Malign
mikst müllerian tümörlerin (MMMT) (karsinosarkom) epitelyal ve mezenkimal
komponentinin ortak bir kök hücreden köken aldığı ve monoklonal bir neoplazm
olduğu, mezenkimal komponentin metaplastik bir transformasyon ile geliştiği görüşü
günümüzde kabul görmekte olup bu tümörlerin metaplastik karsinom olarak
tanımlanması önerilmektedir. Literatürde bir çalışmada uterin mezenkimal
tümörlerin gelişiminde WNT/β-Katenin sinyalizasyonunun etkili olabileceği ve
stromada disregüle olan β-kateninin endometriyal glandüler hiperplazi ve
adenokarsinomu indükleyebileceği öne sürülmüştür. β-katenini ortak kullanarak E kaderin aracılıklı adezyon ve WNT/β-katenin yolağı birbirine bağımlı hareket eder.
E-kaderin hücreler arası adezyon oluşumunu sağlar ve epitelyal mezenkimal fenotip
değişiminin en önemli bileşenidir. Son yıllarda karsinosarkomların gelişiminde
epitelyal mezenkimal dönüşümün rolü olduğu düşünülmekte olup, bu çalışmada
MMMT’lerin her iki tümör komponentinde E-kaderin ve β-katenin ekspresyonu
araştırılmıştır. Çalışmamızda kadın genital sisteminde gelişmiş toplam 32 MMMT
değerlendirildi. Vak’aların % 69.7’sinde epitelyal komponentte E-kaderin
ekspresyonu, %72.8’inde β-katenin ekspresyonu görülürken, mezenkimal
komponentte E-kaderin ekspresyonu bir vak’ada, β-katenin ekspresyonu 11 vak’ada
fokal olarak saptandı. Tümörün karsinomatöz komponentinde E-kaderin ve β-katenin
ekspresyonunun, kontrol ile kıyaslandığında daha az olduğu gözlendi (p<0.05).
Tümörün epitelyal komponentinde değişken ve düşük oranlarda E-kaderin ve β katenin ekspresyonunun görülmesi hücreler arası adezyon mekanizmalarında
regülasyon bozukluğu olduğunu, mezenkimal komponentte E-kaderin
ekspresyonunun olmayışı ve bazı vak’alarda β-kateninin pozitifliği bu tümörlerin
histopatogenezinde epitelyal mezenkimal dönüşümün rolü olabileceğini
düşündürmüştür.
Both of epithelial and
mesenchymal components of Malignant mixed müllerian tumours (MMMT) are
regarded as deriving from a stem cell and monoclonal in origin. In recent years, it is
accepted that mesenchymal component is transformed by metaplasia and it is
proposed that these tumors should be described as metaplastic carcinomas.
Dysregulated WNT/β-catenin signaling has effects on uterine tumorogenesis and can
induce development of endometrial glandüler hiperplasia and adenocarcinomas. By
sharing the β-catenin molecule, E-cadherin mediated adhesion and WNT/β-catenin
pathway are dependent upon each other and given the central role of E-cadherin in
epithelial mesenchymal transitions. Recently, it is suggested that epithelial
mesenchymal transition has a significant role at development of carcinosarcomas
and, we researched expression of E-cadherin and β-catenin in both two components
of MMMT. We evaluated 32 MMMTs which are originated from female genital
tract. E-cadherin and β-catenin expressions were observed in 69.7 % and 72.8% of
MMMTs in the epithelial component respectively. In the mesenchymal component
we determined E-cadherin expression in only one patient and β-catenin expression in
11 patients focally. We find out that E-cadherin and β-cathenin expressions have
decreased when we compared them with controls (p<0.05). Variable expressions of
E-cadherin and β-catenin in the epithelial component of tumor can suggest that there
is a dysregülation in adhesion of cells and absence of E-cadherin expression and also
the presence of β-cathenin positivity in the mesenchymal component of these tumors
can suggest that epithelial mesenchymal transition can have a significant role of
histopathogenesis of MMMTs.
