuç bulunamazken, 24. ayda 12. aya göre istatistiksel olarak anlamlı bir şekide daha
fazla kemik kaybı gözlendi ve 24. ayda kaydedilen komplikasyon varlığıyla, kemik kaybı
arasında anlamlı bir ilişki gösterildi.
Sonuç: Çalışmamızdan çıkardığımız sonuç, komplikasyonların önlenmesi ve
azaltılmasına katkı sağlamak amacı ile, implant üstü köprü gibi çok üyeli sabit protez
yaptığımız hastaları ilk yıl sonu yerine daha kısa periyodlarda kontrollere çağırmak,
okluzyon üzerinde kontrol sağlayarak ve komplikasyonlar açısından hastaları
bilgilendirerek, komplikasyon riskini minimuma indirmektir
The aim of this study was to retrospectively examine the relationship
between the demographic and prosthesis types of complications that occur in implant
supported fixed prosthesis, their causes and the resulting bone losses for 2 years.
Material and Method: The information of patients who applied to Eskişehir
Osmangazi University Faculty of Dentistry Department of Prosthodontics with the need
for implant-supported fixed dental prosthesis between October 2013 and October 2018
has been reached. The study was carried out with a total of 664 implants, 286 Astra (Astra
Implant Systems, AstraTech, Mölndal, Sweden) and 378 Biohorizons (Birmingham,
Alabama, USA) placed in 240 patients. The relationship of mechanical complications
such as veneer fracture, porcelain chipping, abutment screw loosening, abutment screw
fracture and desimantation with age, gender, localization, implant brand, prosthesis type,
and single or splinted implants were retrospectively examined and bone loss measured
with X20 magnification using Corel Draw 11.0 software at 12 and 24 months. While
evaluating the data we obtained, SPSS for Windows software (IBM, 2013, version 20.0)
for statistical analysis was performed on the basis of p <0.05 criterion at 95% confidence
interval.
Results: Although the age range of 133 women and 107 men was between 18-74,
the average was 49.8, and 321 of 664 implants were placed in female patients and 343

were in male patients. While there was no statistically significant difference in the
presence of complications and age, prosthesis type, implant location, implant brand and
implant support in the 12th and 24th month evaluation, statistically significantly more
complications were observed in male patients at the 24th month. In the 12th and 24th
month evaluations, a higher rate of complications in the prosthesis abutments compared
to the prosthetic body was detected. The most common types of complications observed
in the 12th month were porcelain chipping, porcelain fracture and screw loosening,
respectively, while porcelain fracture had the highest rate in the 24th month. While no
statistically significant result was found between bone loss and gender, age, prosthesis
type, implant brand, implant location, a statistically significant higher bone loss was
observed at 24 months compared to 12 months, and with the presence of complications
recorded at 24 months, bone loss a significant relationship was shown between.
Conclusion: The conclusion we have drawn from our study is to call patients for
whom we have made multi-member fixed prosthesis such as implant supported bridge for
controls in shorter periods instead of the end of the first year in order to contribute to the
prevention and reduction of complications, by providing control over occlusion and
informing patients in terms of complications, to minimize the risk of complications.
