İdiyopatik inflamatuvar
miyopatiler (İİM), immun aracılıklı kas hasarlanması ile giden, proksimal iskelet
kaslarının inflamasyonuna bağlı bağ dokusu hastalığıdır. İİM’ler içinde polimiyozit
(PM), dermatomiyozit (DM) en iyi bilinen ve en sık görülen tipleridir. İİM’ler kas
tutulumu dışında cilt, eklem, akciğer, özefagus ve kalp gibi organları da etkileyebilen
sistemik hastalıklardır. Bu hastalıkların en önemli ortak özelliklerinden biri de
malignite ile ilişkileridir. Çalışmamızın amacı İİM tanısı ile izlenen hastaların klinik
ve laboratuvar özelliklerini, prognoz üzerine etkili faktörleri saptamak, sonuçlarımızı
literatürdeki diğer verilerle karşılaştırmak ve hastalarımızdaki tedaviye yanıt
oranlarını belirlemekti. Bu amaçla Romatoloji bölümünde 1975 Bohan-Peter tanı
kriterlerine göre İİM tanısı konulup takip ve tedavisi yapılan 53 hasta retrospektif
olarak değerlendirildi. Çalışmaya dahil edilen 53 hastanın (32 kadın, 21 erkek) yaş
ortalaması 55.24±16.12 yıl, miyopati tanı yaşı ortalaması 44.45±17.62 yıl idi. Miyozit
alt gruplarının dağılımı incelendiğinde; 23 hasta (%43.4) PM, 27 hasta (%50.9) DM
ve 3 hasta (%5.7) inklüzyon cisimcikli miyozit (İCM) şeklindeydi. En sık görülen
klinik bulgu %77.2 ile halsizlik idi. Kreatin fosfokinaz (CK) düzeyi ortalaması tanı
sırasında 2792.18 U/L, tedavinin 1. ayında 1400.86 U/L idi. Tanı sırasında kas gücü
düzeyi hastaların %64.2’sinde 4/5 idi. Elektronöromiyografide (EMG) miyopati
varlığı hastaların sadece %49.1’inde saptanmışken, kas biyopsisi yapılan hasta oranı
%77.4 idi. Hastalarda steroid kullanımı %98.1, azatioprin %34 ve metotreksat
kullanımı %32.1 oranında idi. Mortalite sıklığı %13 ve malignite sıklığı benzer şekilde
%13 olarak saptandı. Malignite gelişen hastaların yaşı gelişmeyenlere göre daha
yüksek ve yine miyozit tanı yaşı da daha yüksekti (sırasıyla p<0.03 ve p<0.02). En sık
mortalite nedeni malignite idi. Bizim kohortumuzda 5 yıllık kümülatif sağkalım oranı
% 80.75, 10 yıllık sağkalım oranı % 72.68 idi. Sonuç olarak kadın baskınlığı ve PM
sıklığı, beklenen yaşam süresi ve mortalite üzerine en önemli faktörlerin ileri tanı yaşı
ve kanser varlığı olması literatüre benzer iken klinik bulgular açısından farklılıklar
saptadık. Bu farklılığın genetik faktörlerden ve hasta sayısının az olmasından
kaynaklanmış olabileceği düşünüldü
Idiopathic inflammatory myopathies (IIM) is a connective tissue disease due to
inflammation of the proximal skeletal muscles, leading to immune-mediated muscle
injury. Among IIM’s, polymyositis (PM) and dermatomyositis (DM) are the best
known and most common types. IIMs are systemic diseases that can affect organs such
as skin, joints, lungs, esophagus, and heart, as well as muscle involvement. One of the
most important common features of these diseases is their relationship with
malignancy. The aims of our study were to determine the clinical and laboratory
features of patients who were followed up with the diagnosis of IIM, to identify factors
affecting prognosis, to compare our results with other data in the literature, and to
determine the response rates of our patients to the treatment. For these purposes, 53
patients who were diagnosed with IIM according to the diagnosis criteria of Bohan Peter in 1975 in the Department of Rheumatology, were followed up and treated
retrospectively. The mean age of 53 patients (32 females, 21 males) included in the
study was 55.24±16.12 years, and the mean age of myopathy diagnosis was
44.45±17.62 years. When the distribution of myositis subgroups is examined; 23
patients (43.4%) were PM, 27 patients (50.9%) were DM and 3 patients (5.7%) were
inclusion body myopathy (ICM). The most common clinical finding was malaise with
77.2%. The mean creatine phosphokinase (CK) level was 2792.18 U/L at the time of
diagnosis and 1400.86 U/L in the 1st month of treatment. At the time of diagnosis,
muscle strength level was 4/5 in 64.2% of patients. While the presence of myopathy
in electroneuromyography (EMG) was detected only in 49.1% of patient, the rate of
patients undergoing muscle biopsy was 77.4%. Of patients, uses of steroid,
azathioprine, and methotrexate were 98.1%, 34%, and 32.1%, respectively. The
frequency of mortality was 13% and the frequency of malignancy was similarly 13%.
Patients who developed malignancy were older than those who did not, and the age of
myositis diagnosis was also higher (p<0.03 and p<0.02, respectively). The most
common cause of mortality was the malignancy. In our cohort, the cumulative 5-year
survival rate was 80.75% while the 10-year survival rate was 72.68%. In conclusion,
female dominance, frequency of PM, life expectancy, and the most important factors
on mortality are advanced age of diagnosis and presence of cancer are similar to the
literature, we found some differences in terms of some clinical findings. It was thought
that these differences might have been due to genetic factors and a low number of
patients
