Endometriyum kanseri; endüstrileşmiş ülkelerde kadın genital
sisteminin en sık rastlanan kanseridir. Bu hastaların çoğu düşük evrelerde yakalanır
ama halen bu hastalığa bağlı mortalite önemlidir. Hastaların % 75’i sınırlı hastalıkla
karşımıza çıkar, bu grupta tedavi cerrahidir. Hastaların üçte birinde tekrarlayan ya da
metastatik hastalık nedeniyle sistemik kemoterapiye ihtiyaç vardır. Vasküler
endotelyal büyüme faktörü (VEGF) , endometriyum kanserinde tümör büyümesinde
önemli bir düzenleyici rolü olan glikoproteindir. Preklinik çalışmalarda, VEGF’e
karşı geliştirilmiş monoklonal antikorlardan olan Bevacizumab birçok nonjinekolojik
insan solid tümörlerinde umut veren etkiye sahiptir. Tez kapsamında sisplatin,
doksorubisin, bevacizumab gibi maddelerin bazı farklı kombinasyonlarının sitotoksik
ve apoptotik etkilerini saptamak için in vitro testler kullanılmıştır. Bu amaçla,
Ishikawa hücre kültürü kullanılarak sitotoksik etkiyi belirlemek için Tetrozolium
Testi ( MTT Testi), apoptosizi saptamak için ise DAPI boyama ve kaspaz–3 testi
kullanılmıştır. Bu bilgiler ışığında amacımız antianjiojenik faktörün sık görülen bir
jinekolojik kanserde klasik kemoterapilerle kombine ve tek başına kullanımının
etkilerini araştırmak ve klinik pratiğe yansıtmaktır. Ishikawa hücreleriyle yapılan in
vitro sitotoksisite deneyleri sonucunda üçlü ilaç kombinasyonu tekli ve ikili ilaç
kombinasyonlarına göre daha yüksek bir inhibisyon göstermiştir. DAPI boyama ve
Kaspaz 3 aktivite test sonuçları, üçlü ilaç kombinasyonunun diğer iki uygulamaya
göre daha fazla apoptotik etki meydana getirdiğini göstermektedir. Bu sonuçlar çoklu
ilaç kombinasyonunun ümit verici olduğunu düşündürmektedir.
Endometrial
carcinoma is the most common malignancy of the female genital tract in
industrialized countries. Although most endometrial carcinomas are detected at low
stage, there is still a significant mortality from the disease. About 75 % of the
patients present limited disease, confined to the uterus that can be cured by surgery.
However, one third of the patients will need systemic treatment because of metastatic
or relapsing disease. Vascular endothelial growth factor (VEGF) is an important
regulator of angiogenesis in endometrial cancer growth. Preclinical studies have
shown that a humanized variant of monoclonal antibody against VEGF,
Bevacizumab, has shown promising activity in many non-gynecologic human solid
tumors. Invitro tests have been used to determine the cytotoxic and apoptotic effects
of agents which are the main topic of our study (cisplatin, doksorubisin,
bevacizumab). Cytotoxic effect was determined by the standard MTT assay and
apoptosis by DAPI staining, caspase-3 in Ischikawa cell culture. Based on these
informations, the aim of our study is to determine the effect of a antiangiogenic agent
Bevacizumab with or without classic chemotherapeutics which used in one of the
most common gynecologic malignancy and use these informations in our clinical
practice. In vitro cytotoxicity and apoptosis tests against Ishikawa cells indicated that
the triad drug combination showed a higher inhibition than a single and dual drug
combination, which holds a promise for multidrug combination.
