ÇalıĢmanın amacı kronik otitis media ve kolesteatomlu olgularda operasyon öncesi
orta kulak kemikçiklerinin 64 dedektörlü bilgisayarlı tomografi ile incelenmesi ve
operasyon sonuçları ile korelasyonunu saptamaktı. Prospektif olarak 5 aylık periyotta
100 olgu çalıĢmaya dahil edildi. Kulak-burun-boğaz kliniği tarafınca değerlendirilen
olgulara 7 gün içersinde 64 dedektörlü bilgisayarlı tomografi ile temporal BT tetkiki
gerçekleĢtirildi. Olgular 2 ay içinde opere edildi.Operasyon altın standart kabul
edildi. Çok kesitli BT ile 100 olguda orta kulak kemikçiklerinin değerlendirilmesinde
malleus için duyarlılık %95,1, özgüllük %92,3, inkus için duyarlılık %84,6,
özgüllük %98,4 ve stapes için duyarlılık %71,2, özgüllük %100 saptandı. Çok kesitli
BT ile duyarlılık en yüksek malleus anterior proseste (%97,1) bulunurken, en düĢük
stapes anterior krusta (%71,2) bulundu. Ayrıca ÇKBT ile operasyon bulguları
arasında yapılan karĢılaĢtırmalarda kemikçik değiĢikliği saptanmaması, rezorbtif
değiĢiklikler ve tam rezorbsiyon saptanması açısından uyum kappa analizleri ile
araĢtırıldı.En yüksek uyum malleus medial proceste (к=0.936) saptanırken, en düĢük
uyum stapes posterior krusta (к=0,768) saptandı. Sonuç olarak operasyon öncesinde
orta kulak kemikçiklerinin değerlendirilmesinde 64 dedektörlü bilgisayarlı
tomografinin yüksek doğrulukla kullanılabileceği saptandı.
The aim of this study was to make preoperative assessment of the middle
ear ossicles with 64- multidetector CT in cases of chronic otitis media and
cholesteatoma and to determine the correlation between CT findings and surgery
results.100 patients were included in this prospective study within a 5-month-period.
After a clinical examination in ear-nose-throat clinic all patients underwent 64-
multidetector temporal CT within 7 days. All patients underwent surgery within 2
months and the operation results were accepted as gold standart. Assessment of
middle ear ossicles with multidetector CT in 100 patients yielded sensitivity and
spesificity values of % 95.1 and % 92.3 for malleolus; % 84.6 and % 98.4 for incus,
% 71.2 and % 100 for stapes, respectively. The sensitivity values in assessment of
ossicles with MDCT were highest for anterior process of malleolus (% 97.1) and
lowest for anterior crus of stapes (% 71.2). Using kappa analysis, a comparative
analysis of MDCT and operation findings were made in respect to absence of
ossicles changes, resorptive ossicle changes and complete ossicle resorption.
Correlation between MDCT and operation findings were highest for medial process
of malleolus (κ = 0.936) and lowest for posterior crus of stapes (κ = 0.768). In
conclusion we found that 64- multidetector computed tomography examination of
middle ear ossicles can be used in pre-operative assessment of middle ear ossicles
with high accuracy rates.
