Fonksiyonel tek böbrekli (FTB) çocuklar başlangıçta hiperfiltrasyon ve kompansatris
hipertrofi ile böbrek fonksiyonlarını korurken, zamanla böbrek hasarına bağlı
hipertansiyon, proteinüri ve kronik böbrek hastalığı gelişimi açısından risk altındadır.
Bu çalışmada doğumsal ve edinsel FTB’li çocuklarda mikroalbuminüri ile glomerül
filtrasyon hızı (GFH) ve hipertansiyon arasındaki ilişkinin değerlendirilmesi
amaçlandı. Çalışmada, Eylül 2009-Eylül 2018 yılları arasında Eskişehir Osmangazi
Üniversitesi Tıp Fakültesi Çocuk Nefroloji Bilim Dalında izlenen 5-18 yaş arasındaki
59 doğumsal FTB’li, 41 edinsel FTB’li hasta ile 26 sağlıklı çocuğun dosya verileri
incelendi. Doğumsal FTB’li hastalar ‘doğumsal’, edinsel FTB’li hastalar ‘edinsel’ ve
sağlıklı çocuklar ‘kontrol’ grubu olarak isimlendirildi. Doğumsal, edinsel ve kontrol
grubunun böbrek fonksiyon testleri, spot idrar miroalbumin/kreatinin düzeyi, GFH’si,
ofis kan basıncı ve yaşam içi kan basıncı ölçümleri değerlendirildi. Doğumsal, edinsel
ve kontrol grubunda yaş, boy, vücut ağırlığı, vücut kitle indeksi, kan üre azotu,
kreatinin ve GFH değerleri açısından anlamlı farklılık saptanmadı (p>0,005). Edinsel
grupta spot idrar mikroalbumin/kreatinin oranı, doğumsal ve kontrol grubundan
anlamlı olarak yüksek bulundu (p<0,001). Doğumsal, edinsel ve kontrol grubunda ofis
kan basıncı ölçümleri arasında farklılık görülmezken, total ortalama diyastolik ve gece
diyastolik kan basıncı değerleri edinsel grupta, doğumsal ve kontrol grubundan
anlamlı yüksek bulundu (hepsi için p<0,001). Edinsel grupta hipertansiyon, diyastolik
hipertansiyon ve non-dipper görülme sıklığının doğumsal gruptan daha fazla olduğu
görüldü (sırası ile p<0,001, p<0,001, p=0,003). Edinsel FTB’li hastalar hipertansiyon
ve mikroalbuminüri açısından yakın takip edilmelidir. Hipertansiyon ve
mikroalbuminürinin yakın izlem ile erken dönemde farkedilmesi antihipertansif ve
antiproteinürik tedavilerin başlanarak kronik böbrek hastalığının ilerlemesinin
yavaşlatılmasında yardımcı olabilir

(SFK) initially maintain kidney function with hyperfiltration and compensatory
hypertrophy, they are at risk for the development of hypertension, proteinuria and
chronic kidney disease due to kidney damage over time. Aim of this study is to
evaluate the relationship between microalbuminuria, glomerular filtration rate (GFR)
and hypertension in children with congenital and acquired SFK. In this study, the data
of 59 congenital SFK patients, 41 acquired SFK patients and 26 healthy children
between the ages of 5-18, who were followed up in Eskişehir Osmangazi University
Faculty of Medicine, Department of Pediatric Nephrology between September 2009
and September 2018 were examined. Patients with congenital SFK were tagged as
'congenital', while patients with acquired SFK as 'acquired' and healthy children as
'control' group. Renal function tests, spot urine microalbumin/creatinine level, GFR,
office blood pressure and ambulatory blood pressure measurements of all groups were
evaluated. There were no significant differences in age, height, body weight, body
mass index, blood urea nitrogen, creatinine and GFR values among various groups
(p>0,005). Spot urinary microalbumin/creatinine ratio in the acquired group was
significantly higher than the congenital and control group (p<0,001). No significant
difference was observed between office blood pressure measurements among the
groups, however, total mean diastolic and nocturnal diastolic blood pressure values
were significantly higher in the acquired group than in the congenital and control group
(p<0,001 for all). The incidence of hypertension, diastolic hypertension, and non dipper was found to be higher in the acquired group than in the congenital group
(p<0,001, p<0,001, p=0,003, respectively). Hence, patients with acquired SFK should
be followed closely for hypertension and microalbuminuria. Early detection of
hypertension and microalbuminuria with close monitoring may slow the progression
of chronic kidney disease by starting antihypertensive and antiproteinuric treatments
