enfeksiyonlarında kullanılan antibiyotiklere karşı direnç gelişimi tedavi
başarısızlığına neden olmaktadır. Bu nedenle bu çalışmada üropatojenlerin yıllara
göre direnç profilinin değerlendirilmesi, antibiyotik direnç gelişimini etkileyen risk
faktörlerinin bulunması ve tedavide kullanılacak uygun ampirik antibiyotiğin
belirlenmesi amaçlanmıştır. Bu tek merkezli, retrospektif çalışma Eskişehir
Osmangazi Üniversitesi Tıp Fakültesi, Çocuk Sağlığı ve Hastalıkları bölümünde
Ocak 2010–Aralık 2019 tarihleri arasında yürütüldü. İdrar yolu enfeksiyonu tanılı 1
ay-18 yaş arası 1852 çocuk hasta ve onlara ait 105 CFU/mL üzerinde üropatojen
üremesi olan 2340 idrar kültürü çalışmaya dahil edildi. Hastaların yaşı, cinsiyeti,
idrar yolu enfeksiyonu atak sayısı, ürolojik anomali varlığı, antibiyotik profilaksi
kullanımı, üropatojenlerin yıllara göre antibiyotik dirençleri, ESBL pozitif hastaların
özellikleri ve ESBL pozitif üropatojenler ile ilgili veriler kaydedildi. Hastaların
%70.6'sı (n=1308) kız, %29.4'ü erkekti. Yaş ortalamaları 73±61 ay (1-215 ay) olup
idrar yolu enfeksiyonu atak sayısı 2.5±3.8 (1-39) idi. En sık görülen üropatojenler
E.coli (%59.6) ve Klebsiella spp. (%11.5) idi. Yıllara göre E.coli, Klebsiella
pneumoniae ve Acinetobacter spp. üremesinde istatistiksel olarak anlamlı artış
görüldü (p<0.001). ESBL pozitif üropatojenlerin oranı %16.2 olup infantlarda,
tekrarlayan İYE geçirenlerde, üriner anomalisi olan ve profilaktik antibiyotik
kullananlarda ESBL pozitifliği daha yüksekti (p<0.05). Direncin en yüksek olduğu
antibiyotikler; penisilin (%60.8), ampisilin (%55.7), trimethoprim-sülfametaksazol
(%40.8)'dur. ESBL pozitif Klebsiella suşlarında imipenem ve meropenem direncinin
arttığı saptandı (p<0.05). Sonuç olarak, üropatojenlerin antibiyotik dirençleri
hastaların klinik özelliklerine göre değişir. İYE tedavisinde özellikle 12 aydan küçük
infantlarda, üriner anomali ve tekrarlayan İYE öyküsü olanlarda direncin daha
yüksek olması nedeniyle yerel çalışmalar doğrultusunda uygun antibiyotik seçimi
yapılmalıdır
Development of resistance to
antibiotics used in urinary tract infections causes treatment failure. Therefore, the
aim of this study is to evaluate the resistance profile of uropathogens by years, to
find risk factors affecting the development of antibiotic resistance, and to determine
the appropriate empirical antibiotic to be used in treatment. This single center,
retrospective study was conducted between January 2010 and December 2019 at
Eskişehir Osmangazi University, Faculty of Medicine, Department of Pediatrics.
1852 pediatric patients between 1 month and 18 years of age with a diagnosis of
urinary tract infection and 2340 urine cultures of these patients with uropathogen
growth above 105 CFU/mL were included. Data on age, gender, number of urinary
tract infection episodes, presence of urological anomalies, use of antibiotic
prophylaxis, antibiotic resistance of uropathogens by years, characteristics of patients
with ESBL positivity and ESBL positive uropathogens were recorded. 70.6%
(n=1308) of the patients were girls and 29.4% were boys. The mean age was 73 ± 61
months (1-215 months) and the number of urinary tract infection episodes was
2.5±3.8 (1-39). The most common uropathogens were E.coli (59.6%) and Klebsiella
spp. (11.5%). There was a statistically significant increase in the growth rate of
E.coli, Klebsiella pneumoniae and Acinetobacter spp. by years (p<0.001). The rate of
ESBL positive uropathogens was 16.2% and ESBL positivity was higher in infants,
patients with recurrent UTIs, urinary anomalies, and those who used prophylactic
antibiotics (p<0.05). Antibiotics with the highest resistance were penicillin (60.8%),
ampicillin (55.7%), trimethoprim-sulfamethoxazole (40.8%). It was found that
imipenem and meropenem resistance increased in Klebsiella strains (p<0.05). In
conclusion, antibiotic resistance of uropathogens varies according to the clinical
characteristics of the patients. In the treatment of UTI, especially in infants younger
than 12 months, and those with urinary anomalies and a history of recurrent UTIs,
appropriate antibiotic should be chosen in accordance with local studies
