Bu çalışma, 07.01.2004-16.01.2011 tarihleri arasında tek
taraflı işitme kaybı (USNİK) şikayeti ile polikliniğimize başvuran 18 yaş altı 33
hasta ( 14 erkek, 19 kadın, ortalama yaş 12,2 ± 3,68 ) ile yapıldı. Kronik otit veya
bilateral işitme kaybı olan hastalar çalışma dışı bırakıldı. İşitmesi normal olan 25
sağlıklı gönüllü (13 erkek, 12 kadın, ortalama yaş 12,4 ± 4,21 ) ile kontrol grubu
oluşturuldu. Çalışma grubunun tümünde çok ileri derecede işitme kaybı mevcuttu.
Sensörinöral işitme kaybının etyolojisi oldukça geniş olduğundan bu çocuklarda
etyolojiyi aydınlatmaya yönelik geniş kan tetkikleri, genetik inceleme ve yüksek
çözünürlüklü temporal kemik tomografisi ve temporal kemik magnetik rezonans
görüntüleme yapıldı. Yapılan tetkikler sonucunda % 66 hastada kabakulak IgG
pozitifliği saptandı. Ailelerden alınan anamnez sonucunda 6 çocuğun işitme kaybının
kabakulak parotiti sonrası geliştiği öğrenildi.Kan değerleri normal saptandı ve
genetik inceleme doğaldı. Tek taraflı işitme kayıplı hastaların iç kulak denge
fonksiyonlarını değerlendirmek amaçlı tüm hastalara ayrıntılı nörootolojik muayene,
Elektronistagmografi (ENG) ve Vestibüler Uyarılmış Kas Potansiyelleri (VEMP)
testi yapıldı. Kontrol grubuna aynı değerlendirmeler yapıldı. Çalışma grubunda 33
hastadan 20’sinde dengesizlik ve baş dönmesi şikayetleri mevcuttu. Çalışma
grubunda işitme kaybı olan tarafta kalorik testte kanal parezi oranı % 68 bulundu.
Bu çocuklara yapılan testler sonrasında ayırıcı tanı yapılarak uygun medikal tedavi
başlandı. Görüntüleme yöntemleri sonrasında bu çocukların 1’inde vestibülokoklear
anomali, 16’sında vestibül veya semisirküler kanal anomalisi tespit edildi.
This study was performed with 33 patients
aged under 18 (14 male, average age 12,2 ± 3,68) who admitted to our out-patient
department with the complaint of unilateral hearing loss between the dates
07.01.2004 and 16.01.2011. The patients with bilateral hearing loss and chronic otitis
media were excluded from the study. The control group was consisted of 25 healthy
volunteers (13 males, average age was 12.4 ± 4,21). In the study group all the
patients had profound hearing loss. As the etiology of sensorineural hearing loss has
a wide spectrum, we performed detailed examination, blood test evaluations, DNA
mutation analysis as well as imaging techniques including high resolution computer
tomography scans and Magnetic Resonance Imaging of the temporal bone.As a result
we obtained IgG pozitiviness for mumps in % 66 in the study group. At the history of
these patients 6 children had sensorineural hearing loss immediately after mumps
infection. Hemogram and biochemistry values were normal. Genetic analysis were
normal. Detailed neuro-otological examinations and vestibular tests including
electronistagmography (ENG) and Vestibular evoked myogenic potentials (VEMP)
were performed to both study and control groups. Dizziness and vertigo were
observed in 20 patients out of the 33. In the study group, we found % 68 caloric test
weakness on the side of the hearing loss. After the investigation procedures, we
started the medical treatment to these children. We found severe vestibulocochlear
anomaly in one of the patients. Different vestibular and semisircular canal anomaly
were found in 16 patients.
