Bu çalışmaya 01.06.2019-01.06.2020 tarihleri arasında son 1 aydır sistemik tedavi
almamış 50 alopesi areata (AA) hastası ile kontrol grubu olarak hastalarla yaş ve
cinsiyet açısından eşleştirilmiş, diğer yönlerden sağlıklı, hastane personelinden oluşan
50 gönüllü dahil edildi. Bir sosyodemografik veri formu hazırlanarak katılımcıların
yaşı, cinsiyeti, medeni hali, eğitim durumu gibi demografik verileri kaydedildi. Hastalarda
ayrıca AA'nın klinik alt tipi, AA başlangıç yaşı, geçirilen atak sayısı, hastalığın
toplam süresi, en son gelişen AA atağının ay olarak süresi ve hastalık şiddeti kaydedildi.
Psikolojik stres durumunun değerlendirilmesinde sıkıntıya dayanma ölçeği
(SDÖ) ve depresyon anksiyete stres ölçeği-21 (DASS-21) kullanıldı. AA ve nörotrofin
ilişkisinin değerlendirilmesi amacıyla beyin kaynaklı nörotrofik faktör (BDNF), sinir
büyüme faktörü (NGF), nörotrofin-3 (NT-3) ve nörotrofin-4 (NT-4) serum düzeyleri
ölçüldü. Çalışmamızda AA hastalarında toplam DASS-21 ve depresyon, anksiyete,
stres alt ölçeklerinin puanları daha yüksek, toplam SDÖ ve tolerans, regülasyon, özyeterlilik
alt ölçeklerinin puanları daha düşük bulunmuştur. Çalışmamızda serum
BDNF ve NT-3 seviyeleri hasta ve kontrol grubu arasında anlamlı farklılık göstermezken,
serum NGF seviyesi AA grubunda kontrol grubuna göre anlamlı yüksek, NT-4
seviyesi ise anlamlı düşük saptanmıştır. Çalışmamızın sonuçları AA’nın depresyon ve
anksiyete gibi psikolojik faktörler ve nörotrofinler ile ilişkisini desteklemektedir. Selektif
nörotrofin reseptör agonistleri ve antagonistleri ilerleyen dönemlerde tedavi hedefi
olarak görülebilir ve bu sayede mevcut tedavi rejimlerine yenileri eklenerek
AA'nın daha iyi yönetimi sağlanabilir. AA'nın ve çeşitli psikiyatrik hastalıkların ortak
patofizyolojisinin daha iyi anlaşılması için AA ve stres nöroimmünolojisi ilişkisini
araştıran daha fazla çalışma yapılması gerekmektedir
This study was conducted between 01.06.2019-01.06.2020 with 50 alopecia
areata (AA) patients who had not received systemic treatment for the last 1 month
and 50 otherwise healthy, hospital personnel as a control group were included. A sociodemographic
data form was prepared and demographic data of the participants such
as age, gender, marital status and educational status were recorded. The clinical
subtype of AA, the age of onset of AA, the number of attacks, the total duration of the
disease, the duration of the most recent AA attack in months, and the disease severity
were also recorded in AA patients. In the evaluation of the psychological stress state,
the distress tolerance scale (DTS) and the depression anxiety stress scale-21 (DASS-
21) were used. In order to evaluate the relationship between AA and neurotrophin,
serum levels of brain derived neurotrophic factor (BDNF), nerve growth factor (NGF),
neurotrophin-3 (NT-3) and neurotrophin-4 (NT-4) were measured. In our study, scores
of total DASS-21 and depression, anxiety, and stress subscales were found to be higher,
scores of total DTS and tolerance, regulation, and self-efficacy subscales were
found to be lower in AA patients. In our study, serum BDNF and NT-3 levels did not
differ significantly between the patient and control groups, while the serum NGF level
was found to be significantly higher in the AA group compared to the control group,
and the NT-4 level was found to be significantly lower. The results of our study support
the relationship between AA and psychological factors such as depression and
anxiety, and neurotrophins. Selective neurotrophin receptor agonists and antagonists
can be seen as treatment targets in the future, and thus better management of AA can
be achieved by adding new ones to existing treatment regimens. More studies are needed
to investigate the relationship between AA and stress neuroimmunology to better
understand the common pathophysiology of AA and various psychiatric diseases
