Bu plasebo kontrollü
çalışmadaki amacımız servikal vertigonun tedavisinde transkutanöz elektriksel sinir
stimülasyonu (TENS) uygulamasının etkinliğini araştırmaktı. Servikal vertigo tanısı
konan 34 hasta randomize olarak iki gruba ayrıldı. On yedi hastadan oluşan birinci
gruba aktif TENS ve egzersiz programı, ikinci gruba ise plasebo TENS ve egzersiz
programı uygulandı. Tüm tedaviler toplam 15 gün olmak üzere haftanın 5 günü 20
dakika süreyle günde bir kez uygulandı. Klinik değerlendirmeler tedavi öncesi,
tedavi sonrası ve 3. ayda gerçekleştirildi. Vizüel analog skala, Vertigo Şiddet Skalası,
Vertigo Sıklık Skalası, Verbal Global İyileşme Skalası, Dizziness Handicap
Inventory, Activities Spesific Balance Confidence Skala ve boyun eklem hareket
açıklığı ölçümleri kullanıldı. Ayrıca posturografik analiz yapılarak Düşme Riski
İndeksi skorları hesaplandı. Her iki grupta da tüm parametrelerde grup içi
değerlendirmelerde anlamlı düzelmeler saptandı. Gruplar birbiriyle
karşılaştırıldığında aktif TENS grubundaki hastalar boyun eklem hareket açıklığı
ölçümleri hariç diğer tüm parametrelerde plasebo grubundaki hastalara göre daha
belirgin gelişmeler gösterdi. Bu çalışmanın sonuçları servikal vertigo tedavisinde
egzersiz programına, TENS uygulamasının eklenmesinin etkili bir tedavi modalitesi
olduğunu göstermektedir.
Our
aim was to investigate the efficacy of transcutaneous electrical nerve stimulation
(TENS) application in the treatment of cervical vertigo in this placebo-controlled
study. Thirty-four patients diagnosed with cervical vertigo were randomly assigned
into two groups. First group, which consisted of 17 patients were treated with active
TENS application and exercise program and the second group were treated with
placebo TENS application and exercise program. Both treatments were applied for
20 minutes per session, once a day, five days a week, and a total duration of 15 days.
Assesments were performed at baseline, at the end of the treatment and 3rd month.
For the clinical assessments VAS score, Vertigo Severity Scale, Vertigo İntensity
Scale, Verbal Global Scale, Dizziness Handicap Inventory, Activities Spesific
Balance Confidence scale, and cervical range of motion (ROM) measurement were
used. In addition, fall index scores were calculated by using posturographic analysis.
In intra-group evaluations, statistically significant improvements were observed for
all parameters in both groups. In comparing the groups with each other, patients in
active TENS group showed greater improvements in all parameters except cervical
ROM measurements from those of patients in the placebo group. In conclusion, the
results of this study suggest that adding TENS application to exercise program in the
treatment of cervical vertigo is an effective treatment modality.
