Bu çalışmada; aspirin (ASA) duyarlı olan astımlılarda kanda CD4+ T
lenfositlerden Th1 tipi sitokinleri temsil eden IFN-γ ve IL-2 ile Th2 tipi sitokinleri temsil eden
IL-4 sunumunun, lenfosit altgruplar›n›n ve lenfosit apoptozunun bir ayl›k desensitizasyon
tedavisi sonrası değişikliklerinin gözlenmesi amaçlanmıştır. Daha önce nazal polip ve/veya
sinüs cerrahisi geçirmiş, rinit, astım bulguları olan asetil salisilik asit (ASA) ve/veya non steroidal antiinflamatuvar (NSAİİ) ile solunumsal yakınmaları olan, 12 ASA’e duyarlı astımlı
(ADA) hasta çalışmaya alındı. 15 sağlıklı kişi ve 12 ASA’e duyarlı olmayan astımlı hasta
kontrol gruplarını oluşturdu. Desensitizasyondan 48 saat önce tüm hastalar›m›z
değerlendirildi. Solunum fonksiyon testi (SFT), ast›m kontrol testi (AKT), rinomanometrik
ölçümleri ve koku skor düzeyi saptand›. Burun sürüntü örneği alınan hastalarımızdan lenfosit
apoptozisi ve IFN-γ, IL-2, IL-4 sal›n›m› yapan CD4+ T lenfositlerin yüzdesine, lenfosit alt
grupları ve lenfosit apoptozis oranına bakılmak üzere kan örneği alındı. Desensitizasyon
işleminin ilk günü plasebo ile teste başlanıldı. Bu sırada hiçbir hastamızda yakınma oluşmadı.
2. gün 3’ er saat aralar ile 25 mg, 50 mg, 75 mg ve 100 mg toplam 250 mg ASA uyguland›. 3.
gün 150 mg ve 300 mg, toplam 450 mg asetilsalisilik asit (ASA) uyguland›. 4. gün 600 mg
2x1 şeklinde uyguland›. 1. ayl›k kontrolda tetkikler tekrar edildi. Desensitizasyon öncesi IFN γ oran› normal-kontroldan düşük olarak saptanmıştır (p=0.04). ADA’ lı grupta
desensitizasyon sonras› IFN-γ oran› normal-kontroldan düşük olarak saptanmıştır (p=0.036).
ADA’ lı grupta desensitizasyon sonrası CD19 değeri, normal-kontroldan düşük olarak
saptanmıştır (p=0.047). Guplar arasında lenfosit subtipleri, lenfosit apoptozisi ve diğer
sitokinlerin salınımı bakımından fark saptanmamıştır.
The present study aimed to assess changes in the expression of IFN-γ
and IL-2 as Th1 type cytokines and IL-4 as Th2 type cytokine from CD4+ T lymphocytes,
lymphocyte subgroups and lymphocyte apoptosis in peripheral blood from ASA sensitive
asthmatics after one month of ASA desensitization period. 12 ASA sensitive asthma patients
with previous history of nasal polyp and/or sinus surgery, rhinitis, asthma findings and asetil
salisilik asit (ASA) and/or NSAID activated respiratory problems were included in the study
(ADA). 15 healty people and 12 ASA non sensitive asthma patients were set the control group
and single blood samples were obtained to determine the lymphocyte subgroups and
apoptosis, intracellular cytokine ratio of CD4+ lymphocytes. The blood samples were taken
from the patients from whom the nasal smear was also obtained in order to detect intracellular
cytokine ratio of CD4+ lymphocytes, subgroups, and also apoptosis level. The test was begun
at the first day of desensitization. There was no any complain for the patients during this
time. At the second day 25 mg, 50 mg, 75 mg, and 100 mg ASA was applied with 3 hours
interval. At the third day 150 mg and 300 mg ASA was applied. At the fourth day 2x600 mg
ASA was applied. Peripheral blood from ASA sensitive asthmatics after one month of ASA
desensitization period. The expression of IFN-γ from CD4+ T lymphocytes was decreased in
ASA sensitive asthmatics than those normal-controls both before (p=0.04) and after
desensitization period (p=0.036). CD19 lymphocytes were decreased in ASA sensitive
asthmatics than those normal-controls after desensitization (p=0.047). There was no change in
respect to expression of other cytokines, lymphocyte apoptosis and lymphocyte subtypes
between groups.
