Bu prospektif klinik çalışmanın amacı, hibrit ve akrilik bonded RME apareyinin dentofasiyal yapılar üzerine etkilerini, KIBT ve USG ile değerlendirip elde edilen sonuçları karşılaştırmaktır.
Materyal ve Metot: Prospektif çalışmamızın materyalini, 30 hastanın genişletme öncesinde (T0), aktif genişletmenin sonunda (T1) ve 3 aylık pekiştirme dönemi sonunda (T2) alınan ultra düşük doz KIBT ve USG kayıtları oluşturmaktadır. Hastalar randomize olarak 2 gruba ayrılmıştır. Akrilik bonded RME grubunu, yaşları ortalama 12.6 ± 1.4 olan 15 hasta, hibrit RME grubunu yaşları ortalama 13.1 ± 1.8 olan 15 hasta oluşturmaktadır. Genişletme vidası, midpalatal sutur açılıncaya kadar sabah, akşam günde toplam 2 çeyrek tur (0.44 mm/gün), sutur açıldıktan sonra ise günde 1 çeyrek tur aktive edilmiştir. Maksiller 1.molar dişin palatinal tüberkülü ile mandibular 1.molar dişin bukkal tüberkülü arasındaki ilişkiye bakılarak retansiyon dönemine geçilmiş, apareyler pasif bir şekilde 3 ay boyunca ağızda tutulmuştur. Sert ve yumuşak dokuda meydana gelen değişimler lateral ve postero-anterior sefalometrik analizlerde, KIBT verilerinden elde edilen kesitlerde ve USG aksiyal kesitinde değerlendirilmiş ve karşılaştırılmıştır. Elde edilen veriler, IBM SPSS V23 programı ile analiz edilmiştir.
Bulgular: İki aparey grubunda sagital, vertikal yönde meydana gelen dişsel, iskeletsel etkiler benzerdir. Midpalatal suturun açılmasıyla maksillar ve nazal genişlikte önemli artışlar tespit edilmiştir. (p<0.001) Koronal kesitte, iki grupta da iskeletsel transversal artışlar benzerdir. Dişsel ölçümlerdeki artışlar, iskeletsel ölçümlerdeki artışlardan yüksek bulunmuş, piramit şekilli bir genişleme gerçekleşmiştir. 1.premolar dişlerin pulpa odaları arasındaki mesafe, tedavi sonunda akrilik grupta hibrit gruba göre istatistiksel olarak anlamlı derecede yüksektir. (p=0.024) Aksiyal kesitte, midpalatal sutural açılma özellikle anterior bölgede hibrit grupta daha fazladır, USG’de DR-DL, ER-EL mesafelerinde iki

grup arasındaki fark anlamlıdır. (p=0.022, p=0.018) Hem KIBT, hem USG’de anterior, orta ve posterior seviyelerdeki midpalatal sutural açılma birbiriyle yüksek korelasyon göstermektedir. Fakat, KIBT ile USG ölçümleri arasındaki korelasyon zayıf bulunmuştur.
Sonuç: Her iki apareyin de dentofasiyal etkileri benzer bulunmuştur. İki aparey grubunda da, midpalatal sutural ayrılma KIBT ve USG görüntülerinde tespit edilmiştir. USG, RME tedavisinde sutural açılmanın tespitinde radyasyon içeren görüntüleme yöntemleri yerine kullanılabilir. Fakat açılma miktarının ölçümünde güvenilirliği sınırlıdır
The aim of this prospective clinical study is to evaluate the effects of hybrid and acrylic bonded RME appliances on dentofacial structures with CBCT and USG, and then to compare the CBCT and USG findings with each other.
Material and Method: The material of our prospective study consists of ultra-low-dose CBCT and USG records of 30 patients taken before expansion (T0), at the end of active expansion (T1) and at the end of the 3-months retention period (T2). Patients were randomly divided into 2 groups. Acrylic-bonded RME group included 15 patients whose mean age was 12.6 ± 1.4 years and hybrid RME group included 15 patients whose mean age was 13.1 ± 1.8 years. Expansion screw was activated 2 quarter turn (0.44 mm / day) a day until the midpalatal suture was opened, and 1 quarter turn a day after the suture was opened. The retention period was initiated by examining the relationship between the palatinal cusps of the maxillary 1st molar and the buccal cusps of the mandibular 1st molar, the appliances were kept in the mouth passively for 3 months. Changes in hard and soft tissue were evaluated and compared in lateral and postero-anterior cephalometric analyzes, sections obtained from CBCT data and USG axial section. The obtained data were analyzed with IBM SPSS V23 program.
Results: Using both appliances revealed similar results in the sagittal and vertical directions. Significant increases in maxillary and nasal width were found with the opening of the midpalatal suture. (p <0.001) In coronal section, skeletal transversal increases were similar in both groups. Increases in dental measurements were higher than skeletal measurements, a pyramid-shaped expansion occurred. The distance between the pulp chambers of the 1st premolar is statistically significantly higher in the acrylic group compared to the hybrid group at the end of the treatment. (p = 0.024) In the axial section, the midpalatal sutural opening is more especially in the anterior region in the hybrid

group, the difference between the two groups in USG is significant in DR-DL ER-EL distances. (p = 0.022, p = 0.018) Midpalatal sutural opening at the anterior, middle and posterior levels in both CBCT and USG are highly correlated with each other. However, the correlation between CBCT and USG measurements was found to be weak.
Conclusion: Dentofacial effects of both appliances were found to be similar. In both appliance groups, midpalatal sutural separation was detected in CBCT and USG images. USG can be used instead of imaging methods involving radiation in the detection of sutural opening in RME treatment. However, its reliability in measuring the opening amount is limited
