Acil servise (AS) başvuran hastaların
özelliklerinin bilinmesi, işleyişin takip edilmesi ve pandemi sürecinin oluşturduğu
etkinin belirlenmesi AS uygulamalarının planlanması ve kalitenin arttırılması
açısından önemlidir. Bu çalışmanın amacı, COVİD-19 pandemi döneminde, alınan
tedbirlerin, toplum ve hekim davranışlarındaki değişikliklerin AS kullanımı
üzerindeki etkisini değerlendirmek, hizmet sunumundaki öncelikleri belirlemektir.
Çalışmamıza 11.03-11.06.2019 (2019 grubu) ve 11.03-11.06.2020 (2020 grubu)
tarihlerinde Eskişehir Osmangazi Üniversitesi Sağlık, Uygulama ve Araştırma
Hastanesi Erişkin AS’sine başvuran tüm hastalar dahil edilmiştir. Demografik
özellikler, AS süreçleri ve maliyetleri açısından iki dönem arasında karşılaştırma
yapılmıştır. 11 Mart 2020’de pandemi kararı ile birlikte alınan önlemlerle 2020
grubunda AS’ye başvuran hasta sayısında keskin bir düşüş yaşanmıştır. Hasta
sayısının azalmasının yanında, kırmızı triaj kategorisindeki hasta başvuru oranları
artarken sarı ve yeşil kategorideki hasta gurubunun oranı azalmıştır. Ciddi şikayetleri
olan hasta gurubunun AS başvurularının sayıca azlması sonucu ambulans ile
başvuru, görüntüleme, konsültasyon, servis ve yoğun bakım yatış hacimlerinde
azalma saptanırken, toplam başvurular içindeki oranlarında artış görüldü. Ortalama
maliyette ise 2020 grubunda artış saptanmıştır. Görüntüleme, konsültasyon, kronik
hastalıklar ve ortalama acil seviste kalış süresinin maliyetle ilişkili olduğu saptandı.
Ülke çapında alınan önlemlerle 2020 grubunda çocuk travma hastalarının hacmin
azaldığı saptandı. AS’de değerlendirilen ve 2020 grubuna dahil edilen Covid-19
tanısı alan hastaların yaş ortalaması, kronik hastalık oranı, ortalama AS'te kalış
süresi, toraks BT görüntüleme, konsültasyon, hastane yatış ve ortalama maliyet
oranları genel AS başvuru oranlarına göre yüksek saptanmıştır
Knowing the characteristics of the patients admitted to the emergency department
(ED), monitoring the functioning and determining the effect of the pandemic process
are important in terms of planning the ED practices and increasing the quality. The
aim of this study is to evaluate the effects of the measures taken and the changes in
the society and physician behaviors on the use of ED during the COVID-19
pandemic period, and to determine the priorities in service delivery. All patients who
applied to Eskişehir Osmangazi University Health, Practice and Research Hospital
Adult ED between 11.03-11.06.2019 (2019 group) and 11.03-11.06.2020 (2020
group) were included in our study. Comparisons were made between the two periods
in terms of demographic characteristics, ED processes and costs. With the measures
taken with the pandemic decision on March 11, 2020, there was a sharp decrease in
the number of patients admitted to ED in the 2020 group. In addition to the decrease
in the number of patients, the rate of patient admissions in the red triage category
increased, while the rate of the patient group in the yellow and green category
decreased. As a result of the decrease in the number of ED applications of the patient
group with serious complaints, there was a decrease in the volume of admission by
ambulance, imaging, consultation, service and intensive care unit admissions, while
an increase was observed in their rate in total admissions. There was an increase in
the average cost in the 2020 group. It was determined that imaging, consultation,
chronic diseases and average length of stay in the emergency department were
associated with cost. It was determined that the volume of pediatric trauma patients
in the 2020 group decreased with the measures taken across the country. The average
age, chronic disease rate, average length of stay in the ED, thoracic CT imaging,
consultation, hospitalization and average cost rates of patients diagnosed with Covid-
19, who were evaluated in the ED and included in the 2020 group, were higher than
the general emergency service admission rates detected
