Çalışmamızda sıçanlarda monokrotalin ile oluşturulan pulmoner hipertansiyon
modelinde sildenafile eklenen everolimus kombinasyonlarının hemodinamik ve
morfolojik değerlendirmeler sonucunda ne ölçüde etkili olduğu araştırılmıştır. Sağ
ventrikül basınçları, sağ ventrikül hipertrofisi ve pulmoner arterlerde miyografta
vazokonstriktör ve vazodilatör ajanlara alınan yanıtlar yanında histopatolojik değişimler
gruplar arasında karşılaştırılmıştır. Sağ ventrikül basınçları ve sağ ventrikül/kalp ağırlık
oranları tedavisiz monokrotalin gruplarında kontrole oranla anlamlı şekilde artarken
tedavili gruplarda bu artış azalarak kontrolden farksız değerlere ulaşmıştır. Everolimus
eklenen gruplarda sildenafil tedavisine oranla bu parametrelerde anlamlı fark
gözlenmemiştir. Pulmoner arterlerde potasyum klorür, endotelin-1 ve noradrenalin
kasılma yanıtları tedavisiz monokrotalin gruplarında kontrole oranla azalırken serotonin
kasılma yanıtları artmıştır. Tedavili gruplarda özellikle de everolimuslu gruplarda
azalan kasılma yanıtlarının kontrolden farksız değerlere ulaştığı, serotonin yanıtlarının
ise tedavisiz monokrotalin gruplarında olduğu gibi arttığı görülmüştür. Asetilkolin ve
sodyum nitroprussid gevşeme yanıtlarının tedavisiz monokrotalin gruplarında azaldığı
tedavili gruplarda özellikle de everolimuslu gruplarda belirgin şekilde düzeldiği
saptanmıştır. Histopatolojik incelemelerde monokrotalin gruplarında meydana gelen
vasküler ve kardiyak hipertrofinin tedavi ile düzeldiği görülmüştür. Özellikle de
everolimuslu tedavi gruplarında bu düzelmeler inflamatuar değişiklikleri de içine alacak
şekilde daha ön plana çıkmıştır. Tüm bu sonuçlar değerlendirildiğinde monokrotalin ile
indüklenen pulmoner hipertansiyon modelinde sildenafil+everolimus kombinasyonunun
kardiyovasküler toksik etki oluşturmaksızın remodeling sürecinde salt sildenafil
tedavisine oranla daha etkili olduğu söylenebilir.
In our study, we investigated the efficacy of everolimus combination treatment
with sildenafil on hemodynamical and morphological parameters in the monocrotaline
induced pulmonary hypertensive rats. Right ventricular pressure, right ventricular
hypertrophy and the response to vasocontrictor and vasodilator agents in pulmonary
arteries by myograph and also the histopathological changes were compared between
the groups. Right ventricular pressures and right ventricule/heart weight ratios were
increased in non-treated monocrotaline groups versus the controls, these increased ratios
were decreased in the treated groups and reached to similar values as controls. There is
no significant difference between the everolimus added groups and sildenafil groups.
The contractile responses to noradrenaline, potassium chloride and endothelin-1 were
decreased in the non-treated monocrotaline groups versus controls; in contrast the
contraction responses to serotonin were increased in pulmonary arteries. Reduced
contractile responses were reached to control values and the contractile responses to
serotonin were increased as non-treated monocrotaline groups in the treatment groups
especially in the everolimus groups. Responses to acetylcholine and sodium
nitroprusside relaxation were decreased in non-treated monocrotaline groups and
improved significantly in the everolimus groups. At the histopatological examinations,
vascular hypertrophy and cardiac hypertrophy occured in monocrotaline groups was
improved by the treatment. Particularly these improvements became remarkably,
including the inflamatory changes in the everolimus treatment groups. In the light of
these results sildenafil+everolimus combinations were more efficient than sildenafil
treatment alone in the remodeling process without any cardiovascular toxic effects in
the monocrotaline induced pulmonary hypertension.
