. Bu çalışma, egzersiz tedavisi ile karşılaştırmalı olarak diz OA’ inde oral GS tedavisinin
ağrı, fonksiyonel durum, kıkırdak dejenerasyonu ile ilişkili serumda bulunan Nitrik Oksit ( NO
), Malondialdehid (MDA), İnterlökin-1 Beta ( İL-1 Beta ), Matriks Metalloproteinaz-3 ( MMP-3
) düzeylerine ve MR ile değerlendirilen kartilaj kalınlığı üzerindeki etkilerini değerlendirmek
amacıyla yapılmıştır. Diz OA tanısı olan 70 hasta, randomize olarak iki gruba ayrıldı. Kırk
kişiden oluşan birinci gruba 1500 mg/gün oral glukozamin sülfat (GS) tedavisi, 30 kişiden
oluşan ikinci gruba ise ev egzersiz programı 6 ay boyunca uygulandı. Hastaların tedavi öncesi
ve sonrası VAS (istirahat, aktivasyon, yürüme ), WOMAC (ağrı, tutukluk, total ), 20 m yürüme
süresi, serum inflamatuar mediyatör düzeyleri (İL-1 Beta, NO, MDA, MMP-3 ) ve MRG ile diz
eklemi kartilaj kalınlığı sonuçları değerlendirildi. Her iki grupta VAS (istirahat, aktivasyon,
yürüme ), WOMAC (ağrı, tutukluk, total ), 20 metre yürüme süresi parametrelerinde tedavi
sonrasında anlamlı düzelme izlenirken (p<0.001), gruplararası karşılaştırmada ise iki tedavinin
birbirine üstünlüğü bulunmadı. Serum inflamatuar mediyatörlerinden sadece MDA
düzeylerinde her iki hasta grubunda anlamlı düzeyde düşme saptandı (grup:1, p<0.05; grup:2,
p<0.001) ve gruplar karşılaştırıldığında bu fark grup 2 lehine olmak üzere daha anlamlı
düzeyde idi (p<0.05). Eklem kartilaj kalınlığının MRG ile değerlendirilmesi sonucunda ise
tedavi sonrası ev egzersiz programı verilen grupta sadece medial femoral kondil alanında
iyileşme saptandı (p<0.001). Çalışmamızın sonuçları, oral GS ve egzersiz tedavisinin diz OA’ li
hastaların semptomatik tedavisinde etkili olduğunu göstermiştir. Ancak kondroprotektif etkiler
göz önüne alındığında, egzersiz tedavisinin daha üstün olduğu sonucuna varılmıştır.
This study was carried out to evaluate the effects of oral GS treatment for
knee OA on pain, functional status, NO, MDA, IL-1, MMP-3 levels in the serum with regard to
cartilage degeneration and cartilage thickness, which is evaluated by MR. 70 patients diagnosed
with knee OA were randomly separated into two groups. First group, which consisted of forty
persons, were treated with 1500 mg/day oral glucosamine sulphate and the second group, which
consisted of thirty persons, were applied a home exercise programme for a period of 6 months.
Patients’ pre-treatment and post-treatment VAS (rest, activation, walking) WOMAC (pain,
dysfunction, total), 20 m walking time, serum inflammatory mediator levels (IL-1 Beta, NO,
MDA, MMP-3), MRI and knee cartilage thickness results were evaluated. For both groups,
significant post-treatment improvement was observed for the parameters of VAS (rest,
activation, walking) WOMAC (pain, dysfunction, total) and 20 m walking time (p<0.001) and
there was no superiority of one of the treatments over the other in the comparison between
groups. Of the serum inflammatory mediators, a significant decrease was determined for both
groups only in the MDA levels (group 1, p<0.05; group 2, p<0.01) and when the groups were
compared, such difference was more significant for group 2 (p<0.05). Upon the evaluation of
joint cartilage thickness by MRI, an improvement only in the area of medial femoral condyle
was determined for the group which was given a post-treatment home exercise programme
(p<0.01). The results of our study showed that oral GS and exercise treatments were efficient in
the symptomatic treatment of patients with knee OA. However, as the chondroprotective effects
are taken into consideration, it is determined that exercise treatment is more efficient.
