Akut kolanjit safra
ağacının akut inflamasyonu ve enfeksiyonudur. En sık iki nedeni koledokolithiyazis
ve benign biliyer darlıktır. Drenaja ek olarak antibiyoterapi ve destek tedaviye yanıt
veren seyir yanında ciddi sepsis ve çoklu organ yetmezliği sonucunda ölümle
sonuçlanan seyir de gösterebilir. Akut kolanjitin temel tedavisi biliyer drenaj olmakla
birlikte en kısa zamanda ampirik antibiyotik başlanmalıdır. Biliyer drenaj sırasında
safra örneği alınıp kültür çalışılmalı, kültürde üreyen etken ve antibiyogramına göre
antimikrobial ajan devamına karar verilmelidir. Biz de çalışmamıza kolanjit nedeni
ile ERCP yapılmış 163 hastayı retrospektif olarak hastane kayıtlarından taradık.
Hastaların safra kültürlerinde üreyen ajanları ve bu ajanların antimikrobiyal
duyarlılıklarını; kolanjit şiddeti, mortalite, hastanede yatış süresi, akut faz reaktanları
ile ilişkisini inceledik. Çalışmamızın sonucunda tüm kolanjitli hasta populasyonunda
Escherichia Coli (E.Coli)’nin safra kültüründe en sık üreyen ajan olduğunu;
Enterococcus Faecium (Ent.Faeceum) ile kolanjit şiddeti arasında anlamlı ilişki
olduğunu; Tokyo 2018 kolanjit kriterlerine göre hesaplanan kolanjit şiddeti ve safra
kültüründe Klebsiella Oxytoca (K.Oxytoca) üremesi olmasının hastanede yatış süresi
ile ilişkili olduğunu; kolanjit şiddeti, semptom başlangıcı ile biliyer drenaj arasındaki
sürenin uzunluğu, koledokolithiyazis dışı etyoloji varlığı ve daha önce kolanjit
nedeni ile biliyer girişim öyküsünün olmasının mortalite üzerine anlamlı etkisi
olduğunu tespit ettik
Acute cholangitis is an acute inflammation and infection of the
biliary tree. The two most common causes are choledocholithiasis and benign biliary
stricture. First treatment option is biliary drainage. In addition to drainage, it may
show a course that responds to antibiotherapy and supportive treatment, as well as
severe sepsis and death resulting in multiple organ failure. Although the main
treatment of acute cholangitis is biliary drainage, empirical antibiotics should be
started as soon as possible. During biliary drainage, bile sample should be taken and
culture should be studied, and the continuation of antimicrobial agent should be
decided according to the antibiogram and the reproductive agent. In our study we
retrospectively scanned 163 patients who underwent ERCP for cholangitis from their
hospital records. We examined the frequency of agents reproducing in bile cultures of
patients and their antimicrobial susceptibility, cholangitis severity, mortality, length
of hospital stay, and their relationship with acute phase reactants. As a result of our
study, we found that in all patients with cholangitis, E. Coli was the most common
agent in bile culture; there is a significant relationship between Ent. faecium and the
severity of cholangitis; The severity of cholangitis which calculated according to the
Tokyo 2018 cholangitis criteria and the presence of K. Oxytoca growth in the bile
culture were related to the length of hospital stay; cholangitis severity, the length of
time between the onset of symptoms and biliary drainage, candida spp. reproduction
and previous history of biliary intervention due to cholangitis had a significant effect
on mortality
