HBV enfeksiyonu çözülmüş hastalarda (hepatit B yüzey antijeni (HBsAg) negatif, hepatit B çekirdek antikoru (anti-HBc) pozitif ve hepatit B yüzey antikoru pozitif) hepatit B virüsünün (HBV) reaktivasyonunu önlemek için kemoterapi veya immünosupresif tedaviden önce antiviral profilaksinin rolü açık değildir. Bu çalışma, kemoterapi veya immünosüpresif tedavi alan resolved HBV (HBsAg-negatif, anti-HBc-pozitif ve Anti-HBs pozitif) olgularinda reaktivasyon oranını ve profilaktik antiviral tedavinin etkinliğini değerlendirmeyi amaçlamaktadır. 2010-2019 yılları arasında tek bir üniversite hastanesinde kemoterapi veya immünosupresif tedavi gören HBsAg-negatif, anti-HBc-pozitif ve Anti-HBs pozitif hastaların tıbbi kayıtlarını geriye dönük olarak değerlendirdik. Hastaların %54,5’ü (n=486) erkek, %45,6’sı (n=408) kadın, yaş ortalaması 61,83±12,73’dür. Çalışmaya dahil edilen hastaların %11,5’nin (n= 103) HBV reaktivasyon riski yüksek derecede, %74,8’in (n=669) orta, %13,6 (n=122) düşük derecededir. 34 (%3,8) hastaya immünosüpresif tedavi sırasında HBV reaktivasyonu teşhisi konuldu. Tanı grupları ile HBV reaktivasyonu arasında istatistiksel olarak anlamlı bir ilişki vardı. HBV reaktivasyonu gelişen hastaların %11,4 (n=24) hematolojik malıgnıte, %1,6 (n=7) solid organ kanserleri, %6,9 (n=2) immünosüpressif tedavi alan nefroloji, %1,5 (n=1) ve dermotolojik tanısı olan hastalarıdır (p:0,01<0,05). Toplam hasta sayısının %2,6’ı (n=23) oluşturan kök hücre nakli olanların %50’sinde (n=11) HBV reaktivasyonu gelişmiştir (p:0,01<0,05). HBV reaktivasyonu gelişen hastaların kullandığı immünosüpresif ajanların risk gruplarına göre dağılımında istatistiksel olarak anlamlı farklılık mevcut (p:0,01<0,05). Reaktivasyon gelişen yüksek risk grubu hastalarında tedavide en çok %36,8 (n=7) Rituksimab bazlı kemoterapiler, orta risk grubu hastalarda ise en %40’ı (n=6) platin grubu ajanlar kullanılmıştır. HBV profilaksisi durumu ile risk grupları arasında istatistiksel olarak anlamlı bir ilişki vardır (p:0,01<0,05). Yüksek risk grubu hastalarında %86,1 (n=31) Rituksimab tedavisi görenlere profilaksi verilmişdir. Yapılan ikili karşılaştırmalar sonucunda yüksek risk grupunda Anti -HBs değerleri, düşük ve orta risk grubu değerlerine göre istatistiksel olarak anlamlı biçimde daha düşüktür (p<0,016). HBV reaktivasyonu durumuna göre HBV reaktivasyonu olmayanların Anti-HBs değerleri, HBV reaktivasyonu olanların medyan değerlerine bakıldığında daha yüksektir (p<0,05).HBV reaktivasyonu gelişen hastaların ALT, AST, GGT direkt ve total bilirubin değerlerinde zaman diliminde anlamlı derecede fark vadır (p<0,05)
In patients with resolved HBV infection (hepatitis B surface antigen (HBsAg) negative, hepatitis B core antibody (anti-HBc) positive and hepatitis B surface antibody positive) the role of antiviral prophylaxis before chemotherapy or immunosuppressive therapy to prevent reactivation of hepatitis B virus (HBV) isunclear. This study aims to evaluate the reactivation rate and the effectiveness of prophylactic antiviral therapy in resolved HBV (HBsAg-negative, anti-HBc-positive and Anti-HBs positive) cases receiving chemotherapy or immunosuppressive therapy. Between the years 2010-2019 in a single university hospital receiving chemotherapy or immunosuppressive therapy HBsAg-negative, anti-HBc-positive and anti-HBs positive patients and their medical records were retrospectively evaluated. 54.5% (n = 486) of the patients were male, 45.6% (n = 408) were female the average age was 61.83 ± 12.73. 11,5 % of the patients included in the study (n = 103) a high degree of risk of HBV reactivation, 74.8% (n = 669) medium, 13.6% (n = 122) is at a low degree HBV reactivation was diagnosed in 34 (3.8%) patients during immunosuppressive therapy. There was a statistically significant relationship between the diagnostic groups and HBV reactivation. Of the patients who developed HBV reactivation, 11.4% (n =24) hematological malignancy, 1.6% (n=7) solid organ cancers, 6.9% (n =2) nephrology receiving immunosuppressive therapy, 1.5% (n =1) and patients with a diagnosis of dermatology (p: 0.01 <0.05). HBV reactivation developed in 50% (n =11) of those with stem cell transplantation, which constitutes 2.6% (n =23) of the total number of patients (p: 0.01 <0.05). There is a statistically significant difference in the distribution of immunosuppressive agents used by patients with HBV reactivation according to risk groups (p: 0.01 <, 05). Rituximab-based chemotherapies were used the most in 36.8% (n =7) in the treatment of high-risk group patients who developed reactivation, and platinum group agents were used at most 40% (n =6) in medium-risk group patients. There is a statistically significant relationship between HBV prophylaxis status and risk groups (p: 0.01 <0,05). In the treatment of high risk patients developing reactivation most 36.8% (n =7) rituximab-based chemotherapy, while 40% of patients at intermediate risk group (n=6) kullanılmışd agents are platinum group. There is a statistically significant relationship between HBV prophylaxis status and risk groups (p: 0.01 <0,05). Prophylaxis was given to 86.1% (n=31) of high risk group patients who received rituximab treatment. As a result of paired comparisons, Anti-HBs values in the high risk group are statistically significantly lower than the low and medium risk group values (p <0.016). According to the HBV reactivation status, the Anti-HBs values of those without HBV reactivation are higher when looking at the median values of those with HBV reactivation (p <0.05). Patients with HBV reactivation show a significant difference in ALT, AST, GGT direct and total bilirubin values (p <0.05)
