Bu
çalışmamızda N2O’li ve N2O’siz düşük akım anestezi uygulamasında desfluranın
hemodinamik etkilerini araştırmayı amaçladık. Çalışmaya farklı tanılarla, elektif
cerrahi girişim planlanan genel anestezi verilecek ASA I-II, 18-65 yaş arası 40 hasta
alındı. Hastalar rastgele, Grup I (N2O’li Düşük Akımlı Anestezi Grubu) ve Grup II
(N2O’siz Düşük Akımlı Anestezi Grubu) olarak 20’şer kişilik iki gruba ayrıldı.
Premedikasyon uygulanmayan hastalardan Grup I’e 10 L/dk %100 O2 ile 2 dk
preoksijenizasyon sonrası, intravenöz yoldan sırası ile 4-7 mg/kg pentotal, 0,1 mg/kg
veküronyum ve 1 μg/kg fentanil sonrasında 3 dk beklenerek endotrakeal entübasyon
uygulandı. Entübasyonu takiben operasyonun ilk 10 dakikasında, 4 lt/dk (%50 O2 -
%50 N2O) normal akım uygulandıktan sonra, grup I’e 1 lt/dk (%50 O2 - %50
N2O),grup II’ye 1lt/dk (%50 O2-%50Hava) olacak şekilde düşük akıma
geçildi.Desfluran konsantrasyonu, 4-8 MAK olarak preoperatif OAB ± %20
sınırlarında tutacak şekilde ayarlandı. Grup II’ye ise N2O yerine hava kullanıldı.
Entübasyondan hemen sonra ve takip eden 15, 30, 45. ve 60. dakikalarda, hastaların
KAH, OAB, SpO2, EtCO2, FiO2, FeO2, FiN2O, FeN2O değerleri kaydedilerek
operasyonun bitimine 15 dakika kala tekrar 4 lt/dk normal akıma geçilmiştir.
Operasyonun bitiminde, taze gaz akışı 6 L/dk % 100 O2’e çıkartılarak ekstübe
edilmişlerdir. N2O’li ve N2O’siz düşük akım anestezi uygulanan iki grup arasında
yapılan karşılaştırmada, her iki grupta; KAH, SAB, DAB, OAB , SpO2, EtCO2
değerlerinde anlamlı bir farklılık saptanmadı. Sonuç olarak çalışmamızda N2O’li ve
N2O’siz düşük akımlı anestezide desfluranın hemodinamik parametreler üzerine
etkilerini karşılaştırdık ve çalışmamızda N2O kullanılmamasının hemodinamik
açıdan N2O kullanılan gruba göre bir farklılık göstermediği, aksine N2O
kullanılmamasının ekonomik ve ekolojik açıdan avantaj sağladığı kanısına vardık.
We aimed to investigate the
hemodynamic effects of desflurane in low flow anesthesia without nitrous oxide.
Fourty adult patients, ASA I-II, aged between 18-65 years, undergoing elective
surgery with different diagnoses under general anesthesia were included in the study.
Patients without premedication are randomized into two equal groups; Group I (low
flow anesthesia with nitrous oxide), Group II (low flow anesthesia without nitrous
oxide). Group I received preoxygenization for 2 minutes with 10 lt/minute %100 O2.
Then we give IV 4-7 mg/kg pentothal, 0,1 mg/kg vecuronium, 1 mcg/kg fentanyl and
waited for three minutes before endotracheal intubation. Normal flow is performed
the for first 10 minutes after intubation, 4 lt/minute (% 50 O2 - % 50 N2O). After 10
minutes 1 lt/minute (% 50 O2 - % 50 N2O) low flow is performed. The desflurane
concentration is adjusted to 4-8 MAC according to MAP ± %20 preoperative values.
Group II received air instead of N2O. Just after intubation and following 15, 30, 45,
60 minutes, we recorded MAP, HR, SpO2, EtCO2,FiO2,EtO2,FiN2O,EtN2O,EtDes
values. 15 minutes before end of surgery, we used again normal flow 4 lt/minute.
After the operation anesthetics are stoped and % 100 O2 in 6 lt/minute is
administered. On comparison of two groups with or without nitrous oxide we found
no significant difference of HR, MAP, SpO2, EtCO2 values. In conclusion, we
investigated the hemodynamic effects of desflurane in low flow anesthesia with and
without nitrous oxide. As there is no hemodynamic difference between with or
without nitrous oxide usage, we may conclude that anesthesia without nitrous oxide
might provide economical and ecological advantages.
