Son dönemlerde keşfedilen
ve visfatin olarak adlandırılan bir adipokinin insüline benzer etkileri olduğu
belirtilmiştir. Bu çalışmada obez çocuklarda oral glukoz tolerans testi (OGTT)
yapılarak serum visfatin düzeylerinde değişiklik olup olmadığı araştırıldı.
Çalışmamıza 40 ekzojen obezitesi olan çocuk ve 20 sağlıklı çocuk dahil edildi.
OGTT işleminin başlangıcında, visfatin, glukoz, c-peptit (CPE), insülin, trigliserid
(TG), düşük ve yüksek dansiteli lipoprotein kolesterol (LDL-C ve HDL-C), total (T)
kolesterol, aspartat ve alanin aminotransferaz (AST, ALT) düzeyleri ve tiroid
fonksiyon testleri, 60 ve 120. dakikalarda glukoz, insülin, CPE ve visfatin düzeyleri
çalışıldı. Kontrol grubunda bu araştırmalar sadece açlık sırasında yapıldı. Hasta
grubunun 0. dakika ortalama serum visfatin düzeyleri (11.74 ± 3.76 ng/ml) kontrol
grubuna (7.92±2.18 ng/ml) göre anlamlı derecede daha yüksekti (p=0.02). Hasta ve
kontrol grubu birlikte değerlendirildiğinde visfatin düzeyi ile vücut ağırlığı (VA),
vücut kitle indeksi (VKİ), bel çevresi, kalça çevresi, bel/kalça oranı arasında pozitif
korelasyon bulundu (hepsi için p<0.001). Visfatin düzeyiyle, insülin, CPE,
Homeostasis Model Assesment-Insulin Resistance (HOMA–IR), TG ve LDL-C
arasında pozitif korelasyon (sırasıyla r= 0.45, p=0.01; r= 0.42, p<0.001, r=0.42, p=
0.01, r= 0.27, p=0.03; r=0.3, p= 0.01) saptandı. Tüm çalışma grubunda oluşturulan
lojistik regresyon modelinde açlık serum visfatin düzeyi sadece VKİ ile zayıf ilişki
gösterdi ve VKİ bir birim arttığında visfatin düzeyinin 0.2 birim arttığı saptandı
(R²=0.195, p<0.001). OGTT sırasında serum visfatin düzeyleri 0., 60. ve 120.
dakikalarda farklılık göstermedi (p>0.05). Sonuç olarak obez çocuklarda visfatin
düzeyinin OGTT sırasında ortaya çıkan akut glukoz düzeyi değişikliklerinden
etkilenmediği saptandı. Obez çocuklarda visfatin düzeyinin daha yüksek olması ve
sadece VKİ ile pozitif korelasyon göstermesi; serum visfatin düzeyinde obezite,
karbonhidrat ve lipid metabolizması dışında çalışmamızda araştırılmayan başka
faktörlerin etkili olduğunu düşündürmektedir.
It has been reported that an adipokine, termed
visfatin has insulin-like effects. In this study, oral glucose tolerance tests (OGTT)
were performed on obese children to investigate any possible changes in serum
visfatin levels during the test. Fourty with exogenous obesity and 20 healthy children
were included in the study. At the start of the OGTT procedure, glucose, c-peptide
(CPE), insulin, visfatin, triglyseride (TG), low and high density lipoprotein
cholesterol (LDL-C and HDL-C), total cholesterol, aspartate and alanine
aminotransferase levels, and thyroid function tests were performed whereas glucose,
insulin, CPE, and visfatin levels were investigated at minutes (min) 60 and 120.
These tests were performed only in the fasting phase in the control group. The serum
visfatin levels of the patient group at min 0 (11.74 ± 3.76 ng/ml) was significantly
higher as compared with that of the control group (7.92±2.18 ng/ml)(p=0.02). When
the patient and control groups were evaluated together, positive correlations found
between the visfatin levels and body weight, body mass index (BMI), waist and hip
circumferences, waist/hip ratio (p<0.001 for all). Positive correlations was found
between the visfatin level and insulin, CPE, Homeostasis Model Assesment- Insulin
Resistance (HOMA–IR), TG, LDL-C (r=0.45, p=0.01; r=0.42, p<0.001; r=0.42,
p=0.01, r=0.27, p=0.03; r=0.3, p=0.01 respectively). Logistik regression model
showed positive relationship of visfatin level with BMI in the study group. The
visfatin level was increased 0.2 units in case BMI increased one unit (R² = 0.195, p
<0.001). Serum visfatin levels didn’t differ during OGTT at min 0, 60, and 120
(p>0.05). It was concluded that visfatin levels in obese children weren’t influenced
by acute glucose level changes during OGTT. Because of high visfatin levels in
obese children and positive correlation of visfatin level with only BMI in logistic
regression model, there might be some other factors that effect serume visfatin levels
other than obesity, carbohydrate and lipid metabolisms, which weren’t evaluated in
this study.
