HLA-B27
geninin, insanlarda Ankilozan Spondilit (AS) geliştirdiğine inanılsa da bu konu hala
tartışmalıdır. Bu çalışma, HLA-B27 negatif AS hastalarının klinik, laboratuar verileri,
radyolojik özellikleri ve tedavilerinin nasıl olduğunu değerlendirmek ve HLA-B27
pozitif AS hastaları ile karşılaştırmak amacıyla yapılmıştır. Çalışmada Eskişehir
Osmangazi Üniversitesi Tıp Fakültesi İç Hastalıkları ABD, Romatoloji BD‘da 1984
Modifiye New York sınıflama kriterlerine göre AS tanısı ile takip edilen 451 hastanın
dosyaları retrospektif olarak değerlendirildi. Hastaların 111’i (%24,6) HLA-B27
negatifken, 340’ı (%75,4) HLA-B27 pozitifti. Hastaların cinsiyet dağılımı,
semptomların başlama yaşı, tanı yaşı, tanıda gecikme süresi, ilk ve son vizitteki
eritrosit sedimentasyon hızı ve C-reaktif protein değerleri, ilk başvuru şikayeti,
spondiloartropati bulgularından inflamatuar bel ağrısı, periferik eklem tutulumu,
entezit, daktilit, psöriazis, üveit, inflamatuar barsak hastalığı, Ailevi Akdeniz Ateşi,
nefrolityazis ve eşlik eden komorbid hastalık sıklığı, ailede spondiloartropati (SpA)
öyküsü, radyolojik bulgular ve tedaviler değerlendirildi. Bu değerlendirmede, üveit
HLA-B27 pozitif grupta, inflamatuar barsak hastalığı ise HLA-B27 negatif grupta
daha yüksek prevelans gösterdi. Ailede SpA öyküsü, HLA-B27 pozitif grupta belirgin
fazlaydı. Sakroiliak Manyetik Rezonans Görüntülemede (MRG) izlenen sakroiliit ise
HLA-B27 negatif grupta daha fazla saptandı. Diğer bakılan bulgular açısından HLA B27 ile anlamlı ilişki saptanamadı. Sonuç olarak bu çalışma, HLA-B27 negatif ve
pozitif grupların Ankilozan Spondilit hastalığındaki bulgularının benzer olduğunu ve
HLA-B27’nin çok fazla belirleyici olmadığını ortaya koymuş olup, HLA-B27 geninin
Ankilozan Spondilit hastalığına olan etkisinin yeniden değerlendirilmesi için teşvik
edicidir
Although the HLAB27 gene is believed to develop Ankylosing Spondylitis (AS) in humans, this issue is
still controversial. This study was conducted to evaluate the clinical, laboratory data,
radiological features and treatments of HLA-B27 negative AS patients and compare
them with HLA-B27 positive AS patients. In this study, 451 patients who were
followed up with the diagnosis of AS according to the modified New York
classification criteria in Eskişehir Osmangazi University Faculty of Medicine
Department of Internal Medicine, Department of Rheumatology were evaluated
retrospectively. 111 patients (24.6%) were negative for HLA-B27 and 340 patients
(75.4%) were positive for HLA-B27. Gender distribution of patients, age at onset of
symptoms, age at diagnosis, delay in diagnosis, erythrocyte sedimentation rate and Creactive protein values at first and last visit, first application complaint, inflammatory
back pain, peripheral joint involvement, enthesitis, dactylitis, psoriasis, uveitis
,inflammatory bowel disease, Familial Mediterranean Fever, nephrolithiasis and
comorbid disease, familial spondyloarthropathy (SpA) history, radiological findings
and treatments were evaluated. In this evaluation, uveitis showed higher prevalence in
HLA-B27 positive group and inflammatory bowel disease in HLA-B27 negative
group. Family history of SpA was significantly higher in HLA-B27 positive group.
Sacroiliitis observed in sacroiliac Magnetic Resonance Imaging (MRG) was higher in
HLA-B27 negative group. There was no significant relationship between HLA-B27
and other findings. In conclusion, this study showed that HLA-B27 negative and
positive groups had similar findings in Ankylosing Spondylitis, and that HLA-B27
was not much predictive, and it was encouraging to reevaluate the effect of HLA-B27
gene on Ankylosing Spondylitis
