Bu çalışmanın amacı kardiyopulmoner bypass cerrahisinde
kullanılan sevofluran ve desfluranın miyokart üzerindeki koruyucu etkilerini
miyokardiyal hasarlanma işaretleyicileri aracılığı ile karşılaştırmaktır. Çalışma
elektif KABG planlanan ASA III-IV 18- 69 yaş arası 45 erkek, 15 kadın 60 hasta
üzerinde yapıldı. Hastalar rasgele 20’ şer kişilik 3 gruba ayrıldı. Operasyon
masasında hastalarda KAH, SAB, OAB, DAB, SpO2, CVP monitorizasyonu
yapılarak takipleri alındı. İntravenöz 0,3 mg /kg Etomidat ve 0,1 mg/kg
Pankuronyum verildi. Grup 1’deki hastalara %2-4 Sevofluran, Grup 2’deki
hastalara %7-8 Desfluran, Grup 3’deki (kontrol grubu) hastalara ise 50-100
µg/kg/dk propofol infüzyon uygulandı. Entübasyonu takiben hastalar mekanik
ventilatöre bağlanarak 0,1-0,3 mikrogr/kg/dk’den Remifentanil infüzyonu başlandı.
Ekstrakorperal dolaşım süresince Grup 1’e % 1-2 Sevoflurane, Grup 2’ye % 2-3
Desfluran, Grup 3’ ise 50 µg/kg/dk propofol verilmeye devam edildi. Cerrahinin
sona ermesi ile anestezik ajanlar kesildi. Yoğun bakım ünitesinde mekanik
ventilatöre bağlanan hastalarda hemodinamik kayıtlar alınmaya devam edildi.
Troponin I, CK ve CKMB düzeyleri her iki grupta da indüksiyon öncesi değere
göre kros klemp sonrası ve postoperatif 2. ve 24. saatte artış gösterdi. Ancak
gruplar arasında anlamlı bir fark bulunamadı. Ayrıca MDA ölçümlerinde her iki
grupda da anlamlı bir artış saptanmadı. Bu sonuçlar kontrol grubumuz olan propofol
grubunda da benzer şekildeydi. Sonuç olarak KPB cerrahisinde kullanılan
inhalasyon anestezik ajanlarından sevofluran ve desfluranın yeterli anestezi düzeyi
ve hemodinamik stabilite sağladığı, her ikisinin de benzer düzeyde kardiyak
koruma yaptığı ayrıca, bir IV anestezik olan propofolün de inhaler ajanlar kadar
kardiyak koruma yaptığı söylenebilir.
The aim of this study is to compare
myocardial protective effects of sevoflurane versus desflurane via markers of
myocardial injury in coronary artery by-pass graft surgery. The study was conducted
on 60 patients scheduled for elective CABG surgery ; who are ASA III-IV, aged
between 18-69 years. 45 of them males, and 15 of them were females. Patients were
randomized into 3 groups; each group contained 20 patients. On the operation table,
standard monitorization was performed; HR, SBP, DBP, MBP, SPO2, CVP. For
anesthetic induction, intravenous etomidate (0,3 mg/kg) and pancuronium (0,1
mg/kg) were administered. For anesthetic maintenance, group 1 received 2-4%
sevoflurane and group 2 received 7-8% desflurane, whereas (control) group 3
received propofol (50-100 µg/kg/minute) infusion. After endotracheal intubation,
patients were connected to mechanical ventilator. Remifentayl (0,1-0,3
µg/kg/minute) infusion was started for all patients. During extra-corporeal
circulation, group 1 received 1-2% sevoflurane and group 2 received 2-3%
desflurane, whereas (control) group 3 received propofol (50 µg/kg/minute) infusion.
At the end of surgery, all anesthetic agents were discontinued. Patients were taken
into Intensive Care Unit and standard hemodynamical measurements were recorded.
With regard to baseline (preinduction) levels, Troponin I, CK and CK-MB levels of
both groups were increased after cross-clamp placement and at postoperative 2nd and
24th hours. However, these increases were not statistically significant. Also, there
was no significant increase of MDA level measurements on both groups. These
findings were in accordance with our control group which received propofol. In
conclusion, both sevoflurane and desflurane provided sufficient anesthesia level as
inhalational anesthetic agents. Both of them provided sufficent hemodynamical
stability. Besides, both of them has similar cardioprotective characteristics. In
addition, it is possible to report that an intravenous anesthetic agent “propofol” had
myocardial protective properties as much as inhalational agents.
