Koroner arter by-pass cerrahisi
kardiyopulmoner by-passın uygulamaya baĢlaması ile rutin ve güvenli bir
prosedür olarak uygulanmakta ve elektif Ģartlarda mortalite %2-3 olarak
belirtilmektedir. Kardiyak stabilizatörlerin geliĢmesi ile off pump koroner arter
by-pass uygulamasında artıĢ bulunmaktadır. Ancak koroner by-passların
sadece %18‟inin oluĢturmaktadır. Prospektif çalıĢmamızda randomize olarak
15 on pump ve 15 off pump koroner arter by-pass uygulanan 30 hasta dahil
edildi. Hastaların böbrek fonksiyonlarının değerlendirilmesi için preoperatif ve
postopertif ilk 24 saatteki üre, kreatinin değerleri, karaciğer fonksiyonlarının
değerlendirilmesi için ALT, AST değerleri ve miyokardiyal hasarın
değerlendirilmesi için sternotomi sonrası, on pump grubunda kros klemp
kaldırıldıktan hemen sonra, off pump grubunda son distal anastomoz
yapıldıktan sonra (arter oklüzyonu ile oluĢturulan iskemi sonrası) ve her iki
grupta postoperatif 24. saatte kan alınarak, serumları -70 ºC de saklandı.
Elisa yöntemi ile FABP (fatty acid binding protein) değerleri ölçüldü.
Postoperatif bulgular değerlerlendirildiğinde off pump grubta on pump CABG
grubuna göre karaciğer ve böbrek fonksiyon testlerinin daha az etkilendiği,
KPB‟nin önemli derecede bu fonksiyonları etkilediği saptandı. Myokardiyal
iskemi açısından değerlendirildiğinde off pump grupta FABP değerlerinin on
pump gruba göre iskemik süreç sonrası anlamlı derecede az etkilendiği
belirlendi. Tüm bu bulgular ıĢığında kardiyo pulmoner by-pass‟ın vücuttaki
tüm sistemleri etkilediği göz önüne alınırsa; her ne kadar off pump CABG de
atan kalpte çalıĢılması nedeniyle görüĢ ve manipulasyon sınırlı olması,
anastomoz kalitesinin düĢük olması gibi dezavantajları bulunmasına rağmen
stabilazatörlerin yardımı ve cerrahın deneyimi ile uygun olgularda KPB‟de
olduğu gibi tam revaskülarizasyonun sağlanabileceğini düĢünmekteyiz.
Coronary artery by-pass
grafting (CABG) with the aid of cardiopulmonary by-pass (CPB) is a routine
and safe procedure, associated with a mortality rate of 2% to 3% in elective
cases. The introduction of better stabilizing systems, such as the Octopus
and the Starfish, has revived interest in performing multivessel CABG on the
beating heart. Altough offpump coronary artery by-pass surgery has some
advantages over on pump CABG, It‟s percentage is about 18. 30 adult
patients of either sex who were scheduled to undergo CABG at our institution
were distributed to 1 of 2 groups: on-pump (n=15), in which coronary
revascularization was done with the use of CPB and moderate hypothermia;
and the other group was off pump (n=15) While kidney functions of patients
were evaluated with ure and creatinin value, liver function was evaluated with
ALT and AST measurements in preoperative and postoperative blood
samples. FABP (Fatty acid binding protein) was measured for evaluation of
myocardial ischemia in blood samples taken after sternotomi in both of
groups, after taking of cross klamp in on pump group, and after termination of
last distal anastomosis (after ischemia occured by occlusion of artery) in off
pump group and after 24 hours in both groups. In evaluation of postoperative
values, kidney and hepatic functional tests are less changen in off pump
CABG group regarding to other group. In aspect of myocardial ischemia,
FABP levels were lower in off pump group meaningfully after ischemic
period. CPB is tought to affect whole of the systems in body, despite the
disadvantages of off pump CABG as lower manipulation ability and vision
regarding to other group; in suitable cases with the aim of special
stabilizaters and expertise of surgeon, revascularization can be ensured in
coronary artery diseases.
